    Tinnitus is a hallucinatory sound which is felt in the ear or in the intracranial area. It occurs regardless of the presence or the absence of the source of the sound exterior to the body. It is thought to appear with the disorder of the inner ear, including the sensorineural hearing loss. However, there are several cases for which there is no obvious abnormality of the ear function. These pathoses have not been completely proven. We thought that a functional check of the brain is useful in cases of tinnitus, who demonstrate no abnormal organic findings. Therefore, we examined these cases, which did not have a recognized worsening of auditory acuity and laterality, as assessed by Magnetencelography. With respect to the place in which the dipole occurred upon auditory stimulation, both examples were normal. A shortening of the latency which was induced by the maximum amplitude appearance via auditory stimulation in the magnetic field, was found in two cases. A significant difference was not seen in the tinnitus group versus the mutual intervals of the non-tinnitus group with respect to latency. We examined the association of the latency between the frequency with the audibility threshold, and THI (tinnitus handicap intency) score and the quality of tinnitus expected by the pitch-match test, but these results also did not show any significant differences. Therefore, we thought that the abnormality of the audition perception disposal in the nucleus level or the sound which should not have been heard might occur in tinnitus side. Since tinnitus is a sensation in the whole highly advanced nucleus, it is thought that our approach of a functional check as magnetoencephalogram was useful in the elucidation of pathology of this problem.

    Ethmoidal sinus and sphenoid sinus cysts sometimes occur with ophthalmic symptoms, and they normally requir emergency surgery. A clinical study was conducted on 33 patients with the cyst in the ethmoidal sinus and sphenoid sinus at Kyushu University Hospital between 2000 and 2008. A total of 22 of these patients had postoperative cysts. Of the 33 patients, 24 had ophthalmic symptoms, and all of them were treated surgically. Their symptoms were disturbance of vision (12), diplopia (6), and protrusion of eyeball (6). A total of 7 cases of the disturbance of vision were recovered after the surgery. With respect to the diplopia, 4 of 7 cases were recovered. All cases of the protrusion of eyeball were recovered. It was concluded that an immediate surgical treatment was necessary for the case with a disturbance of vision.

    Bell's palsy has a good outcome and Stennert reported a high cure rate in the treatment of high-dose steroids. However, advanced age is thought to be one of the poor prognostic factors. Therefore, we applied the treatment of high-dose steroids to elderly patients with Bell's palsy, although elderly patients have a higher incidence of adverse effects on drug therapy than younger patients. We compared the treatment results in six extremely elderly patients with Bell's palsy. Four of six patients had complete hemifacial palsy. All patients were cured with high-dose steroids. A short course of treatment did not lead to the development of any side effects. These results suggest that we should treat extremely elderly patients suffering from Bell's palsy aggressively with the short-term treatment of high-dose steroids.

    We performed cochlear implantation for a deaf patient induced by eosinophilic otitis media. The patient has showed constant and good hearing abilities for 2 years after the surgery without any difficulties. Eosinophilic otitis media is characterized by persistent glue like otorrhea, and is known to be an intractable disease. It is rare but sudden, and severe sensorineural hearing loss may develop in eosinophilic otitis media patients, persist for an extended period, which may worsen the quality of life of the patient. A cochlear implant appeared to be an applicable means to recover a verbal communication in the deaf patient with eosinophilic otitis media.

    It is known that most senior citizens experience drying of the oral cavity and a poor state of oral hygiene. This causes upper respiratory infection, such as pneumonia. We herein investigated the state of oral cavity drying of senior citizens for the care of an elderly welfare institution tenant of an institution which was associated with this university. I examined the changes in the dry state of the oral cavity by performing oral cavity care positively. The oral cavity inland waters quantity measurement method used an oral cavity water meter, Mucus and three times of the following was performed : 10 times a month for a total of 6 months ; the oral mucosa (cheeks mucous membrane, a tongue mucous membrane) measurements were performed once in March, 2007. In order to guide the dental hygienist, I performed oral cavity care positively from June, 2007. I recently conducted an investigation into the pneumonia of outbreak number for four years. The oral cavity inland waters quantity of the senior citizen was not much different from that of a physically unimpaired person. The change of the quantity of water was not accepted, even if I performed oral cavity care. I decreased the incidence of pneumonia in patients from 2007, the time that I initiated oral cavity care. Based on the above findings, the utility of the above descrived oral cavity care method for the senior citizen is therefore expected.

    This report described a case of supraorbital abscess observed in a 15-year-old male patient with acute sinusitis. Endoscopic sinus surgery was performed twenty days after the onset of the event, and both diplopia and ocular proptosis improved. The supraorbital abscess was rated as Group 3 according to Chandler's classification ; however, surgical treatment was not performed because the patient was hospitalized in this hospital while visiting his parents' hometown, and his family members requested that the patient undergo surgical treatment after returning to his own home. However, endoscopic sinus surgery is indicated for an abscess localized in the upper part of the orbit, which was seen in this patient. The prognosis is dependent on the presence or absence of a reduced visual acuity, the presence of which will prompt the physicians to perform surgical treatment as early as possible.
