    Two types of columella materials for tympanoplasty made of alumina ceramics (Bioceram®) were newly designed and used for ossicular reconstruction. From experience with 40 ears, the following results were obtained:
    (1) CORP can easily be contacted with fascia grafts because of the characteristic convex shape of its head. CORP can be trimmed to a suitable shape and length using a diamond burr.
    (2) CORP is supported by a nasal cartilage disc in most cases. In some cases, CORP can be fixed by the chorda tympani nerve if available, or only interposed between the tympanic membrane and the stapes in the second stage operation.
    (3) CORP is highly biostable even in the aerated middle ear space because of its good compatibility with the grafted fascia and stapes. In 18 months, 40 have so far been implanted with no rejection and good hearing results. Ten of the 13 ears (followed up over 6 months) had an average level of air conduction for speech frequencies within 30 dB.
    (4) CORP is visible through the grafted fascia both during and any time after operation. The position of CORP in the middle ear can be confirmed by X-ray because it is roentgenopaque.

    A case of extra-abdominal desmoid tumor in the left maxillary sinus is presented. A 12-year-old male who had complained of swelling of the left cheek 16 months previously was operated upon by Caldwell-Luc's procedure. As diagnosed based on roentgenography and CT to be a benign tumor of the maxilla, this tumor was found on operation to be a massive fibrous lesion. Histological examination revealed extra-abdominal desmoid tumor. Local recurrence has not so far been demonstrated for 10 months postoperatively.

  • And 8 Other Cases of Otolaryngeal Tuberculosis encountered in the past 5 Years

    A rare case of primary epipharyngeal tuberculosis and 8 other cases of tuberculosis in the otolaryngeal region encountered in the past five years were reported. The primary epipharyngeal tuberculosis was seen in a 51-year-old female. She complained of tinnitus and uncomfortable feeling in the left ear, although did not complain of earache, hearing impairment or sore throat. On histopathological examination of the ulcerative lesion of the pharyngeal orifice of the eustachean tube, this patient was diagnosed as having tuberculosis.
    The other 8 cases encountered were 4 tuberculous otitis media, one laryngeal tuberculosis, one epipharyngeal tuberculosis, one lymphadenitis colli tuberculosa and one retropharyngeal abscess with cervical vertebral tuberculosis.
    The recent characteristics of tuberculosis were analysed.

    We reported a case of an esophageal foreign body in a one year old boy which had not been found for five-and-a half months. When he was 9 months old, he used to vomit baby food suddenly, although he could take milk. When he was 1 year and 2 months old, after he caught a cold he stopped being able to take milk. He was admitted to the pediatrics department in our hospital where a chest X-ray film revealed a foreign body in the esophagus. Under general anesthesia rigid esophagoscopy was performed and a polyethylene tube that had a lumen was discovered. After esophagoscopy the patient was fed through a nasal tube for seven days. X-ray examination using contrast medium revealed a good finding, based on which he was discharged.

    A congenital bifid nose, a rare abnormality, in a 5-month-old girl was reported.
    The infant's nasal dorsum was flat and broad with several irregular fissures running lengthwise over its central part. A spindle-shaped excision was perfomed on the fissured part together with correction of the nasal cartilage.
    This anomaly appears more frequently in females, but a heredity relationship has not yet been determined. It is to be treated by surgical procedures, which should essentially include excision of the fissured part of the nasal dorsum and correction of the separated cartilage. Cosmetic operation for severe cases should be considered at the earliest possible stage.
    Failure in fusion of the two medial nasal swellings during the 7-10th embryonal week results in a bifid nose.

  • Parametric Changes before and after Operation

    As possible indicators in carcinoma-affected individuals, the following parameters were adopted: 1) The ADCC (Antibody Dependent Cell-mediated Cytotoxicity) activity, 2) The rate of active E rosette formation and 3) in vitro PPD.
    Examination was made with respect to these parameters on the pre- and post-operative changes in 5 patients with laryngeal carcinoma and ethmoid sinus carcinoma.
    Although no particular changes were noted in the first two parameters, there was a tendency that PPD negatives turned positive postoperatively.

    Forty-four cases of stage I and II non-Hodgkin's lymphoma were treated with radiotherapy with or without chemotherapy. All of the 44 patients had the lesion in Waldeyer's ring or paranasal sinuses. Antineoplastic agents used in chemotherapy were Bleomycin, 5-Fuorouracil, Chromomycin A3 or a combination regimen (cyclophosphamide-Vincristine-Predonison). Thirteen patients treated with radiotherapy alone received irradiation doses over 4, 000 rad, while 31 patients treated by the combined modality received more than 2, 400 rad.
    Relapsing lesions developed in 61.5 per cent of the patients after radiotherapy, while in 38.5 per cent treated with the combined therapy. The overall 5-year survival rate was 53.8 per cent for radiotherapy alone, while 74.2 per cent for the combined therapy, but this difference was not statistically significant. A low relapse rate and a high survival rate were seen in the group with the combined modality regardless of irradiation doses. The relapse-free 5-year survival rates for stage II cases were signifcantly different between the two treatment groups compared.
    We concluded that chemotherapy for stage I and II non-Hodgkin's lymphoma of the head and neck decreased the rate of relapse. The combined treatment for stage I and II non-Hodgkin's lymphoma is recommended.

