    A case of perilymph gusher was reported.The patient was an 11-year-old boy with bilateral congenital mixedhearing loss. On the suspicion of a fixed stapes footplate, an exploratory left tympanotomy was performed. On making a control hole in the footplate, an abrupt gush of perilymph was observed. Sealing of the hole was effectively done immediately with temporal fascia and muscle. Postoperative examination revealed that the left hearing acuity wasremained at the preoperative level. The computed tomography scan of the temporal bone demonstrated normal contours of the inner ear with abnormal bony defects of the fUndi of the bilateral internal auditory meatus.This was considered to be the route of the cerebrospinal:Huid gush.

    The closing function of the eustachian tube was examined by manosonotubometry in 96 ears with otitis media with effusion(OME), 47 ears of patients with cleft palate with OME, 41 ears with chronic otitis media(OMC)and 17 ears with cholesteatoma.
    1. Eustachian tube closing disturbance was found in 65% of the cases of cholesteatoma, 49% of those of cleft palate with OME, 32% of OMC and 24% of OME cases.
    2. Attic retraction in OME was more severe in patients with closing disturbance than in those with normal closing function.
    3. Negative middle ear pressure was frequently induced by “sniffing” in patients with closing disturbance.
    4. It was suggested that eustachian tube closing disturbance might be the cause of retraction type cholesteatoma.

    Eustachian tube function was examined in 45 patients with chronic otitis media who underwent middle ear surgery. In 80% of patients, the forced opening pressure of the tube was normal. About half of the patients could equilibrate applied negative middle ear pressure by swallowing. Clearance function as examined by dye technique was not impaired in 70% of patients. Patients with otorrhea showed no statistical difference in ventilatory or clearance function from those with dry ears. However, pathological changes around the tympanic orifice were related to the clearance function of the tube. These results suggest that eustachian tube function in chronic otitis media is not significantly impaired.

    Temporal muscle flaps are usually used for defects of the upper two thirds of the face and anterior skull base after trauma and cancer surgery. A 75-year-old female had complicated defects after intra-arterial infusion for maxillary cancer. Defects of the external auditory meatus and pre-auricular soft tissues were caused by extravasation of anti-neoplasmic agent. A temporal fascio-musculo-periosteal flap with a free skin graft was used successfully to reconstruct the defects.

    Two patients with cancer of the nasal cavity are reported. The patients were a 57-year-old female and, a 54-year-old female. Both tumors developed in the mucosa of the lower turbinate, without one destruction of the nasal lateral wall. They were extirpated by an extended Denker's operation. Histological diagnosis was adenocarcinoma in case one and non-keratinizing squamous cell carcinoma in case two.

    Unilateral sinusitis includes maxillary sinusitis of dental origin, fungal maxillary sinusitis, and acute sinusitis. Microorganisms of 50 cases of unilateral maxillary sinusitis were studied. Eighty-six strains and 24 kinds of bacteria were detected. They included mainly Streptococcus pneumoniae(15.1%), Haemophilus influenzae(12.8%), Peptostreptococcus(10.5%), and Bacteroides (14%). Of the total 86 strains isolated, 41(47.7%)were aerobic and 45(52.3%) were anaerobic.
    Anaerobic microorganisms play a more important role in unilateral maxillary sinusitis than in bilateral sinusitis. This should be considered when selecting antibiotics for the treatment of unilateral maxillary sinusitis

    Twelve patients with chronic sinusitis were treated with classical sinusectomy and eight with antrostomy using the Nd-YAG-LASER technique. In the Iatter group, normal sinus mucosa was well retained and the natural ostium was widely opened by Nd-YAG-LASER. All patients received antral irrigation for 3-4 days after surgery. Seven ofthe l2 patients treated with classical sinusectomy had Ps. aeruginosa infection at the site of operation, while no sinus infection was found in the eight patients with Nd-YAG-LASER antrostomy except in one case in which intensive LASER vaporization of pathological sinus mucosa was performed. Sinus drainage after Nd-YAG-LASER antrostomy appears to be much better than after classical sinusectomy, because mucocilliary clearance plays an important role in the prevention of bacterial infection

