    The main function of the teeth is mastication as a digestive organ. Teeth are thus required to have a structure hard enough for the chewing and crunching of food as well as the toleration of strong forces of mastication. Diseases peculiar to the teeth and periodontal tissues are observed in the oral region. Clinicians should not diagnose and treat oral
    diseases without having a knowledge of dental diseases. In this paper we describe the odontogenesis and structure of teeth, dental infections, and odontogenic tumors and cysts.

    A 40-year-old female was operated on for nasal polyps, but the tumor had extended already into the cranial space. Olfactory neuroblastoma was diagnosed. After chemotherapy we conducted transnasal dissection for the skull base lesion and performed craniotomy for the intracranial one. Postoperative irradiation of 50 Gy was given. Although no local recurrence or distal metastasis has been continuing found during the four years since treatment, careful follow up is necessary because of the high rate of local recurrence.

    A 51-year-old male with a history of palsy and atrophy of the right side of the tongue was admitted to our clinic in April, 1991. General physical examination revealed nothing remarkable, and no cafe au lait spots were found. Ultrasound examinations delineated the size and boundary of the neck mass and its relationship to the great vessels. MRI in short spin echo revealed a low density tumor mass. A 3-dimension CT scan showed destruction and enlargement of the jugular foramen and hypoglossal canal. CAG demonstrated a hypovascular mass and no tumor stain. A biopsy was performed via the oral cavity, and the histopathological report was Antoni type B neurinoma, without malignancy.
    On July 1991, mastoidectomy and suboccipital craniotomy were performed with a staged extracervical approach. The 10th and 11th cranial nerves were intact, but the 12th nerve was involved in a massive cystic tumor which extended intracranially through the enlarged jugular foramen and hypoglossal canal of the skull base. The extracranial 9th nerve was cut because it was involved in the neck mass. Intracranial hypoglossal neurinoma is a rare lesion, and its dumbbell-like extension to the neck is even more rare.
    The best treatment of this tumor is total removal. However when it is strongly adherent to the medulla, subtotal removal is essential because of the risk of brain damage, since this tumor is benign.

    Recent advances in imaging technique, including high resolution thin section computed tomography scanning and magnetic resonance imaging (MRI), permit the easy diagnosis of congenital malformations of the osseous labyrinth, which have so far been lumped together as "Mondini dysplasia". In the present study the anatomic patterns from the radiographic appearance and the clinical manifestation were examined in 18 patients (23 ears) with radiographic abnormalities of the inner ear. The most common abnormal configuration was a large vestibule (20 of 23 ears). This anomaly of the vestibule often involved the lateral semicircular canal, revealed as a round high signal zone on T2-weighted MRI. However, a large vestibule was not always associated with an abnormal cochlea. Abnormal cochleas were found in 8 of the 20 ears with a large vestibule, and most of these 8 ears had total or profound deafness. But 4 of the 10 ears with residual hearing had low tone deafness and 6 had fluctuating hearing loss, which was frequently associated with attacks of dizziness. These clinical manifestations of Mondini dysplasia are similar to those of patients with endolymphatic hydrops.

    Myringoplasty in the office was performed in 92 patients (97 ears) with the subepithelial connective tissue as the autograft attached with fibrin glue (Beryplaste P).
    Subepithelial mesenchym was used for the autograft. In 87.6% of the ears with perforation, operation was successful with the first myringoplasty. In 94.8% of ears the perforation could be closed by the first to the third operation.
    We investigated the correlation between the width or localization of the perforation and the rate of closure.
    The results showed that large and posterior drum perforations with exposure of the malleus were more difficult to close than other perforations. The activity of factor XII in the plasma of patients with perforation of the tympanic membrane tended to remain lower than in normal subjects (70%).

