A tendonous structure between an abbreviated incus long leg and the pyramidal eminence

The patient, a 42-year-old male, who had had nonprogressing hearing impairment of the right ear for about 12 years. Middle ear malformation was diagnosed preoperatively. The pure tone audiogram showed an air-bone gap of 25-40dB. High resolution CT-scanning revealed an abnormal high density shadow at the site of the stapes. Impedance audiometry, the tympanogram was A-type and no stapedial reflex was elicited.
The operative findings were;
(1) The superstructure of the stapes was absent, and the long leg of the incus was abbreviated.
(2) The incudomallear joint was fused osseously.
(3) An abnormal bony mass replaced the stapes.
(4) The oval window was located slightly anteriorly.
(5) A tendonous structure bridged the abbreviated incus long leg and the pyramidal eminence.
The tendonous structure was the most characteristic feature of this case. It has been reported so far only by Isenberg et al. in 1980.
The tendonous structure was considered to be a stapedial tendon, and the reason that no stapedial reflex could be elicited was thought to be the malformation of the stapedial muscle and/or the osseous fusion of the incudomallear joint.

Three kinds of gustatory tests (electrogustometry, filter paper disk method, drop technique) were performed in 80 patients.
1) The drop technique was most sensitive in detecting gustatory disorders.
2) When electrogustometry was performed along with the drop technique, the positive rate was not significantly different from the results obtained when the three tests were used, therefore, for clinical use, electrogustometry and the drop technique are advocated for early detection as well as long term follow up of gustatory disorders.
3) The threshold of electrogustometry correlated well with the average thresholds of the four basic tastes.
4) A positive correlation was found among the thresholds of the four basic tastes.

A very rare case of a partial malignant basal cell adenoma was found on the floor of the mouth in a woman aged 51 was reported. In general, basal cell adenoma occur frequently in the parotid gland and upper lip but are rare in the other regions. Moreover such tumors as part of a layer malignancy are very rare in the literature of Japan, Europe and America.
In our case, histologicaly it was consist of a single cell which resembled an epithelial basal cell. The tumor mixed with tubular and trabecular type. In addition, there was a partial region of mitosis and heterotypic nuclei which showed no transition with benign adenoma.
For treatment, it is said that simple extirpation is sufficient but if malignancy is verified, combination therapy with surgery and irradiation is useful.
However, in our case postoperative radiation was not administered and there seemed to be a recurrence in the floor of the mouth 6 months after surgery.

A 34-year-old male patient complained of left tonsillar calculus without subjective symptoms. X-ray examination revealed an oval radio-opaque mass in the left tonsillar area. Tonsillectomy was performed, and a yellowish gray stone was found in the left tonsil. This stone was 2.1×1.7×1.6cm. in size and its dry weight was 5.2gm..
Chemical analysis revealed 97% calcium phosphate and 3% protein. The etiology of tonsillolith is unknown. The literature is reviewed.

Acute suppurative thyroiditis is rare, particularly in children. Thirty case of acute suppurative thyroiditis, to our knowledge, have been documented in the literature. In most cases the route of infection has not been obvious. We describe here a pyriform sinus fistula which may be a common route of infection in this condition and describe in detail three recurrent episodes of acute suppurative thyroiditis in a 12-year-old girl. A barium meal revealed a fistula originating from the apex of the left pyriform sinus. Fistulas seem to be a common route of infection in acute suppurative thyroiditis, allowing bacterial infection to begin in the perithyroid space and spread to the thyroid gland. Complete extirpation of the fistula is required for permanent cure.

The usefulness of Shaw scalpel (heated scalpel) was examined in terms of the hemostatic effect, wound healing, and thermal tissue damage, using rabbits and rats (Wister).
On the basis of the experimental data, the scalpel was used for 22 head and neck surgeries.
The results are as follows:
1. In contrast to the non-heated scalpel, the heated scalpel demonstrated a marked hemostatic effect, when used for skin incision in rats. However, a week after the incision, wound dehiscence occured in cases of the heated scalpel, the rate of which increased as the temperature of the scalpel rose beyond 110°C. Use of the heated scalpel at the temperature above 200°C resulted in wound dehiscence in all of the cases.
2. The extent of thermal tissue damage by the heated scalpel was studied using rabbits. Histological study revealed that the incised skin margin became degenerated within the range of 300μm at 110°C, and 600μm at 260°C.
3. Clinical application of the Shaw scalpel on the head and neck surgeries showed substantial reduction in operation time resulting from much simplified hemostatic procedures, although skin incision was made by a conventional cold scalpel and electrocoagulator (Bovie) or ligation was also utilized for hemostasis. No complications were noted in connection with the use of the heated scalpel.

