    The possible roles of several hormones such as aldosterone, vasopressin, atrial natriuretic peptide (ANP) and catecholamines in the inner ear are reviewed on the basis of recent research results, including our data. The presence of a mineralocorticoid type I receptor, Na-K ATPase and amiloride-sensitive Na+ channels in the cells involved in the secretion and absorption of endolymph, such as the marginal cells, suggest that aldosterone may control endolymph secretion and absorption. The involvement of vasopressin in the homeostasis of the endolymphatic system, which has been suggested by several lines of evidence, remains to be clarified. Recent results have also suggested that ANP may control the secretion and absorption of endolymph in the inner ear, regulate cochlear nerve function, and control inner ear blood flow. Finally, recent evidence also suggests that catecholamines may regulate the function of the endolymphatic sac.

    Speech discrimination and hearing aid performance in patients with hearing loss have been evaluated by speech audiometry where a tape or CD is utilized as a sound source. Since all sound samples in a list on the tape or CD are consecutively presented to the examinee, this conventional method provides the perception rate of the whole list. It has been difficult however, to evaluate the perception of a specific sound in the list.
    We developed a new method of interactive speech audiometry. Using this method, the patient's response to each sound sample can be monitored at any required sound level. All sound samples such as the 5 vowels, 13 consonant-vowel syllables, and sentences are stored in a computer as sound files. Any sound sample can be randomly selected to present to the examinee, who replies verbally. Interactive speech audiometry can provide a more accurate evaluation of speech discrimination in patients with hearing loss.

    Twenty-three cases of BPPV were treated by Epley's canalith repositioning procedure (CRP). The efficacy of CRP was evaluated by changes in symptoms and the nystagmus. In all patients, both vertigo and nystagmus disappeared after one or two sessions of CRP. There was no recurrence. These results indicate the validity of canalolithiasis as a possible mechanism of BPPV.

    Hemangiomas are common benign tumors of vascular origin, and rarely progress to malignant transformation. We experienced a case of a hemangioma which showed malignant transformation. A 69 year-old female was admitted to the hospital due to right nasal bleeding. The tumor was then recognized deep in the right nasal cavity. The tumor was surgically removed with an endonasal procedure. The pathologic diagnosis of the resected tumor was a hemangioma. The tumor then recurred twice in the same place after the initial resection. Finally, the tumor was resected from the lateral wall of the nasal cavity. The histopathological findings of the resected specimen demonstrated an angiosarcoma in the hemangioma. There has been no recurrence now for 17 months after the surgery. Based on the clinical progress and histopathological findings, this case was considered to be a hemangioma which underwent malignant transformation into an angiosarcoma.

    In unilateral sinusitis, the walls of the affected maxillary sinus show a thickening on CT scanning in contrast with the non-affected sinus. It is known that this sinus wall thickening is mainly caused by CT artefacts.
    In this report, we investigated the thickness of the anterior and posterior bone walls of the maxillary sinus in 37 cases of unilateral sinusitis (15 cases of bacterial sinusitis and 22 cases of fungal sinusitis) by means of orthopantomography, soft tissue density CT and bone density CT scans. In addition, we investigated the bone mineral density (BMD) of the maxillary sinus walls using quantitative computed tomography (QCT). On the soft tissue CT scans, 30% of the cases showed posterior wall thickening and 60% of the cases showed anterior wall thickening due to the CT artefact. If we evaluated the thickness of the walls by a bone density CT scan, we believe that bone density was actually improved.
    In fungal sinusitis, 80% of the cases showed thickening of the posterior walls and a higher BMD. This suggests that the sinus wall thickening in fungal sinusitis was due to sclerotic changes.

    A case of neurinoma from the infraorbital nerve is reported. A 42-year-old man complained of right cheek swelling and numbness. Rhinoscopic examination was normal, while CT scanning revealed a solitary mass located near the infraorbital foramen. Surgical resection of the tumor was performed under general anesthesia, and the tumor was successfully removed. Histological examination revealed a neurinoma (Antoni AB type). The postoperative recovery was uneventful, with no signs of recurrence for 8 months.

