    The protective effect of sodium thiosulfate (STS) against ototoxicity of cisplatin (CDDP) was studied in the guinea pig.
    CDDP at a dose of 7.5 mg/kg/day was intramuscularly given 3 times a day on every 5th day. The animals in Group 1 were intraperitoneally given 1060 mg/kg STS,5 minutes and 1 hour after each administration of CDDP, and those in group 2 were given STS,3 and 6 hours after each CDDP administration. According to the number of surviving hairs of the outer hair cell (OHC) observed by scanning electron microscopy, OHC damage in the animals in group 1 was very slight compared with that of those given CDDP alone. The protective effect of STS in group 2, was lower than that in group 1. Therefore, two-channel chemotherapy is clinically useful for the protection of CDDP ototoxicity.
    The detoxicant effect of STS through the round window was examined. CDDP of 5 mg / kg / day was intramuscularly given for 3 consecutive days, and 0.1 ml of 10% STS was applied into the tympanic cavity through a silicon tube inserted into a hole on the bulla, soon and 30 minutes and 1 hour after the injection of CDDP. OHC damage in the right ear with STS infusion was slight, compared with that in the left ear which had not been infused STS. Intratympanic application of STS may be a means of redusing CDDP ototoxicity.

    The specificity of the staining of cochlear hair cell stereocilia according to Santi's method was investigated. The inner and three rows of the outer hair cell stereocilias were well stained by alcian blue and were easily visualized against the background. The affinity between the cations of the dye and polyanions of the sulfated compounds in the stereocilia produce this phenomenon.
    We also examined the staina b ility of other cationic dyes such as neutral red and toluidine blue. Although the inner and outer hair cell stereocilia were clearly stained by neutral red, toluidine blue showed less stainability for those tissues than the other two cationic dyes. Further investigations on pH, electrolyte concentration, and temperature on staining are necessary.
    This method of cochlear hair cell staining will be helpful not only for the morphological analysis but also for functional analysis of the stereocilia.

    The outcome of tinnitus therapy, systemically performed in our clinic, was reviewed.
    Lidocain was intravenously injected into 247 patients who complained of tinnitus. Fifty two (21.0%) of them had complete relief from tinnitus,126 (51.0%) had partial relief (over 30% disappearance of tinnitus by subjective evaluation).
    Forty one patients were given sodium valproate (400 mg orally twice a day, Group 1), thirty eight patients were given mexiletine HC1 (300 mg orally three times a day, Group 2), and sixty nine patients were given eperisone HC1 and minor tranquilizer (each one tablet orally three times a day, Group 3). All patients in Group 1to 3 were given the drugs after being tested for the effects of intravenous lidocaine (40-60 mg). The results obtained for Group 3 were the most correlated with the effect of lidocaine.
    Twenty four ears were treated with transtympanic injection of dexamethason. In 12 ears, tinnitus was improved (30-80% subjective disappearance).
    Twenty four patients were treated with band noise ma s kers, and 10 of them (42%)had partial and complete relief from tinnitus.
    It was concluded that these different therapies should be used in combination to obtain better relief from tinnitus.

    We examined the effectiveness of masking therapy for tinnitus by measuring the residual inhibition (RI) induced by acoustic stimuli from a commercial music synthesizer, as well as by pure tone and band noise.
    A personal computer was used to control the music synthesizer. Parameters concerning RI analyzed were masking loudness, masking instrument (sound generator) and duration of RI after repetition.
    The music synthe s izer was the most effective sound source for masking, except in patients who showed fatigue of masking by repetition test. Masker therapy was effective in 47% of the patients with tinnitus caused by various kinds of ear disease. Tinnitus with a small minimum masking level and with long RI was inhibited effectively by masking therapy.
    Side effects of masking therapy, noted in 20% of the patients were increase of tinnitus, hearing impairment and headache.

    We performed the air caloric test in 22 normal ears, and confirmed that changes in the maximum slow phase velocity are nearly linear with changes in the air temperature between 10 and 50°C.
    From the velocity-temp e rature curve obtained by the above evaluation, the conditions of 15°C air,61/min, and 60 seconds were considered to be optimal for producing responses similar to those obtained when using 5 ml water at 20°C infused over 20 seconds. The results of the air caloric test and the water caloric test performed under these conditions in patients with vertigo were comparable. Judging from these findings, the air caloric test is considered to be a useful examinations of vestibular function.

    We report 2 cases of meningitis and temporal lobe abscess of otogenic origin. In case 1, a 52-year old man was admitted to our hospital for abrupt hea d ache and right otalgia. He was diagnosed as having acute meningitis secondary to cholesteatoma. He was cured by intravenous antibiotic therapy and removal of cholesteatoma by the intact canal wall tympanoplasty (staged tympanoplasty). In case 2, a 54-year old man was diagnosed as havin g chronic mastoiditis and received tympanoplasty, but 9 days after the operation, a CT scan of the brain showed temporal lobe abscess. Administration of antibiotics in massive doses and mannitol, redused the size of the abscess progressively. Then total removal of the abscess was performed after 6 weeks. Postoperatively, he was asymptomatic except for slight aphasia.