    Synkinesis between facial mimetic muscles is most frequently observed in sequelae of facial palsy as compared with crocodile tears, spasms, contractures and aural symptoms. Aural symptoms, such as tinnitus, ear fullness, and hearing loss are considered to be due to abnormal associated movements between facial mimetic and stapedial muscles. We examined the incidence of the synkinesis between facial mimetic and stapedial muscles as sequelae of facial palsy.
    To detect these phenomena objectively, the changes in acoustic compliance of the middle ear during voluntary contraction of mimetic muscles were measured in 31 patients who were in a post therapeutic state of peripheral facial palsy.
    In total, there were 24 patients with any type of sequela. The synkinesis between facial mimetic muscles was most common and observed in 20 cases. Abnormal associated movements between facial mimetic and stapedial muscles were observed in 16 (66.6%) of the 24 sequelae cases, and in 15 (75.0%) of the 20 in the facial mimetic synkinesis cases. The incidence was higher than that previously reported.

    A 39-year-old man with dull pain in both eyes visited our clinic. Magnetic resonance imaging (MRI) revealed a midline cyst in the posterior wall of the nasopharynx which had high intensity signal characteristics in both T1-and T2-weighted pulse sequences. After removal of the anterior wall of the cyst, his pain disappered, and he has been well since. Histological sections revealed epitheliallined cyst with no evidence of lymphoid tissue, consistent with Tornwaldt's cyst.
    Tornwaldt's cyst results from the embryological relationship of the notochord and the pharyngeal endoderm.
    MRI is well suited to distiguish this cyst because of its greater soft-tissue contrast resolution and of the anatomic advantage obtained by multiplanar views.

    Angiofibroma in the nasal cavity is very rare.
    Our patient was a 28-year-old female with right nasal obstruction and tearing of the right eye. A nodular mass was seen ocupying the right inferior meatus. X-ray examination showed clear paranasal sinuses.
    Under general anesthesia, the tumor was removed with contact type Nd-YAG laser through a perimaxillary approach. The tumor, which originated from the posterior half of the right inferior nasal turbinate, measured 28 × 17× 7mm.
    The histopathological diagnosis was angiofibroma.
    The patient was followed for 28 months after the operation and showed no recurrence of the tumor.

    The improvement of the sense of smell in patients with chronic sinusitis and olfactory disturbance was assessed pre- and postoperatively by T & T olfactometry and the intravenous Alinamin® test.
    Overall percentage of improvement was 62.3%. Patients who did not respond to the Alinamin® test preoperatively did not recover the sense of smell after surgery. The percentage of improvement was higher in patients with initial operation than in those needing reoperation. The percentage of improvement was lower in those with recurrent polyps and/or purulent rhinorrhea after surgery. Surgery of the middle turbinate resulted in the highest percentage of improvement. There was no correlation between the degree of opacification of the ethmoid sinus seen on X-ray films and the degree of improvement.
    It was concluded that special care is necessary in the treatment of patients needing reoperation. Furthermore, trimming of the middle turbinate, if pathological change is present, can improve the sense of smell if olfactory disturbance is caused by chronic sinusitis.

    A case of adenosquamous carcinoma of the maxillary sinus is reported. A 70-year-old man with adenosquamous carcinoma in the maxillary sinus was treated with radiotherapy, chemotherapy and radical surgery. Adenosquamous carcinoma is very rare in the paranasal sinuses. The prognosis is extremetly poor, similar to that for undifferentiated carcinoma. Radiation therapy and chemotherapy evidently have little ability to cure this tumor. We believe that the treatment of adenosquamous carcinoma requires aggressive
    and radical surgery.

    The incidence of neurinoma in the oral-mandibular region is relatively low. It is usually seen in the tongue. We treated a patient with neurinoma of the floor of the oral cavity-a rare site for neurinoma.
    An 83-year-old woman complained of a mass on the floor of her mouth. Examination on her first visit revealed a firm elastic mass the size of an egg on the floor of her oral cavity. There was no tenderness, and no difficulty in speaking, chewing or swallowing. Examination of a biopsy revealed Antoni type A neurinoma.
    The tumor was removed under general anesthesia. No adhesion to surrounding tissue were encountered, and the mass was easily resected by extra-capsular dissection. S-100 protein staining was positive within the tumor cells, leading to the immunohistochemical diagnosis of neurinoma.

