    It has been generally accepted that Ca2 + plays a significant role in various cellular functions, for example, the release of neurotransmitter. Also in inner ear research, many investigations have been done to clarify the role of intracellular free calcium ([Ca2+]i) on both inner and outer hair cells by recording the intracellular potential using a patch-clamp electrode or measuring [Ca2+]i with microspectrofluorometry with fura-2. From these data, the mechanoreceptive mechanism of hair cells is postulated to be as follows: when the hair bundle is deflected, the transducing K+ channel opens, K+ flows into the cell through the cuticular plate and depolarization occurs. This depolarization leads to an opening of the Ca2+ channels present over the entire plasma membrane and Ca2+ enters the cell from the extracellular space. The increase in [Ca2+]i leads to vesicular fusion and release neurotransmitters. These transmitters excite the neuron, and a message is transmitted to the brain.
    The relationship between [Ca2+]i and the motile responses of outer hair cells and the role of endolymphatic Ca2+ on the whole cochlear function are also discussed.

    It is well known that some patients with acousic neurinoma present with sudden hearing loss as a symptom. However, audiograms specific for acoustic neurinoma have not yet described.
    In our institutes, 12 out of 34 patients with acoustic neurinoma had sudden hearing loss as their chief complaint. Four of these 12 had audiograms indicating sensory neural hearing loss with “valley type” patterns.
    We describe these four patients and urge that any patient with sudden hearing loss who has a “valley type” audiogram be evaluated for possible acoustic neurinoma.

    A patient with chronic otitis media developed sensorineural hearing loss caused by ear drops containing an aminoglycoside antibiotic.
    Our study of this case indicates that the production of ototoxicity by ear drops in patient with chronic otitis media is influenced not only by drug factors such as volume, character, and local factors such as the nature of the otitis media, tubal function, degree of otorrhea, but also by patient factors, such as consciousness of hearing loss and correct understanding of the purpose and use of the otic solution.

    Bone fragments in the nasal cavity are rarely observed. The following two case reports describe a rhinolith and an ectopic tooth in the nasal cavity.
    Case 1. A 68-year-old male with recurrent epistaxis on both sides of the nose had a hard immobile bone fragment in the posterior portion of each side of the nasal cavity X-ray examination showed a transversely placed, sharply defined, homogeneous calcified lesion in both nasal cavities. The calcification was considered to have dropped from the choanae spontaneously. The rhinolith had bone fragments as its nucleus.
    Case 2. A 54-year-old female who had been aware of nasal obstruction, purulent rhinorrhea and epistaxis on the right side for 3 years. Nasal examination showed a hard immobile mass located between the nasal septum and the inferior turbinate. X-ray examination showed a fully developed ectopic tooth on the right side of the nasal cavity. Under local anesthesia, the ectopic tooth was removed by the intranasal approach and it was identified as a canine tooth.

    We treated 24 patients with anosmia caused by traumatic injuries from 1987 to 1990 in the Department of Otolaryngology, Kanazawa University School of Medicine.
    1) Eleven (45.8%) were female and nine (37.5%) were over 50 years of age. The percentage of both females and of aged patients was much higher than in previous reports. It was considered that changes in the social structure might influence the incidence of trauma.
    2) Trauma was frequently observed in the occipital and frontal regions.
    3) There was no definite correlation between anosmia and bone fracture.
    4) Improvement of the olfactory disorder occurred in only two patients (8.3%).

    Bacterial infection via the eustachian tube is thought to be one of the main factors in the development of otitis media with effusion in the non-cleft palate population. It has also been reported that mucociliary dysfunction of the nose and nasopharynx might incease the risk of infection through the tube.
    In the present study, nasal mucociliary clearance was examined by the saccharin test to determine whether it has a role in the development of middle ear diseases in patients with cleft palate. The normal upper limit of the saccharin time was determined to be 11 minutes, as reported previously.
    Ninety-five patients were divided into two groups ; 41 patients with chronic sinusitis or allergic rhinitis and 54 patients who did not have these nasal diseases. In the former group, there was no significant difference in the incidence of prolonged saccharin time (over 11 minutes) between those with middle ear diseases (67.7%) and those without (69.6%). In the latter group, however, the incidence of prolonged saccharin time was significantly Niger in patients with middle ear diseases (66.7%) than in those without (26.8%).
    Thus nasal mucociliary dysfunction appeared to be one of the factors in the development of middle ear diseases such as otitis media with effusion in cleft palate patients, although other factors were also needed for the development of these diseases.

    Mucoepidermoid carcinoma involving the nasopharynx is very rare. A mucoepidermoid carcinoma in the orifice of the eustachian tube of a 66-year-old male was excised with contact YAG laser through a transpalatine approach. The tumor was clearly demarcated and localized within a limited area in the nasopharynx, but radical removal with an adequate margin of safety through a transpalatine approach was very difficult because of the limited field of vision. The development of mucoepidermoid carcinoma from minor salivary glands and surgical treatment of the malignant tumor in the nasopharynx are discussed.

