    A 28-year-old man (merchant) was seen by us on March 19, 1979 with sudden hearing loss in his right ear whose onset was on March 16. Under the diagnosis of sudden deafness he was started on therapy on March 19. Various therapeutic agents were tried such as, inhalation of 5% CO2-95%, O2 76%, urographin, stellate ganglion block, vitamin B complex, vasodilator drug, and steroid hormone. Subsequently, hearing loss occurred suddenly in his left ear on July 2. While on this therapeutic regimen, he developed sensori-neural hearing loss in both ears, of the reversible type in the right ear and the recurrent type in the left ear without accompanying vertigo. This paper describes the clinical course of this patient and discusses the relation between reversible and recurrent sensori-neural hearing loss and sudden deafness. Drug therapy is of questionable benefit in this condition. Recovery is likely to be spontaneous.

    A new office closure method using collagen sheet was carried out on 40 ears with central perforation of the tympanic membrane. This method is recommended for general use in cases of prolonged otitis media with sizable perforation. Successful closure was achieved in 30 of the 40 cases, and the average time required for treatment in these 30 cases was 7.6 weeks. This method is thought to be useful because of its good results and the general availability of collagen sheet.

    A 78-year-old woman visited our hospital on October 9, 1979 with complaints of cheek swelling on the left side and nasal bleeding.
    On examination she was found to have an obvious appearance of maxillary cancer. Histologic examination of the lesion showed squamous cell carcinoma.
    On the other hand, serum electrophoresis of the patient showed total protein of 8.9g/dl, with a broad peak in the gamma region. The IgG level was 4, 169mg/dl, the IgA level, 174mg/dl and the IgM level, 67mg/dl. Her serum reacted to anti sera. The Bence-Jones protein was negative. Bone marrow aspiration showed 11.6% abnormal plasma cells.
    Since the first report of Sugizima (1963), many cases of stomach cancer coexistent with multiple myeloma have been reported. However, combination of maxillary cancer and multiple myeloma in the same patient is rare. Therefore, the present case was described in detail with a discussion of the relationship between multiple myeloma and cancer from the immunological standpoint.

    A rare case of a bifid tongue in a 8-month-old boy was reported. The condition was characterized by a bifid apex of the tongue and an attached pedicled mass. Embryologically the apex of the tongue is formed by two lateral lingual prominences. Failure in fusion of any portion of the lateral halves during the 6th embryonal week results in bifurcation of the tongue. Cosmetic operation for the cleft should be considered even in cases of mild grade.

  • Adaptations and Limitations

    Radioisotopes were effectively employed for supplementary diagnosis of parotid tumor in 49 patients. A combined method (99mTc and 67Ga) yielded the best results in classifying parotid diseases and in improving diagnostic accuracy for malignancy. This method has two main advantages: 1) Tumors and inflammations were discriminated by 99mTc, while benign and malignant tumors were further differentiated by 67Ga. 2) The combined method was capable of detecting a greater total number of tumors than the 67Ga method alone.
    We think that RI is applicable to all kinds of parotid diseases. If the suspected tumor is large and hard, RI is especially advantageous. Results are less conclusive in the case of inflammation. The test is not recommended for small tumors under 2cm in diameter.

    20 cases (14 patients) of head and neck tumors were treated by Co2 laser surgery in ENT clinic, the University of Tsukuba, during the period from Nov. 1, 1979 to Aug. 15, 1980.
    It was thought that laser surgery is a powerful treatment for benign tumors and local recurrence of malignant primary tumors.
    It was speculated that laser surgery will be desirable treatment for malignant melanoma and small, superficial primary cancer.
    It was concluded that 50W-60W, defocused beam vaporization (1mm-2mm) for benign tumors, and 1mm-0.5mm beam vaporization for malignant tumors are recommendable laser conditions for the head and neck tumors.

    Rating paralysis score (YANAGIHARA, 1976) is an excellent psychometric method for clinician to estimate degree of facial paralysis. In addition to this, integrated EMG method proposed by YAGI, et al. (1979) also gives an additional electromyographic information of affected facial muscles. When we use both paralysis score and integ. EMG, we can easily follow recovery process of facial paralysis expressed as increase of score and percentage of integ. EMG. Both increases are almost linear to daily recovery of paralysis, and could be plotted in semilogarithmic paper as a linear line. By applying regression analysis to this, it is possible to calculate and determine slope and constant of following equation:

    In equation, slope means speed of recovery such as recovery become rapid when it is high.
    In 29 cases of peripheral facial paralysis, the effect of methycobalgroup (12 cases) and steroid group (17 cases) was compaired after calculation of each regression line in all cases. As to slope, methycobal group and steroid group were 80.07, 92.21 respectively and showed no significant difference statistically.
    This means that methycobal has same effect as steroid on treatment of facial paralysis.

    Bacampicillin, an ester derivative of Ampicillin, was administered to 19 patients with acute infections, i. e. 5 cases of acute tonsillitis, acute exacerbation in 4 cases of chronic sinusitis and in 4 cases of chronic otitis media, one each of earlobe abscess, nasal furuncle, acute otitis media, acute parotitis, sialadenitis of the submaxillary gland and retroauricular lymphadenitis. Two patients did not return for evaluation. The clinical effects were excellent in 8 (89%) and good in 1 case (11%) for simple acute infection and were excellent in 1 (12.5%), good in 1 (12.5%), fair in 3 (37.5%) and poor in 3 cases (37.5%) for acute aggravation of chronic infection. As side effect of slight intestinal disorder was recorded in 1 (1/19), but was not serious. The preparation of Bacampicillin demonstrated a high therapeutic efficacy against various acute infections in the field of E. N. T. practice.