    In a 10-year-old girl, computed tomography showed a lobulated encapsulated mass in the masseter muscle. The mandible was greatly deformed by the tumor. Although benign tumor of the masseter muscle was suspected, we could not rule out malignancy. Histological examination of frozen sections revealed hemangioma. The tumor was removed totally without complications. In this patient, the extraoral approach, which provides wide exposure and facilitates total removal of the tumor, seemed more suitable than the intraoral approach. The characteristics, clinical features and treatment of intramuscular hemangioma are discussed.

    A 6-year-old boy had noticed a soft mass on the right side of his neck and pururent drainage from small openings on the left side of the neck and both preauricular regions. A cystic mass and three fistulas were removed surgically. Microscopic examinations showed that the cervical cyst had cilliated columnar epithelium and the cervical columnar epithelium.
    Thirteen cases, similar to ours, have been reported in Japan. These cases are reviewed and their clinical features are compared.

    Between 1983 and 1991, 18 patients with a clinical diagnosis of primary hyperparathyroidism (PHPT) were treated in our department.
    In this series, there were 5 men and 13 women, between 19 and 78 years old (a mean age of 48 years). According to histological type 16 were adenomas (89%), and 2 hyperplasias (11%).
    In preoperative localization studies (imaging), the sensitivity for detection of abnormal parathyroid glands was 88.9% on ultrasonography (US), 88.2% on computed tomography, 64.7% on T1-Tc subtraction scintigraphy and 76.5% on angiography. US is the most useful imaging in PHPT.

    In adult humans, hypothyroidism has been associated with a reduced sense of smell. There are few reports on the effect of hypothyroidism on the sense of smell in adult animals. We examined whether hypothyroidism abolishes the sense of smell in adult mice and examined the changes of the olfactory epithelium. Hypothyroidism was produced in adult mice by addition of thiamazole to their drinking water or intra-peritoneal injection. The olfaction of mice was estimated by successful recording of behavioral task, using cycloheximide, which has been commonly used as a repellent in rodents. The sense of smell was tested before, during, and after thiamazole treatment. Thyroid function was assessed by radioimmunoassay of blood serum for free thyroxine. The mice were killed after treatment and the olfactory epithelium was studied at the light microscopic level to determine the effects of thiamazole treatment.
    Mice given 0.01% thiamazole in the drinking water for 100 days were not hyposmic. The control mice given tap water to drink for 100 days maintained their sense of smelll. Mice given daily intra-peritoneal injection of 0.5 mg thiamazole for 100 days were not hyposmic. Daily i. p. injection of 1.5mg thiamazole to the mice for 20 days attenuated their sense of smell. By contrast, the control mice, given daily i. p. injection of 0.1ml normal saline were normosmic. In the olfactory epithelium of the hypothyroid and hyposmic mouse, the structures composed of basal cells, olfactory cells, and supporting cells were preserved. However, a marked decrease was observed in the number of olfactory cells. These findings indicate that hypothyroidism produces olfactory disturbance in mice and injures the olfactory receptor neurons.