    A 26-year-old man presented with double vision and headache. Neurological examination revealed impairment of cranial nerves II, III, VI. Bony destruction of the sphenoid sinus as well as the sella-clival region was seen on plain craniogram X-ray films. CT scan showed an enhanced mass in the sphenoid sinus. MRI showed a clear localization of the tumor which involved the surrounding tissues. The contents of the sphenoid sinus could be removed by curettage via a transseptal approach. The histological diagnosis was giant cell tumor. The cranial symptoms were relieved within a few days postoperatively. He has remained asymptomatic for 24 months since the operation.

    A patient with primary malignant melanoma of the mucosa of the hard palate was treated with repeated Laser surgery and combination chemotherapy and immunochemotherapy. Laser surgery, other treatments and cytologic diagnosis are discussed. There has been no recurrence of the tumor during the 3 years since the last treatment.

    A 47-year-old female with metastasizing pleomorphic adenoma, in which both the primary tumor and metastases were composed of benign pleomorphic structures, is reported and previously reported cases are reviewed. She complained of nasal bleeding and tumor of the oral cavity. Clinical investigation showed a left upper neck lymph node metastasis. She had been operated on for a hard palate pleomorphic adenoma eleven years before. This time she was treated by operation and postoperative irradiation (6000 cGy). Microscopic examination demonstrated that the primary tumor had mitotic activity and infiltrative growth pattern.
    We emphasize the importance of removal of the primary tumor without destroying the capsule.

    Extracorporeal shock wave lithotripsy (ESWL) using an LT-01 was performed in a 26-year-old female with sialolithiasis of the left submandibular gland. She was placed in prone position with cervical extension. ESWL was performed for 40 minutes without anesthesia at shock wave discharge frequency of 1.25Hz. The salivary stone was totally fragmented during the first lithotripsy procedure, and nopostoperative complications were observed.
    ESWL is a new method of fragmenting salivary stones which may be indicated for stones in the submandibular gland and transitional zone and in some stones in the duct up to about 10mm or less from the transitional zone.

    This is a report of four cases of parotid hemangioma, a rare tumor. The patients were less than 10 years of age; two were infants. Histopathological diagnoses were cavernous hemangioma in two cases, hemangiopericytoma in one and hemangioendothelioma in one. One of the patients with cavernous hemangioma also had hemangiomas in the tongue, oral floor, bilateral submandibular regions, etc. In two patients the hemangioma had invaded the skin above the tumor. Normocytic normochromic anemia was present in two patients due to storage of red blood cells in the hemangiomas.
    Hemangioma of the parotid gland was successfully removed in all cases without permanent facial nerve palsy. We recommend surgical removal for treatment of this tumor.

    A 40-year-old man had noted left upper neck swelling for 20 days. A 6 x 6 cm smooth, hard, fixed mass was palpable on the left side of the upper neck. CT scan did not show a clear boundary between the mass and the sternocleidomastoid muscle. Biopsy was performed, and the microscopic diagnosis was proliferative myositis. Therapy was conservative, and the mass disappeared completely two months after the start of the therapy. The patient is free of disease for more than one year after the therapy. Proliferative myositis is a benign self-limited lesion, so it is very important to distinguish it from malignant diseases.

    We report Castleman's disease in a 48-year-old male who complained of rapidly enlarging mass on the right side on the neck. He presented unusual association of thrombocytopenia and the facio-scapulo-humeral type of progressive muscular dystrophy. The mass was extirpated, and histopathological diagnosis was the hyaline-vascular type of Castleman's disease. Following resection, thrombocytopenia was ameliorated, but progressive muscular dystrophy did not improve. We suggest that the former was the result of the platelet-trapping mechanism often seen in hemangioma, and that the latter was coincidental.
    Immunological study with a panel of monoclonal antibodies and polyclonal antisera was performed to clarify the nature of this heterogeneous condition. This mass was found to be a polyclonal proliferation composed chiefly of B-cells bearing IgD, IgM, and IgA on their membranes. Helper T cells outnumbered suppressor T cells at the ratio of 2: 1. With reference to the recent literature, we suggest that the hyaline-vascular type of Castleman's disease is an atypical hyperplasia possibly resulting from abnormal immunologic response to antigenic stimulation.

    Case 1: A 64-year-old female had esophageal trauma from a fish bone. Dyspnea necessitated tracheotomy. Incision and drainage produced a large amount of purulent material from which Peptostreptococcus was cultured.
    Case 2: A 74-year-old male had esophageal trauma from a fish bone. Incision and drainage produced much purulent material, but no bacteria grew out in cultures.
    Computer tomography was useful in making the diagnosis of abscess. Electronic endoscopy was used to examine the esophageal membrane of the periesophageal abscess. It showed that the anterior wall of esophageal membrane was diffusely swollen, the central region of the swelling was retracted, and a pus plug had been formed.
    The treatment was surgical, in combination with appropriate antibiotics.

