Since 1991 at Kurume University Hospital, patients with carcinoma of the maxillary sinus have undergone multidisciplinary therapy performed in a team approach by otolaryngologists and radiologist. In this paper, this treatment modality employed after 1991 is described in detal and the treatment outcome of such patients are compared with those before 1990. The detail of recent approach are as follow: Specialists in otolaryngology and radiology initially discuss and determine the field and dose of irradiation before initiating treatments. During the irradiation treatment, otolaryngoligists and radiologists examine the patients together and the treatment plan is flexibly readjusted. Before the application of this multidisciplinary therapy, 111 patients were treated, whereas 34 patients have so far undergone this multidisciplinary therapy. The five-year local control rate and cause-specific survival rate for patients before multidisciplinary therapy were 67.2% and 56.6%, respectively. On the other hand, after multidisciplinary therapy they were 63.5% and 58.6%, respectively. Regarding the T categories, the five-year local control rate and cause-specific survival rate of T4 were 47.3% and 31.3% before this approach, while after multidisciplinary therapy they were 66.5% and 51.6%, respectively. These results suggested that more advanced debulking of the invading tumor appears to be necessary.