A 37-year-old woman was treated for neurilemmoma of the cervical vagus nerve by local excision. In clinical features are described, and the restoration of function after cervical vagectomy is reviewed briefly. Temporary and permanent postoperative sequelae are discussed. The possible occurrence of concomitant unrelated malignancy is emphasized.

Case 1: A 49-year-old woman complained of right nasal obstruction and nasal bleeding.
Case 2: A 40-year-old man complained of tumor of the tongue.
Case 3: A 62-year-old man complained of swelling of the nasal vestibule.
The tumors were removed operatively and no recurrence was seen after the operation. Histopathological examination revealed vascular leiomyoma in all three cases.

Under sodium pentobarbital anesthesia (25-35mg/kg), serum, CSF and inner ear fluids from scala tympani perilymph, scala vestibuli perilymph, and scala media endolymph were collected from normal guinea pigs after the oral administration of glycerol (50%, 12ml/kg) or the intravenous injection of glycerol (1.0ml/kg). The sodium and potassium concentrations were assessed by microflame photometry. The electrolyte dynamics of the inner ear fluids were compared after these two routes of administration.
1. Serum. There was no significant change in Na or K levels after either route of administration.
2. CSF. The Na concentration increased rapidly after both intravenous and oral administration of glycerol and remained high until the end of the experiment. The K concentration did not change significantly after administration by either route.
3. Scala tympani perilymph. The Na concentration increased transiently, after intravenous glycerol, while after oral administration it increased slowly and steadily. The K concentration increased only after oral administration.
4. Scala vestibuli perilymph. The Na level increased slowly and steadily only after oral administration of glycerol, and the K level increased after intravenous injection.
5. Scala media endolymph. No changes in the K level occurred after either route of administration. After oral administration the Na level increased.
These experiments show that dehydration persists longer after oral than after intravenous administration of glycerol. In the diagnosis of Meniere's disease, oral administration of glycerol is more effective than intravenous.

Galvanic stimulation during the performance of the Square Drawing Test (SDT) yields noticeable deviation of the drawn lines of the writing test. Normal subjects and patients suffering from peripheral vestibular disorders or central vestibular disorders were examined using the Galvanic Square Drawing Test. Stimulation was given using bipolar-biauricular electrodes. Results of the Square Drawing Test, which is a modified vertical writing test of Fukuda and which has at least 5 parameters, were evaluated statistically regarding each parameter.
The following results were obtained.
1) The Galvanic Square Drawing Test was applied to normal subjects and patients and was found to be clinically useful.
2) Normal subjects and patients who suffered from peripheral vestibular disorders showed deviation of the drawn lines towards the side of the positive electrode during galvanic stimulation; and towards the side of the negative electrode after the stimulation was removed. Patients with central vestibular disorders showed no definite deviation of the drawing line on galvanic stimulation.
3) The Galvanic Square Drawing Test was useful for the differentiation of central vestibluar desorders from peripheral vestibular disorders.

