    Tonsillectomy is one of the prevailing treatments for IgA nephropathy. This retrospective study aimed to elucidate prognostic factors for the postoperative kidney function of tonsillectomized patients with IgA nephropathy. Forty consecutive patients with IgA nephropathy who underwent tonsillectomy in our department between 1999 and 2008 were enrolled. They were 21 men and 19 women with ages ranging 14-52 years with an average age of 25.5 years. The patients were classified into remission and non-remission groups based on their kidney function assessed 1 year after surgery according to the clinical guidelines for IgA nephropathy of the Japanese Society of Nephrology. Patients' profiles and preoperative physical findings/laboratory data in the remission group were then compared with those in the non-remission group.
    The remission and non-remission groups included 13 and 27 patients, respectively. The remission group showed a significantly shorter interval between onset to surgery (2.3±2.1 vs. 5.0±6.7 years; p=0.032), a lower diastolic blood pressure (66±13 vs. 75±17 mmHg; p=0.040), a higher level of serum total protein (7.6±0.5 vs. 7.0±0.7 mg/dl; p=0.015), and a higher degree of tonsillar hypertrophy (Io: IIo: IIIo=5: 8: 0 vs. 21: 6: 0; p=0.033) in comparison with the non-remission group. Multiple logistic regression analysis also revealed that patients with a higher level of serum total protein and those with a higher degree of tonsillar hypertrophy were more likely to recover. We should carefully consider these prognostic factors when indicating tonsillectomy for the treatment of IgA nephropathy.

    Between 1992 and 2010, we studied audiometric findings obtained from the annual hearing examination for senior high-school students who belonged to an active Kendo team. The subjects comprised 140 males and 88 females with ages between 15 and 18 years. The pure tone audiometry showed that 69 ears of 45 students (19.7%) had sensorineural hearing loss with the highest threshold shift at 2000 Hz, followed by 4000 Hz. Frequent audiometric patterns included a 2000 Hz-dip, a 4000 Hz-dip and a combined loss of 2000 and 4000 Hz. Some of these affected subjects had shown a completely normal audiogram at the previous examination. Moreover, small-dips with a depth less than 25 dB were found to be limited either at 2000 Hz or at 4000 Hz, suggesting early audiometric changes from a temporary or a permanent threshold shift caused by noise and/or head blows during Kendo practice. The incidence of such abnormal audiograms among this Kendo club members appears to be decreasing year by year owing to the annual check-ups over the 18-year study period, and student counseling regarding the possible adverse effect of Kendo on the inner ear function.

    Cochlear nerve deficiency (CND) is diagnosed with magnetic resonance imaging (MRI) by an absent or small cochlear nerve. A small or absent bony cochlear nerve canal (BCNC) detected with computed tomography (CT) has been also considered as CND. We reviewed five bilateral hearing impaired children with BCNC. All patients were born maturely at full term birth. Two of them had undergone newborn hearing screening (NHS), one passed and the other was referred in only one ear. Among five children, only one had a small internal auditory canal (IAC) diagnosed with CT. Two children with intracranial abnormalities also had cochlear anomalies without a small IAC. Hearing aids showed some effectiveness in two patients with normal-sized IACs, and they could communicate with normal speech using hearing aids. One with a small-sized IAC was unable to communicate with speech using hearing aids. The efficacy of hearing aids in the other 2 patients has not been evaluated yet. We concluded that patients with small or absent BCNCs showed various audiometorical findings and clinical courses.

    We report on two cases of spontaneous CSF otorrhea, which were considered to have been caused by enlarged arachnoid granulation with bone erosion of the posterior fossa. Both cases visited us complaining of severe headache, due to bacterial meningitis. In the first patient, a 68-year-old male, a high resolution CT scan showed a bony defect in the posterior fossa plate in the right temporal bone, where CSF leakage was confirmed during the operation. In the second patient, a 54-year-old female, a bony defect was located in the posterior fossa in the left temporal bone. In both cases, the bony defects were repaired by occlusion with the pedicled temporal muscles after the meningitis had been treated. CSF otorrhea disappeared after the surgery, and has not recurred during the postoperative observation period of 1 to 3 years.
