      Home-based management for dysphagia is aimed at promoting safe oral intake in the dietary environment of each patient, using the residual swallowing function where possible; the goal of this treatment is not to cure dysphagia. After evaluation of the swallowing function, a compensatory approach is provided for swallowing rehabilitation, in addition to a therapeutic approach. The compensatory approach can start immediately after the swallowing test, with immediate effect. We adopted this approach in 40 patients who were prohibited from eating at home after receiving a diagnosis of dysphagia by endoscopic examination, and 27 of these patients (67.5%) were subsequently able to resume oral intake. Thus, the compensatory approach can be started at the initial home visit and shows an immediate effect. This approach is also useful as therapy for maintenance and improvement of the swallowing function, and is thus important for home-based management of dysphagia.

      Oncocytomas are rare benign tumors that occur most commonly in the parotid glands, but are also well known to arise in organs such as the salivary glands, kidneys, and thyroid. Herein, we present a case of metachronous and bilateral oncocytoma of the parotid glands, with a review of the literature. The patient was a 78-year-old woman who was referred to our department for surgical treatment of multiple tumors in the right parotid gland. The patient gave a history of having undergone total left parotidectomy 23 years earlier for multiple tumors in the left parotid gland, and postoperative histopathology had revealed the diagnosis of oncocytoma. At the current presentation as well, both the intraoperative pathological and intraoperative findings suggested benign tumors, and we performed a total right parotidectomy while preserving the facial nerve. Although there has been no recurrence since the surgery, we believe that close follow-up and monitoring are necessary as there have been reports of recurrence as well as malignant transformation.
