    Gutzeit gave a definition of focal infection by stating that focal infection means a pathological manifestation, in which a chronic infectious lesion (focus) causes reactive organic or functional disorder in various organs distant from the focus. His finding has received general recognition and is practical and understandable from the stand-point of clinicians, too. We have mainly studied tonsillar focal infection from the clinical standpoint.
    The results thus obtained were described.
    The structure and function of the tonsil, the mechanism of focal infection, chronic inflamation and immune responses in the tonsil and clinical problems were described and discussed.

    The auditory brainstem response (ABR) was evaluated in 19 patients with cere-bello-pontine angle tumor (CPAT) and the following results were obtained.
    All patients exhibited positive test results. The response patterns were divided into three types; namely 1) the prolongation of I-V internal compared to the normal mean (two cases); 2) the presence of wave I but the disappearance of the following (thirteen cases); 3) no response at the maximum stimulation with 115dB (four cases). The simultaneous recording in the Love-Vertex and Membrane-Vertex made the detectability of wave I and V increased and is believed to be an extremely useful method for the detection of cochlear nerve disorders. The results in the unaffected side as well as the other neurotological examinations are also important in evaluating the size of the tumor.

    In order to reestablish aeration of the middle ear in the case of obstruction or chronic stenosis of the Eustachian tube, a new tympano-frontal shunt is developed. Detailed descriptions were given on the structure of the shunt and the surgical procedure for implantation of the shunt. Although the follow up period was not long enough, the results of two cases were highly encouraging.

    This study was undertaken to evaluate the effects of hyposensitization in 435 patients on the nasal allergy with or without extrinsic asthma. There were 360 patients with nasal allergy uncomplicated with asthma. Specific and nonspecific IgE, numbers of eosinophils in the blood and nasal secretion, and subjective nasal symptoms were employed as parameters.
    The incidence of abnormally high serum IgE, specific IgE and blood eosinophils was found to be higher in the patients with nasal allergy with asthma than those uncomplicated with it. The difference in the incidence was statistically significant. There was no significant difference in the incidence of the patients with abnormally increased eosinophils in both blood and nasal secretion between the patients with or without asthma.
    During the course of hyposensitization over one year, it was found that it was only the number of eosinophils in the nasal secretion which was significantly decreased. In the patients with nasal allergy uncomplicated with asthma, the appearance of the blocking antibody was significantly increased after a one-year course of immunotherapy. Other parameters were not changed in the patients with or without asthma.
    It was concluded that the parameters examined were almost identical and that nasal allergy with or without asthma might be improved by hyposensitization over one year.

  • A Review of 97 Cases

    From 1971 to 1978, 97 patients with a polypoid vocal fold, or Reinke's edema, were treated with endolaryngeal microsurgery at Kurume University Hospital. This paper presents the results of a review of these cases. Of all the cases, 57 were male and 40 were female. Thirty-two patients were in their forties, 31, in their fifties and 28, in their sixties. The lesion was bilateral in 73 patients and unilateral in 24 patients (16, on the left; and 8, on the right vocal fold). The most frequent etiologic factor seems to be cigarette smoking. Eighty-six patients were smokers whereas only six were non-smokers. In the other five patients, the history of smoking was not available. All the patients had hoarse voices. The duration of the symptom was one year or longer in many cases. Twelve patients presented a excessive adduction of the ventricle folds during phonation. Besides polypoid swelling, a polyp was observed in 12, the so-called prolapse of ventricle in six, inflammation in four, sulcus vocalis in three, and epithelial hyperplasia in one patient. Histological investigation was available in 37 cases. Edema and teleangioectasia were the most dominant findings. Fibrosis was also observed in some cases. With respect to surgery, it seems to be mandatory to preserve intact mucosa at the edge of the vocal fold in order to assure good phonatory function.

    Analysed were nine cases of undifferentiated carcinoma of the thyroid gland treated in Kyoto University during the past ten years. The patients in this report had usually been bearing goiter for a long period and came to the hospital with such complaints as hoarseness, dyspnea, dysphagia, local pain and recent rapid growth of the goiter. A strong tendency of the tumor to invade the surrounding organs was noted as the nature of this tumor, which made the prognosis of these patients extremely poor. Four cases out of nine suggested a possibility of malignant transformation from differentiated carcinoma.

