    To investigate the factors that contribute to the early detection of cancers, we reviewed retrospectively the process from the initial symptom to the establishment of the final diagnosis in 160 patients with head and neck cancer encountered at Kanazawa University Hospital between 1974 and 1986. Malignancies occurred in the nasopharynx in 8 cases (5%), hypopharynx in 10 (6.3%), larynx in 73 (45.6%), paranasal sinuses in 29 (18.1%), and oral cavity in 40 (25%). The ages ranged from 9 to 88years (mean age 61.5 years).
    Although 89 out o f 160 patients consulted a doctor within one month after the onset of symptoms, a definite diagnosis was not made within one month. Except for patients with laryngeal cancer, more than half of the patients had first visited a doctor other than an otolaryngologist, and had recieved inappropriate treatment for a long time because of misdiagnosis.
    From these results, we stressed the following points for early diagnosis of head and neck cancer: 1) it is very important for the otolaryngologist to suspect head and neck cancer: and 2) the otolaryngologist should enlighten the general public and doctors in other field on the initial symptoms of head and neck cancer.

    Thirty-three patients with malignant maxillary tumor, who were seen over 7 year period, are reported.
    As the p atients were unaware of the presence of the tumor, they did not seek early treatment. Thus, the diagnosis of the tumor at an early stage was usually delayed. The patients were initially diagnosed as having chronic maxillary sinusitis. The patients had consulted many doctors before being admitted to had our hospital; 59% had previously consulted an otolaryngologist. X-ray examination for the patients detected insufficiently the maxillary bone destruction caused by the tumor: therefore we may overlook the tumor.

    Problems in early detection and early diagnosis of nasopharyngeal cancer (NPC)were examined. The early detection of NPC is very difficult, because there are many problems, such the wide range in age of onset, various symptoms and difficulty in the visual examination. But endoscopy and serological study (Epstein-Barr Virus) were useful for early diagnosis of NPC. For early diagnosis of NPC, it is very important to suspect NPC.

    From May to July in 1984, we conducted a mass examination for early detection of glottic cancer and other diseases of the vocal fold in cooperation with eight health centers, twenty four E. N. T. clinics and the local press in Fukui prefecture.
    Six hundred and eleven patients, who complained of hoarseness, un d erwent the examination. They consisted of 343 males and 268 females. Age distribution of the patients was 68 in thirties,211 in fourties,200 in fifties,75 in sixties,50 in seventies,6 in eighties and 1 in nineties.
    Seven males (1.2%) wit h glottic cancer and eight males with precancer (seven with hyperkeratosis and one with leukoplakia) were found among the 611 patients.
    Among these laryngeal cancers, six cases were Tl a of the early stage and one was T3according to the classification of International Union Against Cancer. Among the other 596 patients,296 had chronic laryngitis,60 acute laryngitis,66 laryngeal polyp,41 Reinke's edema and 55 laryngeal nodule.
    Our mass examination was useful for the early detec tion of glottic cancer.

    We retrospective reviewed 139 previously untreated patients with carcinoma of the hypopharynx and carvical esophagus who were seen between 1965 and 1984. The clinical stage was closely related to the prognosis. The prognosis of the hypopharyngeal and cervical esophageal cancer was poor, mainly because the patients who visited us already had advanced diseases. Therefore, we examined the initial sign and final diagnosis of these patients. For earlier diagnosis, screening of the high risk group complaining of throat discomfort should be routinely performed.