    Socio-medical factors in maxillary bone-fractures were investigated statistically in 26 patients. Although incidence of facial irjury increased annually, the increase in the incidence of maxillary bone-fractures for the past 8 years did not parallel that of nasal bone-fractures.
    Surgery for maxillary bone-fbacture should be performed not only for cosmetic plasty but also to improve dysfunction. Our results suggested that sufficient improvements of the maxillo-mandibular dysfunction could be obtained by surgery performed within 2 weeks after the injury, and surgery for the disturbance of eye-movement should be performed in accordance with the indication of operation for the blow out fracture.

    A 12-day-old female was found to have osteomyelitis of the maxilla. Her right eye lids were swollen and reddened, and she had a fever. There was pus exuding from a fistula in the alveolar process of the right maxilla. Culture of pus from this area revealed multi-antibiotic-resistant Staphylococcus aureus. No satisfactory progress was made with antibiotic administration for five days, so surgery was performed with incision and drainage of the right upper eye lid and the right cheek near the inner canthus. On the next day the temperature fell to normal, and within ten days all signs and symptoms subsided.

    From 1973 to 1980, 107 patients with maxillary sinus cancer were treated at Kobe University Hospital. Local recurrence, cervical lymph node metastasis and distant metastasis were reviewed retrospectively. The initial treatment of the primary lesions was of 3 types; 1) total maxillectomy or extended maxillectomy 2) partial maxillectomy 3) conservative therapy. The percentage of local recurrence was; 1) 0% 2) 47% 3) 60%.
    Cervical lymph node metastasis occurred in 26 patients, 21 of whom died from local recurrence and/or distant metastasis.
    Conclusions were as follows; The anterior-inferior-medial type of T3 can be treated conservatively, but the posterior-superior-lateral type requires radical surgery. If cervical lymph node metastasis is present, the patient must be examined carefully for local recurrence and/or distant metastasis. If cervical lymph node metastasis occurs, the prognosis is poor.

    Magnetic resonance imaging (MRI) of the nasopharynx and the eustachian tube was performed in five patients with cleft palate and compared with the results of those without this anomaly. Various degrees of deformity of the eustachian tube cartilage were found in cleft palate patients. The levator veli palatini muscles were situated more laterally in cleft palate patients than in normal subjects. Also, changes in the position of these muscles after palatoplasty were clearly depicted by MRI. Besides several autopsy reports, this is the first demonstration of the characteristic anomaly around the eustachian tube by a non-invasive method.

    Fine-needle aspiration biopsy was performed in 109 patients(143 specimens) with head-neck tumors, and its accuracy and usefulness were evaluated. The total histological accuracy rate was 78.3%, 81.0% for benign tumors and 73.6% for malignant tumors.
    The overall misdiagnostic rate was 21.7%, the false negative rate was 4.9%, and false positive rate 1.4%, with unsatisfactory material accounting for 15.4%. The histological accuracy rate excluding unsatisfactory cases was 92.6%. The accuracy rate of fine-needle biopsy for histological examination of smears will be improved as the number of unsatisfactory cases is reduced. This method is useful for the diagnosis of head and neck tumors.

    The effect of the plasmapheresis using an IBM blood cell separator was evaluated in 5 patients. Three patients with malignant tumor of head and neck were histologically diagnosed as having squamous cell carcinoma, adenoid cystic carcinoma and malignant melanoma, the remaining 2 patients had Wegener's granulomatosis.
    After plasmapheresis, the improvement of performance status was observed in 4 patients, and all patients showed a remarkable reduction of serous immunosuppressive factors. However, these effects were transient and diminished soon after the treatment.
    To maintain the condition improved by the plasmapheresis, other immunochemotherapies should also be given.

