    A 33-year-old man complained of positional vertigo every time he rolled to either side of his body for 5 months. He exhibited a direction changing geotropic positional nystagmus with paroxysmal vertigo. The positional nystagmus and vertigo were the most intense on the right lateral position. This positional nystagmus had a shorter latency, a longer duration and no fatiguability in comparison to the posterior canal benign paroxysmal positional nystagmus (PC-BPPN). Because these features of positional nystagmus corresponded to that of horizontal canal-BPPN (HC-BPPN) already reported, he was diagnosed to have right horizontal canal benign paroxysmal positional vertigo (HC-BPPV). Since the HC-BPPV attack period has been reported to last for about 2 or 3 months at most, this case was considered to be an unusually prolonged one. He was treated by “Lempert's positional maneuver” on two occasions but the positional nystagmus and vertigo remained. However, he recovered soon after being treated once by the “Prolonged Position of Vannucchi”.

    To understand better the effect of the cervical prorioceptors in the vestibro-ocular reflex, experiments were carried out using rabbits. Rabbits were placed on the rotatory board, in the posture: the head is in normal position and the trunk is rotated 15-90 degrees counter clockwise on the horizontal plane. When the board was rotated to right, the slow eye movement induced by vestibro-ocular reflex was enhanced. On the other hand the board was rotated to left, the slow eye movement was inhibited. The effects were appeared in the postur e ; the angle between head and trunk was only 15 degrees, and the effects were enhanced with increase of the angle. The effects were vanished when we cut the dorsal roots from Cl to C4 on the strained side. It seems reasonable to conclude from these results, the effect of the tonic neck reflex in the vestibro-ocular reflex is occurred by strained side cervical prorioceptors at the level from Cl to C4.

    Facial nerve neurinoma demonstrates a variety of signs and symptoms. Otherwise without in mind of the entity, this possibly mislead us to the correct diagnosis. Three of such cases were presented. Case 1 was a 34-year-old man without any history of facial palsy. He just complained of earache and had conductive hearing loss. Neurinoma arose from the geniculate ganglion through the mastoid segment of the facial nerve. Case 2 was a 56-year-old woman with complete facial nerve paralysis which has occurred 14 years before. She had no ear symptom like hearing loss. The tumor was localized to the geniculate ganglion. Case 3 was a 72-year-old woman showing incomplete facial palsy accompanied with synkinesis. She had ipsilateral sensorineural hearing loss and vestibular malfunction as well. Neurinoma was located in the internal acoustic canal through the geniculate ganglion. Surgical resection was completely achieved and facial nerve was reconstructed by the facio-hypoglossal anastomosis in case 1. Total decompression was performed in case 3. In the case of facial nerve paralysis and/or unilateral hearing loss, facial nerve neurinoma should always be differentiated.

    Follow-up study after tonsillectomy was performed in 89 patients that was suspected to tonsillar focal infection with skin disease in 15 years until 1997 from 1982 and that was 34 of male, 55 of female (pustulosis palmaris et plantaris: 62 cases, erythema exudativum multiforme: 5 cases, psoriasis: 4 cases, erythema nodosum: 4 cases, a rush: 3 cases, others: 11 cases). Patients who were cured or showed recovery accounted for 85.7% after tonsillectomy. It is thought that in general tonsillectomy was effective to focal infected tonsillitis, but long term observation was necessary. The relationship between postoperative prognosis and examination parameter were observed. ASO, ASK and RA were not related to the postoperative prognosis. It is necessary to find a more reliable method for diagnosis of the tonsillar focal infection.

    From 1988 through 1997, in department of head and neck surgery, Gunma Cancer Center, 110 patients with tongue carcinoma, 108 patients with laryngeal carcinoma and 47 patients with hypopharyngeal carcinoma were operated as initial treatment. In these cases, we analyze the recurrent pattern and secondary treatment of recurrent cases. Advanced cases are recurred, then, the treatment is difficult especially recurrent hypopharyngeal carcinoma. In any case, preoperative diagnosis is important. The recurrent tongue carcinoma can be currable, if it can be done operation.