    Sterilization of the flexible laryngeal fiberscope was tested with a newly designed washer which uses running water. Its efficacy was studied in 102 patients undergoing endoscopic examinations.
    The main part of the washer was a cloro-vinyl pipe 16 mm in diameter, into which the fiberscope was inserted, and fresh water was kept flowing for 3 minutes. Statistically, 1.65 strains of bacteria were detected at the tip of the endoscopes immediately after endoscopic examinations.
    The incidence of the cases which all strains of bacteria detected before washing were disappeared was 83.3%, and the non effective cases by washing were 9.8%. The cases were 24.5% which a few new strains of bacteria never detected pre-washing were appeared postwashing. These strains of bacteria detected after washing were small in quantity and revealed pathologically low grade. The tips of the nasal endoscopes used in patients with epistaxis all had negative occult blood tests after washing for 5 minutes. The use of the washer was considered to be clinicaly effective in the occasion of the continuous endoscopy.

    We investigated the role of immunological mechanisms in sensorineural hearing disorders and the relationship between inner ear and renal pathologies.
    Controls, and NZB/kl and MRL/lpr strain mice, in which autoimmune disease can be spontaneously induced, were used in this study. The mice were tested for acoustic brain stem response (ABR), cochlear and renal pathology and circulating immune-complexes (CIC) serology, using enzyme immunoassay (ETA). For ABR, click and high frequency tone bursts were used as stimuli. Pathological studies consisted of tissue changes observed following hematoxylin-eosin (HE) and indirect immunofluorescent staining under light microscopy.
    Compared to controls, hearing was impaired in NZB/kl mice while hearing remained unaffected in MRL/lpr mice. On HE staining, no histological changes were observed in the cochleae of either strain of mice. However, indirect fluorescent staining showed IgG deposits in the stria vascularis in both strains of mice. In both strains, renal pathology consisted of an immune-complex glomerulonephritis. CIC values were significantly higher in MRL/lpr mice, but were not correlated to stria vascular lesions. The lesions in the stria vascularis in the two strains were different, which probably explains the discrepancy in hearing impairment, whereas the lesions in the kidney were similar. NZB/kl mice that had hearing impairment showed severe stria vascular lesions and there was a correlation between the degree of hearing impairment and the severity of stria vascular lesions. However, no correlation was seen between stria vascular and renal lesions and CIC values.
    These findings suggest that autoimmune mechanisms exist yin the cochlea, mainly in stria vascularis. The NZB/kl mouse can be a useful model of sensorineural hearing disorder secondary to immunological disorders.

    Human epidermal growth factor (hEGF) stimulates the growth and differentiation of various tissues. To study the kinetics of hEGF in saliva of normal subjects and patients with oral disease, we measured EGF levels in saliva (n=128), urine (n=94), and serum (n=99) using radioimmunoassay. Salivary EGF level showed apparent diurnal rhythm related to taking of meals. Urinary and serum EGF levels showed no obvious diurnal rhythm. There was no significant correlation between salivary and urinary EGF levels, or between salivary and serum EGF levels. Salivary EGF level in adults (10 to 79 years old) was not correlated with age, but it was significantly lower (3.06±0.32 ng/ml) in children (0 to 9 years old) than in adults. Urinary EGF level in children, however, was significantly higher than that in adults. Moreover, there was no significant difference between males and females in EGF level in saliva, urine, or serum. The relative proportions of EGF levels in the submaxillary gland saliva, parotid saliva, and whole saliva were 1: 6: 4. Salivary EGF level of patients with intraoral inflammation (stomatitis aphthosa or peritonsillitis) or intraoral tumor (cancer of the tongue, oral cavity, hypopharynx, or larynx) was markedly low. The pathophysiologic significance of this finding is worthy of study. Low salivary EGF level may reduce the capacity of the oral mucosal defense mechanisms against injury by physiochemical agents.

    To evaluate the clinical efficacy of Tsumura-Saireito, an open trial study was performed on 45 ears of 33 patients with otitis media with effusion. After 4 weeks of application, marked effects were observed in 2 ears (4.4%), moderate effects in 13 (28.9%) and slight effects in 22 (48.9%), while no effects were seen in 8 ears (17.8%). On the pure tone audiogram, 59.0% of the ears showed improvement, and 47.3% on the tympanogram. Clinical findings of the eardrum were also improved in 26.6% of the ears. Only one patient complained of mild abdominal pain as a side effect. Clinical efficacy was improved according to application duration. Younger cases showed better efficacy compared with older cases. These findings indicate that Tsumura-Saireito is effective in otitis media with effusion.

    Banan® (cefpodoxime proxetil CPDX-PR) was administered to 177 patients with infections in the department of otorhinolaryngology, and its clinical and bacteriological effects, and its concentration in tissue and blood were determined.
    The clinical effect was 90.5% for acute otitis media, 59.0% for chronic otitis media, 92.0% for acute sinusitis, 84.2% for chronic sinusitis, and 88.2% for tonsillitis. The bacteriological bacterial elimination rate was 82.4% for acute otitis media, 62.5% for chronic otitis media, 88.9% for acute sinusitis, 64.7% for chronic sinusitis, and 91.3% for tonsillitis.
    The concentration of the drug in tissue was 0.25 to 0.70μ/ml, and the concentration in blood was 0.12 to 2.45μg/ml.
    Side effects were not observed in any of the cases.