    Effectiveness of adjuvant Chemoimmunotherapy with Carboquone and PS-K was studied in 15 patients. After the initial treatment of cancer, Carboquone in one dose of 0.25mg and PS-K in three doses of 1.0g were given per os every day for at least four weeks up to six months.
    This therapy was evaluated in the light of clinical progress and immunologic parameters. In the three patients who had previously undergone only palliative treatment, (one had cancer of the maxilla and the other two had cancer of the salivary glands) this therapy was most effective, and the tumors are not apparent now. In most cases, the counts of peripheral lymphocytes were slightly decreased; the skin reactions to Candida extract and PPD were increased; and PHA- and PWM-induced lymphocyte transformations were markedly increased. There were no severe side effects.
    This study strongly suggests that this adjuvant chemoimmunotherapy is useful at least in cancer of the salivary glands.

    In the controlled trial of Kameyama et al., methyl-B12, a neurotropic vitamin of the B-complex proved to be safe and effective for diabetic neuropathy. There are several open trials upon the diseases of cranial nerve in otolaryngology, such as hearing loss and facial palsy. We determined the effectiveness of this drug in 40 patients with peripheral vertigo in an open trial.
    Concerning vestibular symptoms, i. e. vertigo and dizziness, 84% of the patients experienced relief while on 6tab/day (3000μg/day) for 4-6 weeks.
    The most significant results as determined by the equilibrium function test were as follows; (1) the caloric tests showed a restoration of the disequilibrium in labyrinthine excitability on the level of 1% of significance. (2) the vertical writing tests yielded a significant (p<0.005) improvement in deviation toward the affected ear which corresponds to the restoration in the caloric tests.
    One patient complained slight anorexia and alimentally discomfort (2.5%).

    The clinical effect of 5-FU Dry Syrup was studied in 50 patients with head and neck cancer. Forty of the cases were squamous cell carcinoma, and most of them received combined therapy of 5-FU and irradiation and/or operation. The patients were given 5-FU Dry Syrup at doses of 300mg daily.
    Following the criteria proposed by the Tokyo Cancer Chemotherapy Association, the initial effect of 5-FU Dry Syrup was evaluated when the tumor reached a minimum size without surgical excision. The rate of effectiveness was 81.4%. The long-term effect was evaluated after about 2 years of a follow-up period and was 62.5%. The rate of initial effectiveness was 80% for maxillary cancer and 87.5% for laryngeal cancer. The rate of initial and long-term effectiveness for squamous cell carcinoma were 82.3% and 56.3%, respectively.
    Clinical symptoms and laboratory findings considered to be attributable to the side effects of this agent were observed in 19 cases. In detail, gastrointestinal disorders were experienced in 10 patients, hematopoetic disorder in 10 and eruption in one. These side effects were encountered during the period of radiation therapy and were temporary in duration without any serious consequences.
    5-FU concentrations in the tumor tissue and serum of 13 patients with head and neck cancer were determined by the bioassay technique. The samples were obtained one to two hours after oral administration in a dose of 200mg of 5-FU Dry Syrup. In four of the 13 tumor tissues and three of the 13 sera the concentrations of 5-FU were quantified and averaged 1.03mcg/g and 0.053mcg/ml, respectively.
    On the basis of the above findings, we consider that 5-FU Dry Syrup is an useful agent against head and neck cancer.

    A new office closure method was carried out on 54 ears with central perforation.
    This method is characterized by the use of the human amniotic membrane which has been exposed to high-voltage cathode-ray irradiation. The purpose of this irradiation is to sterilize the tissue and eliminate its immune response.
    This method was also widely indicated for prolonged otitis media with large perforation.
    Successful closure was accomplished in 44 of the 54 cases and the average time required for treatment in these 44 was six and a half weeks.
    The functioning of the new tympanic membrane closed by this method was appraised with tympanography.

    Fifty patients were examined in order to define the clinical value of carcinoembryonic antigen (CEA) in maxillary cancer.
    Plasma CEA levels of all cases were measured. At the time of testing, 37 of the 50 were tumor-bearing and the others were tumor-free more than six months after treatment.
    Plasma CEA level higher than 2.6ng/ml was estimated positive and 16 of the 37 patients who were tumor-bearing and none of the 13 patients who were tumor-free were positive.
    In 18 out of the 37 patients who were tumor bearing, tumor CEA levels were measured. In 30% of the cases studied, the tumor CEA level was higher than that in the mucous membrane of maxillary sinusitis.
    Correlation between the tumor CEA level and the plasma CEA level was observed.
    No significant findings were shown in immunohistological study.
    Plasma CEA levels in the patients with maxillary cancer correlated with the tumor volume and the tumor CEA level.
    Serial determination of plasma CEA levels may have some value in monitoring tumor metastasis.
    CEA in maxillary cancer seems to be much more affected with CEA-like substances.
