    The purpose of this study was to evaluate the clinicopathological effects of neoadjuvant chemotherapy (NAC) for lymph node metastasis in patients with locally advanced hypopharyngeal cancer. Methods:① The clinical response was compared between the NAC and non-NAC groups. ② The relationship between the clinical response and the pathological response was evaluated. ③ The relationship between the size of the histopathologically NAC effective (fibrotic change) area and the prognosis was examined. ④ Large cross-section specimens were obtained from nine patients, and the detailed findings of lymph node metastasis were examined. Results:① The survival and distant metastasis control rates in the NAC group were superior to those observed in the non-NAC group. ② The clinical and pathological responses were generally consistent. ③ The 5-year survival rate was 100% in the patients whose examined tissues contained more than 50% fibrosis and 43% in the patients who tissues contained less than 50% fibrosis. These results indicate that patients who exhibit a good response to NAC achieve a better prognosis. ④In some specimens, histopathological invasion of cancer cells was often observed in the extracapsular adipose and vascular tissue. These findings suggest that both the clinical and pathological responses to NAC are indications of a better prognosis. Examining large cross-section specimens and quantifying the percentage of fibrosis in the extracted tissues is a possible histopathological method for identifying patients with a better prognosis.

    In recent years, several investigations into patient satisfaction regarding the treatment for Japanese cedar pollinosis have been reported. Making use of a questionnaire which allows patients to choose from two options of antihistamines has led to an improvement in patient satisfaction, thus suggesting that patients desire more detailed information about prescribed drugs. We developed a patient-centered medical questionnaire which leads patients through a self-assessment of their pollinosis and self-choice regarding drugs, and attempted to use it with Japanese cedar pollinosis patients in 2011. Using the questionnaire-guided self-assessment, approximately 55% of the patients were prescribed drugs according to the guidelines, 41.2% of the patients understood the severity classification described in the guidelines and 35.3% of them understood that prescription drugs are selected depending on the severity of symptoms. Among the participants, 64.7% answered that it is good to choose drugs for treatment by themselves; on the other hand, 23.5% of the patients preferred that drugs be selected by the physician. Patients who participated in the questionnaire-guided self-assessment reported a satisfaction rate of 100%, thereby indicating that using a patient-centered medical questionnaire to treat pollinosis can improve patient satisfaction, which may in turn improve adherence.

    The speech discrimination score (SDS) has proven to correlate well with the average hearing level in 349 ears with sensorineural hearing loss (excluding those with vestibular schwannoma), with the equation: y=-1.1102x + 136.15 (R2=0.4876). Seventeen ears showed significant declines in the SDS. Of these 17 ears, eight ears of six aged patients showed progressive hearing loss without any particular disease and five ears were from three patients who had suffered from Meniere's disease for more than 30 years, which suggested that retrocochlear hearing loss might develop in Meniere's disease if the duration of the disease becomes prolonged.

    Langerhans cell histiocytosis (LCH) is a very rare disease in which the number of abnormal Langerhans cells increases. We herein report two pediatric cases of LCH discovered in temporal bone lesions. The first patient was seen in our outpatient clinic due to right otorrhea. Swelling of the posterior wall of the external auditory canal was observed. A CT scan showed a soft tissue shadow with osteolysis in the bilateral temporal bones. The second patient was referred to our clinic due to swelling of the posterior wall of the right external auditory canal. A CT scan showed not only a soft tissue shadow with osteolysis in the right temporal bone, but also multiple lesions in the lungs and cervical lymph nodes. In both cases, a biopsy of the temporal bone lesion was performed under general anesthesia,and the patients were diagnosed with LCH. The administration of systemic chemotherapy in the pediatric department was effective, and both patients made satisfactory progress. When examining children with intractable otitis externa and/or mastoiditis, LCH should be considered as a possible differential diagnosis.

    Tuberculosis is a rare cause of otitis media (called TOM). It often takes time to establish a diagnosis of TOM because of the clinical variability of the disease. We herein report three cases of TOM recently treated at Hamanomachi Hospital between April 2005 and March 2013. It took seven days, six months and four months to establish the diagnosis of TOM from the first visit to our hospital and it took one year, eight months and six month from the first visit to a medical institution until the diagnosis. TOM can induce serious complications, like sensorineural hearing loss and facial palsy, so an early diagnosis and early treatment are needed. Therefore, TOM should be suspected in cases of intractable otitis media, and it benefits patients to start antituberculosis treatment early, because it may prevent the development of severe complications.

    We treated a 31-year-old male with granulomatous tonsillitis who had suffered from a recurrent sore throat,fever and bilateral cervical lymphadenopathy under a diagnosis of chronic tonsillitis. He underwent bilateral tonsillectomy. On the fifth day after the operation, bleeding from the hilus of right tonsil was seen. On the 10th day, he had a high fever, but on the 18th day, he had no fever. On the 25th day, he was discharged. After discharge, he had no sore throat, fever or cervical lymphadenopathy. The histological examination revealed granulomatous tonsillitis in bilateral tonsils that showed focal central necrosis and Langerhans giant cells. The cause of the granulomatous inflammation was investigated, and no systemic isorder was identified.