Development of the fungiform papillae of mice was studied with a scanning electron microscope. Mouse fetuses of gestational ages from 11.5 through 18.5 days and neonates aged one to 21 days were used. Three adult mice weighing about 30gm were used as controls.
1) On the 11.5th gestational day, the dorsum of the tongue was covered with polygonal cells with short microvilli. The surface of these cells was slightly elevated.
2) On the 12.5th day, hemispheroid cells were observed. Small eminences, which consisted of a number of hemispheroid cells, were formed on the dorsum after the 13.5th gestational day. It was observed that these small eminences gradually increased in size and developed to fungiform papillae.
3) On the 16.5th day, mesh-like microridges were found on the free surface of the cells which coverd the fungiform papillae. Desquamation or disappearance of the periderm was noted on the 17.5th day of gestation.
4) In one-day old neonates, outer taste pores were not yet present on the fungiform papillae. A little later outer taste pores appeared and gradually increased in number. In 14-day old mice, an outer taste pore was present on each fungiform papilla.
5) In some cases, outer taste pores disappeared form the fungiform papillae of the tongues of 21-day old mice, presumably because of some mechanical damage to the fungiform papillae.
6) The maximum diameter of the outer taste pore of 21-day old mice was 2.5±0.06μm S. D. (N=20).
Epithelial desquamation on the surface of the fungiform papillae was found around the outer taste pore at this age. The degree of desquamation varied considerably among the fungiform papillae. Both extrinsic and intrinsic factors may influence the degree of epithelial desquamation.

CMX Otic Solution, an antibiotic preparation developed for topical application, was tested for its clinical efficacy and antibacterial activity in patients with external otitis and chronic otitis media. 1% CMX or 0.5% CMX, the pH of which was adjusted to 6.5, was applied to the external auditory meatus or tympanum for 10 minutes, twice a day.
The effectiveness rate of 1% CMX was 88.2% and that of 0.5% CMX was 65.4%. In 94.1% of patients receiving 1% CMX and in 65.4% of those receiving 0.5% CMX culture became negative. 1% CMX was significantly superior to 0.5% CMX against Staphylococcus aureus and Pseudomonas aeruginosa. No serious side effects were observed. When 1% CMX was applied to the tympanum, the concentration of CMX in the serum 30 minutes later was 0.0125-0.02μg/ml.
Topical 1% CMX Otic Solution is effective in the treatment of chronic otitis media and external otitis. Patients should be tested for hypersensitivity before administration of this preparation.

The effect of long-term administration (more than one year) of ATP with a dose of 300mg/day was examined in 20 patients with Meniere's disease.
The rate of improvenent of more than a moderate degree was 100% for subjective symptoms such as vertigo and/or dizziness, hyperacousis and head heaviness, while objective symptoms including the results of auditory and equilibrium examinations improved at an effective rate of 25.0%. They were statistically superior compared to a short-term administration (4 weeks).
When comparing the annual numbers of vertigo attacks before and after treatment, it was also confirmed that long-term administration of ATP markedly reduced the incidence of vertigo. Furthermore, despite its continued administration over one year, no appreciable side effects were observed, indicating its high safety.
This kind of treatment seems to be and extremely useful measure for Meniere's desease which has the characteristic of repeated attacks over long term.

A small tablet, Azulemic, is designed to release slowly water soluble Azulene when inserted into the labiogingival region. The clinical effects of Azulemic on pharyngitis and stomatitis were examined.
The method of this examination was a double-blind controlled trial in which Azulemic was compared with the standard drug.
Removing drop-outs and ruled-outs from a total number of 139, 130 cases were available for statistical analysis of the effects.
Compared with the standard drug, Azulemic showed superior effects in improving both subjective and objective symptoms, and in terms of global evaluation.
No side effects were observed.
Azulemic was regarded as an effective drug of long action for treatment of mucosal inflammation of the oral cavity.

Anchusan is a prescription of crude drugs and has been used for the therapy of gastritis in
Kanpo medicine (Chinese traditional medicine inherited and developed in Japan). The authors administered
Tsumura-Anchusan (7.5g daily for two weeks) to 22 male and 28 female patients with abnormal sensation in the throat in order to evaluate the placeboeffects of the drug for these patients. By analysing the patients' subjective signs and symptoms, the following results were obtained.
1. The drug was effective for 42% of the patients in a week, for 50% in 2 weeks and 44% in 3 weeks (one week after the cessation of the drug).
2. When the evaluation was made one week after the cessation of the drug administration, the rate of effectiveness of
Anchusan for the present study was significantly lower than the previously reported effectiveness of
Hangekobokuto for the female patients and
Saikokaryukotsuboreito for males.
3. Placebo-effects were hardly obtained in patients of 30 to 60 years of age who were skinny, nervous, neurotic and/or dishonest.