    Calcifying epithelioma (pilomatrixoma) is an uncommon benign tumor arising from the hair matrix cells. It often affects the face, neck and upper extremities. A 25-year-old female with a calcifying epithelioma in the preauricular region is reported. It was not difficult to diagnose the tumor as a calcifying epithelioma by soft X-ray, echography and CT scan before surgery because of the calcification. CT scan was particularly useful in the diagnosis.

    Primary small cell carcinomas in the oral cavity are extremely rare. A 72-year-old man with a small cell carcinoma of the oral cavity was treated surgically with neoadjuvant chemotherapy, followed by modified neck dissection and adjuvant chemotherapy.
    The chemotherapy ordinarily given for pulmonary small cell carcinomas was effective in this case.
    He has been without disease for 14 months after the initial diagnosis.

    Hypopharyngeal neurinomas are rare. A 74 year-old male with a hypopharyngeal neurinoma is presented in this report. The patient had no symptomatic complaints, and the tumors were found incidentally. Laryngoscopy was then performed, and a neurinoma was found which had a smooth submucosal surface, was round and was located on the left posterior hypopharyngeal wall. A CT scan showed the mass on the left side of the hypopharynx, with an irregularly-enhanced isodensity portion. MRI imaging was very useful in the preoperative diagnosis, providing much information concerning the extension of the tumor, which could not be obtained by CT.

    Three hundred and ninety-three patients (292 male and 101 female) who visited our clinic complaining of snoring and/or apnea during sleep were evaluated for sleep apnea syndrome (SAS) by Apnomonitor screening. A total of 131 patients (114 male and 17 female) showed an apnea index (A. I.) higher than to apnea episodes/hour, and there was a significant correlation of the A. I. with age, sex, and body mass index (BMI) in these patients. All patients with severe SAS (more than 50 apnea episodes/hour) were men, who had a BMI greater than 25, with the exception of 1 patient. Significantly higher ages and BMI were found to exist in patients with SAS than in those without SAS.

    Three cases, who underwent Isshiki Thyroplasty type I, because their vocal function had not improved with a previous transcutaneous intrafold silicone injection, were reported. Their poor outcomes with previous treatment were thought to result mainly from an insufficient volume of injected silicone and/or an inappropriate portion of silicone in the larynx.
    In all three cases, the so-called manual compression test of the thyroid cartilage ala preoperatively revealed drastic improvement of their voice. During operation, however, their vocal fold tissues were too stiff to medialize the vocal fold, resulting in poor postoperative vocal function in first two cases. Based on these results, in the third case the inner perichondrium was raised more widely than usual and a larger silicone block was inserted with removal of the cartilage fragment, resulting in a moderate improvement in vocal function.
    It seems that after intrafold silicone injection fibrotic changes may occur widely in the vocal fold tissue histopathologically, causing subsequent difficulty in medialization of the vocal fold by the usual Isshiki Thyroplasty type I.

    Between 1992 and 1996, 31 cases (8 men and 23 women) with primary hyperparathyroidism (PHPT) were treated in our department. In this study, we compared which of the preoperative localization methods was most useful in the detection of PHPT. The sensivity for detection of abnormal parathyroid glands was 88.6% on ultrasonography (US), 76.9% on magnetic resonance imaging (MRI), 74.3% on T1-Tc subtraction scintigraphy and 68.8% on computed tomography.
    We concluded that US should be performed first, with MRI as a supplementary method, for the detection of abnormal parathyroid glands and the evaluation of invasion within the body.