    In 11 patients reconstructive oral and pharyngeal surgery was performed with free forearm flaps and a microvascular technique. Four flaps were used for reconstruction of the tongue, four for that of the oropharynx and three for that of the hypopharynx. All flaps survived without any necrosis. Since the forearm flap was thin, soft and pliable, it was suitable for oral and pharyngeal lining. The donor area was covered with a skin graft leaving a scar over the forearm, however, this one-staged, less invasive method was quite useful in oral and pharyngeal reconstruction

    A 36-year-old male had malignant hyperthermia. Laryngomicrosurgery was performed to treat his vocal cord polyp under general anesthesia with GOF and S. C. C. Two hours after the operation, muscle rigidity and myoglobulinuria were noted. Laboratory examination showed high values of CPK, GOT, LDH, GPT and muscle enzymes on the day after the operation. Oral administration of Dantrolene reduced the values of CPK, GOT, LDH and GPT gradually to normal. Muscle rigidity also recovered after three weeks. The possible cause, prevention and treatment of this disease are discussed.

    Cycloheximide(Naramycin®), which is commonly used as an agricultural fungicide, was dissolved in distilled water in a concentration of 0.1%. When mice taste the solution, they show abnormal behavior because of the unpleasant taste. They then refuse to drink the solution because of its specific odor.
    In our experiment, we withheld water for 48 hrs, then offered two bottles of water, one distilled water only and the other 0.1% Cycloheximide. When the mouse chose the distilled water to drink, the response was rated as correct. When the Cycloheximide solution was chosen, an error was recorded. Ten mice were tested ten times during a 10-minute trial, and the percentages of correct responses were recorded.
    1)In the first experiment, there were 79 correct responses in 92 trials(85.9%). In the second and third trial, all l00 responses were correct(100%).
    2)One day after nasal irrigation with a 1% aqueous solution of zinc sulphate, the percentage of correct responses dropped significantly to 72.7% an indication of anosmia.
    3)Seven days after zinc sulphate treatment, the percentage of correct responses started to increase gradually. Twenty days after treatment, it was 98.5%, indicating recovery of olfactory sensitivity.

  • - SEM Observations Using the Back Scattered Electron Mode -

    The present study demonstrates the advantages of back scattered electron images in scanning electron microscopic (SEM) observations of the upper respiratory mucosa. From the atomic number contrast in the back scattered electron signals, the cellular components and their locations can be visualized by staining or incubating with heavy metals.
    The location of the acid glycoconjugates of the basement membrane of the nasal mucosa stained with Ruthenium Red is revealed in the back scattered electron mode and can be represented in a three dimensional structure by color SEM. A paired back scattered electron image and conventional secondary electron image is useful for the observation of the three dimensional structure and cytochemical components of the upper respiratory mucosa.
    Through the back scattered electron mode, cellular activity can be detected. Human neutrophils isolated from nasal secretions were incubated with iron carbonyl particles. The surface structures were observed in the secondary electron image, and the ingested iron carbonyl particles of the same neutrophils were detected in the back scattered electron image. The back scattered electron mode of the scanning electron microscope appears to be very useful in the clarification of the correlation between the surface morphology and phagocytic activity of neutrophils and as a new morphometric indicator of the defence mechanism.

    The long-term therapeutic effects of Dilazep dihydrochroride was investigated in patients with peripheral vestibular vertigo, mainly Meniere's disease and positional vertigo. A total of 45 patients were treated with Dilazep dihydrochroride (300mg/day) for 6 months. The therapeutic effectiveness was evaluated by analysing clinical symptoms and signs. There was subjective improvement in 85.2% of the patients and objective improvement in 74.1 %. The overall rate of improvement was 74.1 %. The frequency of vertigo significantly decreased after the treatment. There were no adverse reaction to this drug. These results indicate that Dilazep dihydrochroride is effective in the treatment of the peripheral vestibular vertigo.

    Thirty patients with abnormal sensation in the throat were given Tsumura-Saikokaryukotsuboreito(No.12)and its clinical usefulness was evaluated. The drug -was remarkably effective in 28% of the patients, effective in 36% and slightly effective in 16% of patients after 4 weeks. It showed good efficacy in the following patients:above 60-year-old female and with II region of CMI health questionaire. One patient complained of slight unpleasant feeling in the abdomen, but this symptom disappeared soon after stopping the administration.