    It has been reported that Shin'i-seihai-to and Sho-seiryu-to, which are Kampo (Chinese herbal) medicines, are effective for sinusitis and nasal allergy, respectively. It has been suggested that excessive active oxygen species are probably involved in the pathogenesis of chronic sinusitis and allergic diseases.
    In this study, the effect of Shin' i-seihai-to and Sho-seiryu-to on the production of active oxygen species by human neutrophils was investigated with an in vitro luminol-dependent chemiluminescence method and a cytochrome-C reduction method.
    Neutrophil production of active oxygen species induced by FMLP (formyl-methionyl-leucyl-phenylalanin) was significantly inhibited by Shin' i-seihai-to and Sho-seiryu-to in a dose dependent manner. The inhibition by Shin' i-seihai-to occurred at lower concentration than that by Sho-seiryu-to.
    Neutrophil production of active oxygen species induced by opsonized zymosan was not changed by Shin'i-seihai-to.
    Superoxide dismutase activity by Shin'i-seihai-to and Sho-seiryu-to could not be demonstrated by the cytochrome-C reduction method.
    These results indicate that inhibition of neutrophil production of active oxygen species by Shin'i-seihai-to and Sho-seiryu-to is one of the pharmacological properties of these two Kampo medicines.

    The therapeutic effect of sulpiride was studied in 33 patients with vertigo, dizziness, and a tendency of depression.
    They were given oral sulpiride, 150 mg daily, and the subjective symptoms and objective findings were recorded before and two and four weeks after the start of treatment. The efficacy of the drug was evaluated; 7.4% showed marked improvement, 51.9% marked or moderate improvement, and 100% more than slignt improvement was seen in the fourth week. Many subjective symptoms, such as nausea and vomiting, headache, loss of appetite, stiff shoulders, general fatigue, obstructive sense of ear, as well as vertigo and dizziness, were also reduced.
    Righting reflex and gaze and positional nystagmus were significantly improved.
    The scores of SDS and CMI decreased markedly at the second and fourth week.
    Only four patients complained of side effects, which were liver dysfunction, and galactorrhea.
    In conclusion, sulpiride is useful in the treatment of vertiginous patients who have a tendency of depression.

    The efficacy of tranilast, an antiallergy agent, in treating otitis media with effusion (OME) was examined by analyzing its effect on prostaglandin synthesis. In 54 patients with OME (54 ears), tranilast was administered for 14 days after elimination of middle ear effusion with tympanotomy. Forty-four ears showed no recurrence of effusion after tranilast therapy, while 10 ears developed effusion again. In these 10 ears, the levels of two prostaglandins (TXB2 and PGD2) in middle ear effusion were higher after tranilast therapy than before tranilast therapy, which suggested that prostaglandins are associated with intractable OME. The administered tranilast was transferred into the middle ear effusion effectively.

    The distribution of ofloxacin (OFLX) in the tonsil and paranasal mucosa was quantitatively analyzed. OFLX was administrated 200 mg orally 3'1-4 hours before surgery. Then blood samples and tissue specimens were obtained simultaneously at operation. The OFLX tissue level proved to be related to the serum level. The tissue concentration of OFLX in the tonsil was 2.70 ±0.97ug/g and was higher than the serum concentration (1.90±0.57ug/ml). The tissue/ serum ratio was 1.44. On the other hand, the tissue concentration in the paranasal mucosa was 1.90±0.55ug/g. There was no significant statistical difference in OFLX concentration between paranasal mucosa and serum. The reason why penetration of OFLX to the tonsil exceeded that to the paranasal mucosa was discussed in the text. These tonsil and paranasal mucosa concentrations of OFLX were higher than the MIC50 of OFLX for bacteria that were found in common infections of both tonsil and paranasal mucosa. These kinetics of OFLX was not altered in elderly patient. These findings suggest that OFLX is a useful antibiotics which should be used for tonsillitis and sinusitis.

    Infections constitute a high percentage of the diseases treated in the field of otorhinolaryngology. We examined the efficacy and safety of Banan® and the trends of the organisms causing infections in 300 patients. A total of 262 strains of organisms were detected and S. aureus was predominant. The efficacy rate of Banan® was 89.5% for acute infections, 62.8% for chronic ones, and 76.5% overall.
    Side effects occurred in 2.7% of the patients and the majority of them were gastrointestinal disorders.
    Thus, we concluded that this drug was useful in the field of otorhinolaryngology.