    Forty patients with spino-cerebellar degeneration were examined neurotologically. The chief findings were as follows:
    1) Spontaneous nystagmus was relatively rare, but gaze paretic nystagmus, direction-changing positional nystagmus (especially apogeotropic type) and counter-beating vertical positioning nystagmus were observed very frequently.
    2) Rebound nystagmus, flutter-like oscillation and transitory alternating saccade were observed in five to eleven patients.
    3) Saccadic dysmetria and disturbance on rapid, alternating change of gaze were remarkable in about two-thirds of the patients, though the velocity of saccade itself was not influenced by the disease.
    4) Optokinetic nystagmus and smooth pursuit movement were impaired in 80-90% of the patients.
    5) The release phenomenon of cerebellar and/or brainstem inhibition of the vestibulo-ocular reflex was seen during the visual suppression test (after Takemori) and pendular rotation.
    6) Disturbance of gaze control and change in the vestibulo-ocular reflex seemed to be important in the diagnosis of the cerebellar and/or brainstem lesions.

    To obtain a better understanding of the characteristic of neck-torsion nystagmus previously reported (Ito and Hinoki 1980) the following experiments were undertaken using the normal rabbits with black eyes. Eye movements were observed using the electronystagmograph (ENG) and were examined in the following four postures. Some of the animals were examined using the electroencephalograph (EEG), together with the ENG. The results obtained were as follows:
    (1) The posture used herein were:
    (i) A posture in which a rabbit's trunk is hung and fixed with cloth and the four limbs are unrestrained. The head of the animal maintains a normal position with an anterior inclination of 45 degrees. (hereafter termed the 1st posture) (ii) A posture in which the animal maintains the left lateral decubitus without any sign of torsion of the neck. (hereafter termed the 2nd posture) (iii) A posture in which the animal takes the 2nd posture for at least 20 seconds and then the trunk is counterclockwise rotated at 90 degrees about the fixed head and hung after the same manner as mentioned in (i). (hereafter termed the 3rd posture) and (iv) A posture in which the animal's head maintains the above mentioned normal position. The trunk is then counterclockwise rotated around the longitudinal axis at more than 90 degrees. Thus, the four limbs of the animal points upwards. (hereafter termed the 4th posture)
    (2) In the second posture the animal usually showed deviation of the eyes alone. Some of the animals showed nystagmic responses in this posture. However, such nystagmic responses were sporadic and did not show any constant reappearance. In contrast, the animal constantly showed regular and active nystagmic responses in the 3rd posture. The animal also showed nystagmic responses when the 4th posture was taken. However, such nystagmic responses were less obvious in frequency and amplitude as compared to those in the 3rd posture.
    (3) There was a close relationship between the appearance of the nystagmic responses and the arousal state in the EEG.
    (4) The nystagmic responses referred to herein showed a phenomenon of “Response Decline” even in the 3rd posture when the examination was repeated for a fairly long time and with a short rest. Such phenomenon was accompanied by the reduced activity of the EEG's. From these findings the following conclusions were drawn.
    (a) The idea that the neck-torsion nystagmus previously reported is induced from the cervical proprioceptors in close collaboration with the otolithic organ of the peripheral labyrinth is supported by the present experiments.
    (b) The appearance and decrease of the neck-torsion nystagmus are closely concerned with the activity of the brain, particularly the activity of the brain stem reticular formation.

  • Comparison Between Normal Controls and Nasal Allergic Patients

    The fine structure of the lymphatic capillaries of the human nasal mucosa was observed with an electron microscope under local anesthesia. Biopsies were taken from the anterior tip of the inferior turbinate of normal controls and patients with nasal allergy caused by Japanese cedar. In normal controls, many loose endothelial cell junctions were observed, which suggested that the lymphatic capillaries in the anterior tip of the inferior turbinate were fully active. The lymphatic capillaries in the nasal mucosa were characterized by flattened endothelial cells, sparse basement lamina and the presence of anchoring filaments which provided a firm connection between the lymphatic capillary wall and the surrounding connective tissue. A few layers of lymphatic capillaries which seemed to be the starting point of lymphatic capillaries were observed. These layers could not be found in inactivity where the space of lumen was shrunk. They were easily identifiable in the widened lumina of allergic patients. Inflow of eoainophilic leucocytes to the lymphatic capillaries was observed 30 seconds after provocation in patients with nasal allergy. 15 minutes after provocation, interstitial edema was remarkable, the luminal space of the lymphatic capillaries widened and the endothelial cell junctions opened wide. It is therefore considered that the inflow of the interstitial fluid to the lymphatic capillaries was remarkable.

    A series of 120 unselected radiographs of the larynx without organic laryngeal disorders was analysed from the standpoint of laryngeal calcification or ossification. The special patterns of laryngeal ossification were described in the lateral X-ray using low kilovoltage technique of the larynx.
    1) Normal “figure 8” configuration of the ossified thyroid cartilage angle was characterized in men (37/45, 82%) but rare in women (10/75, 13%).
    2) A horn-like shadow of the ossified arytenoid cartilage was characteristic in women (43/75, 57%) as compared with men (7/45, 16%).
    3) A crescent-shaped shadow of the ossified top of the cricoid cartilage was observed in 42% of the total cases.
    These special configurations of the laryngeal ossification were also discussed from the viewpoint of laryngeal functional surgery.