    In addition to aspirin and non-steroid antiinflammatory drugs (NSAIDs), aspirin-induced asthma (AIA) can also be provoked by various kinds of foods and additives. In this study, we identified what kind of foods and additives induced attacks of AIA. Six out of 9 AIA patients had a history of asthma and/or rhinitis attacks induced by foods and/or additives. However, most of the symptoms induced by these agents were mild. The causal substances were divided into 5 groups, including; 1) toothpaste, 2) yellow pickled radishes and pickled and vegetables. 3) canned drinks, 4) curry and spices, and 5) fruits and vegetables. Most of these agents contained preservations and/or coloring matter that were thought to be causal agents of AIA. Based on these results, it is clear that AIA patients must be thoroughly trained to avoid these agents in order to prevent a recurrence of symptoms.

    Forty cases with sino-nasal malignant lymphoma who were treated at the Department of Otorhinolaryngology, University of the Ryukyus, Faculty of Medicine from 1974 to 1996 are herein reviewed. The staging of these cases was made based on the Ann Arbor classification system and pathologically according to the Working Formulation (WF). In addition, the lesions showed polymorphism regarding the size of the tumor cells which could invade in an angiocentric pattern with various types of inflammatory cells and necrosis of the surrounding tissue was classified as polymorphic reticulosis (PR). Regarding the immunological phenotype, immunostaining was performed with MB-1, L-26 for B cell type and MT-1, UCHL-1 for T cell type. The survival rate was calculated based on the Kaplan Meier method. The results were as follows; 1) The immunophenotypes were T cell type in 63%, including 8 cases of NK cell type and B cell in 29%. 2) Seventy-five percent of all cases were classified as an intermediate stage based on the Working Formulation. 3) The 5-year survival rates for all cases, consisting of T cell type cases, B cell type cases and PR type cases were 49%, 42%, 50% and 54%, respectively. Chemotherapy, includimg CHOP demonstrated good therapeutic results, but the cases with lymphoma in the high grade group could not be controlled by ordinary therapeutic regimens. A histology-specific treatment regimen for high grade lymphoma is thus needed.

    An investigation of the clinical isolates from patients with acute sinusitis was undertaken in our hospital by an antral puncture and irrigation. A total of 306 strains were isolated and identified from 197 patients between January 1993 and December 1997. The detection rates of bacteria, more than one bacteria and anaerobic bacteria were 90.7%, 40.8%, and 19.4%, respectively. The main organisms isolated were Streptococcus pneumoniae, Peptostreptococcus spp., Flavobacterium spp., Haemophilus influenzae and Streptococcus α-hemolytic. The results of this study should serve as a reference for the selection of antibacterial agents for the empirical therapy of acute sinusitis. After performing an antral puncture and irrigation 5 times or less, about 80% of the patients showed a marked improvement, and thus the efficacy of this procedure was reconfirmed.

    In a certain difficult case, or when intra-operative histology is not possible, it is prudent to delay repair until the definitive pathological examination has been completed. However, delayed operation method increases the risk of infection and be heavily polluted by tissue fluid leakage when repair is finally undertaken. The authors report on their use of a heterogeneity, artificial dermis, composed of a 5mm and/or 3mm thick aterocollagen sponge and covered with a 60 to a 100μm thick silicone membrane, in the delayed primary repair of head and neck reconstruction. This artificial dermis is found to be easy to handle, adhere well and promote hemostat. This artificial dermis effectively served as a biological dressing and caused no inflammatory response and reduced pain and tissue fluid leakage.

    Over the last 13 years, we performed maxillary antral irrigation using Killian's cannula through the middle nasal meatus in over 2, 600 cases of sinusitis in children and excellent results were thus achieved. Maxillary antral irrigation for sinusitis in children is considered to be an appropriate treatment for the following cases: 1) Cases that are intractable to preservative treatment 2) Sinusitis cases with complications, including; a. Acute sinusitis accompanied by headache or fever, in which drug therapy is not effective. b. So-called “sino bronchitis” and its subforms including cases accompanied by continuous coughing, sputum and nasal discharge. c. Cases accompanied by intractable exudative otitis media. d. Cases accompanied by intractable bronchial asthma with frequent attacks.