    We studied the expression of p 21, the ras encoded protein, in 30 specimens of thyroid carcinomas obtained from 29 patients. The presence of p 21 ras protein was evaluated by immunohistochemistry using a specific monoclonal antibody (MAb Y-13259). The evaluation was compared with various prognostic factors such as histological type, sex, age, extent of tumor invasion, tumor size and lymph node metastasis. Our data indicated that the incidence of p 21 positive expression in papillary carcinoma was higher than in other types of thyroid cancer, and that tumors limited in extent of tumor invasion and smaller in size typically expressed p 21. In conclusion, our results suggested that expression of p 21 might be related to cell proliferation in the early stages of neoplastic transformation.

    Thymic carcinoid is a rare tumor. The case of an 86-year-old male with a thymic carcinoid is presented. The patient was discovered to have an abnormal mass, as an incidental finding, at the thoracic inlet during a routine examination. He had slight dysphagia for 6 months. The tumor had displaced the trachea to the right, and compressed the esophagus. A CT scan demonstrated that the tumor was located in the superior mediastinum, and extended to the aortic arch. The preoperative diagnosis was a mediastinal goiter. The tumor was resected completely through a general collar incision, without a median sternotomy. The tumor measured 6.5×5.5×4.5cm in size, was grossly well-encapsulated and was diagnosed histologically as a carcinoid tumor. Mitoticfigures were not seen. The tumor cells had a few argyrophillic granules, and showed positive immunoreactivity for chromogranin A. Electronmicroscopy disclosed small granules with diameters of about 160nm. There were no clinical manifestations suggestive of a specific syndrome, such as a carcinoid syndrome. The patient recieved no postoperative radiation or chemotherapy, and is now quite well at 2 years after the operation.

    Bell's palsy is the most common cause of peripheral facial nerve paralysis. Although herpes simplex virus type 1 (HSV-1) infection has been strongly suggested as a cause of Bell's palsy, the pathomechanism of the facial nerve paralysis is unclear. Previously, we had succeeded in producing an animal model of acute, transient and homolateral facial paralysis by inoculating HSV-1 into the auricle, simulating Bell's palsy. To clarify the mechanism of this facial nerve paralysis, electrophysiological testing of the trigemino-facial reflex (blink reflex) and electroneuronography (ENoG) were carried out, and a histopathological study of the facial nerve was subsequently performed. The blink reflex and ENoG were examined twice ; during facial paralysis on day 10 and after recovery on day 17. The R1 latency of the blink reflex was prolonged or disappeared on the paralyzed side during facial paralysis, but recovered in all animals with the corresponding recovery of facial nerve paralysis. ENoG values were inconsistent during facial nerve paralysis and did not normalize even after complete recovery of the facial nerve paralysis. The histopathological studies demonstrated that mixed nerve damage, demyelination and axonotomesis, were present, although demyelination was dominant. Collectively, the electrophysiological and histopathological findings suggested that the pathomechanism of facial nerve paralysis caused by HSV-1 infection is mainly due to demyelination of the nerve, which was represented as a conduction block in electrophysiological testing. The present study suggested that the facial nerve damage underlying Bell's palsy involves mixed nerve damage including demyelination and axonotomesis, and the prognosis of facial nerve paralysis is dependent on the balance of these two kinds of nerve damage.

    We studied the clinical effectiveness of Lipo-prostaglandin E1 (Lipo-PGE1), which has vasodilating activity on microvessels and anti-platelet properties, in patients with sudden deafness. Fifty-five patients were treated with steroids (group I ) and forty-three patients were treated with Lipo-PGE1 plus steroids (group II). In group I, 10 patients (18.2%) presented with a complete recovery, 9 patients (16.4%) presented with a marked improvement, 20 patients (36.4%) presented with an improvement and 16 patients (29.1%) presented with no change. In group II, 19 patients (44.2%) presented with a complete recovery, 4 patients (9.3%) presented with a marked improvement, 9 patients (20.9%) presented with an improvement and 11 patients (25.6%) presented with no change. It was concluded that combined therapy with steroids plus Lipo-PGE1 was more effective in patients with sudden deafness, especially in severe cases.