    It has been generally accepted that the neck afferents play a major role in postural adjustment. Eye and body movements induced by vibratory stimulation of the unilateral dorsal neck area were examined in patients with vertigo suspected to be of cervical origin, as well as in those with unilateral peripheral vestibular impairment and normal subjects. Whether or not vibratory stimulation was a useful method for the objective examination of cervical vertigo was investigated.
    Although in some normal subjects, fine nystagmus and body movement were evoked on neck stimulation, in those with unilateral peripheral vestibular impairment and vertigo of possibly cervical origin, neck stimulation evoked nystagmus and body movement at much higher rates. In unilateral peripheral vestibular impairment, the evoked nystagmus was always in the direction contralateral to the impaired inner ear. Postural deviation to the ipsilateral side of the impaired vestibulum, namely to the side of the slow phase of the neck-induced nystagmus, was often observed on stimulation of both sides of the neck. In vertigo of possibly cervical origin changing the side of stimulation reversed the direction of the nystagmus in most of the cases. Body movement induced by neck vibration in these patients had a tendency to deviate to the contralateral side of the evoked nystagmus. In patients having localized tender spots or a bruised area on the neck, severe dysequilibrium was often provoked on vibrating these points. Moreover, caloric nystagmus was often influenced by the neck vibratory stimulation. Generally, if the direction of caloric nystagmus coincided with that of neck-induced nystagmus, the caloric nystagmus was reinforced and vice versa.
    The usefulness of neck vibration for the objective diagnosis of cervical vertigo was stressed.

    The effect of Carboquone (CQ) on cancer of the maxillary sinus was experimentally examined in nude mice and clinically studied in 20 patients who received its intraarterial infusion.
    In the experimental study, CQ and 5 Fu at doses equivalent to their LD50 where given after 1000 rad radiation and a significant inhibitory effect was revealed tumor proliferation as compared to the control group, although no effect was observed following the administration of CQ or 5 Fu without radiation. It was suggested from these results that the effect of combination therapy composed of surgical intervention, intraarterial infusion and radiation could be clinically significant.
    In the clinical study, on the other hand, the effect of CQ i. a. plus radiation performed after the partial resection of the maxilla was examined. CQ was administered at 1mg a time by one shot every other day up to 10mg in total.
    Histologically, a marked degradiation of cancer cells was noted.
    Of the 20 cases enrolled in this study, 14 remained alive and 6 died (3 died of causes other than cancer). There were 6 cases of relapse and 2 cases of cervical metastasis. In addition, an extensive operation could be avoided in 11 cases by CQ plus radiation therapy after operation, with a decreased dose of radiation to an average of 3320 rads. No patients suffered from visual disturbance.
    This combination therapy is considered to be useful as a new therapeutic method for treating the cancer of the maxillary sinus on account of its superior therapeutic effect, without side effects or damaged appearance by reducing the extent of resection and the amount of drug and radiation to be used.

    Linovin, a preparation of histamine-carried human gamma globulin, was nebulized for six weeks to 102 patients with nasal allergy to evaluate its clinical efficacy. The patients were divided into four treatment groups: the first group received the medicament once a week, the second and third groups, treated with the drug twice a week and received combined therapy with or without cepharanthin respectively. The highest improvement in nasal symptoms was observed in the group treated with Linovin (1/2 vial) alone twice a week. Of the 102 patients, clinical efficacy was excellent in 28 (27.5%), good or better in 77 (75.5%) and fair or better in 96 (94.1%).
    In particular, more improvement was found in nasal congestion compared with other drugs reported previously. The results of the treatment suggested that the drug should be administered twice a week for at least six weeks, particularly for nasal obstruction.

    We administered perorally chinese medicine, Hangekobokuto to 24 patients with abnormal sensation in the throat. At the same time, three kinds of psychogenic examinations (MAS, Y-G test, CMI) were performed. The following results were obtained:
    1) The efficacy of this chinese medicine was evident in 66.6% of the patients following drug administration for 4 weeks.
    2) Analysis of the relationship between the results of psychogenic tests and the efficacy of this agent revealed much better efficacy for the normal group than the group who had some psychogenic dysfunction.
    3) There was no side effect due to this drug.

    A new antibiotic cefmetazole (CMZ) was administered intravenously to 50 patients with acute otolaryngological infectious diseases, and its clinical effect was discussed. These were comprised 2 cases of middle ear infectious diseases, 1 case of acute sinusitis, 8 cases of peritonsillar abscesses, 26 cases of acute lacunar tonsillitis, 3 cases of acute sialoadenitis, 4 cases of oral infectious diseases and 6 cases of acute epiglottitis.
    Side effects of CMZ were noted in 2 cases. Skin rash was observed in 1 patient, besides serum transaminase was elevated in another.
    The clinical results were excellent in 28 cases, fair in 3 cases, while drug was discontinued in 3 cases, because of skin rash in one patient or diagnosed later as infectious mononucleosis in 2 patients. Of the 47 cases, the clinical effectiveness of CMZ was shown in 44 (93.6%).
    Bacteriological examination revealed gram-positive cocci in 24 cases, gram-negative bacilli in 9 cases and anaerobes in 5 cases. The MIC of CMZ was superior to that of CFX, CER or CEZ in case of gram-negative bacilli.