    Subcutaneous injection of Broncasma Bernahas been reported to be effective for nasal allergy and chronic sinusitis. The purpose of this study was to find the maximal permissible drug concentration for aerosol therapy, without inhibiting cilial activity. Small pieces of human paranasal mucosa taken at surgery, were cultured in RPMI-1640 medium. The ciliary activity was measured by the photo-electric method. The period for drug treatment was 30 minutes once a day. The cilia r y activities after single treatment and three day-consecutive treatment were measured f or three days. The findings indicated that the maximal permissible concentration of Broncasma Berna for aerosol is a two-dilution.

    A surgical procedure for chronic sinusitis has been established by Caldwell-Luc and Watsuji-Denker. However, the value of establishing a counter opening into the inferior meatus has been questioned mainly due to its effects on the postoperative healing process. Therefore, we compared the operation time, blood loss, postoperative cheek swelling and postoperative healing process on the same patient, with (left side)and without (right side) a counter opening. Forty patients who had radical treatment of chronic sinusitis on both sides were studied.
    Without the counter opening procedure the operation time was about 10 min. shorter and the blood loss was about 60 g less than that with the opening procedure. However, cheek swelling without the opening was slightly more than that on the other side. Obstructive healing of the maxillary sinus without it was about one month earlier than that with it.
    In summary, creati on of the counter opening may aggravate the early postoperative condition.

    Cysteine ethylester hydrochloride, at a dose of 300 mg a day for more than 4 weeks, was given to 25 patients with chronic paranasal sinusitis. Mild improvement was observed in 36% and slight improvement was seen in 56% from the overall judgement of subjective and objective symptoms.
    These results suggested t h at cysteine ethylester has a strong mucolytic action. This drug should be taken for more than 2 months. No side effects were noted at all.

    Chitin was used for postoperative treatment of 36 patients undergoing nasal and paranasal sinus surgery. Chitin, a mucopolysaccharide purified from the crab's crust, has an action to promote wound healing. This chitin was used in sheet or stick form or applied to the gauze.
    Chitin in sheet or stick form was not more useful than the conventional gelatinized sponge, which had to be changed every 24 hours due to its frangility and nonfusibility. We found that the gauze coated with chitin on both sides as thin as 2 mm was the most useful. These chitin gauzes were used in 21 patients after conchotomy and/ or deviatomy.
    Advantages of this tampon are as follows: 1. Its bulkiness decreased the number of tampons, three on an average, for one nasal cavity. This could save time.2. Extraction was easy and painless compared to the ordinary gauze. Disadvantages are lower pressing ability due to its elasticity and possible infections medium of chitin.

    A case of cholesterol granuloma in the ethmoidal sinus is reported. The patient was a 44-year-old male whose chief complaint was visual disturbance. Visual acuity improved transiently at the early postoperative day, but his visual impairment could not be healed completely. Although CT was one of the best method for diagnosis, conventional methods, tomography and Rhese's method, were also useful for early detection and diagnosis. The cholesterol granuloma was thought to be caused by hemorrhage. However, closed cavity was also an important facter for the development of cholesterol granuloma.

    A case of malignant lymphoma localized in the left nasal cavity is reported. A 82-year-old man, who had been complaining of left nasal obstruction, visited our hospital. His left nasal cavity was filled with a papillary tumor. Histologically, the first biopsy material was suggestive of poorly differentiated squamous cell carcinoma, but, the second biopsy material was diagnosed as malignant lymphoma. This may reflect the difficulty in histological differentiation between these tumors in hematoxylinandeosin-stained sections. By an immunoperoxidase method, the tumor cells were found to be B-cells that produced 1 gM with λ type light chain.

    New-born babies fuffering from respiratory distress treated at Osaka Medical Center and Research Institute for Meternal and Child Health between Oct.16,1981 and Dec.31,1986 were statistically analyzed. Two cases of congenital choanal atresia among these babies were reported herein.
    Both cases showed bilateral bony atresia. Case 1 was also accompanied by external nasal anomaly. She had an operation to correct the anomaly at 5 months of age and an operation for choanal atresia by the transpalatal approach at 1 year and 5 months of age. Case 2 also had multiple anomalies with a chromosomal disorder. His atresia was operated on by transnasal route at 3 months of age.

    A case of choanal atresia of unknown origin is reported. A 50-year-old female had been complaining of n asal obustruction for 3 years. Bilateral nasal cavities were found to be completely closed at the depth of the orifice of the auditory tube. The soft palate adhered of the posteior pharyngeal wall and was paralyzed completely. Surgical incision was made at the margin of the soft and hard palate, and the adhesive connective tissue was removed from the choana, reconstructing the nasopharyngeal space with rest of the mucosa. However, tendency of adhesion was observed du e to the soft palate palsy and then re-operation was performed to the create the mucosal lining with the soft palatal mucosa. Appropriate nasal passage has been obtained for 16 months since the first operation, and the case followed up closely.

    We performed uvulopalatopharyngoplasty (UPPP) on 14 patients for snoring and evaluated the efficacy of the operation. Seven patients underwent tonsillectomy in addition to UPPP.
    Thirteen patients showed good remission of snoring just after the operation. However, patients who had not undergone tonsillectomy showed less effect and tended to relapse into snoring during the long follow-up.
    It was concluded that tonsillectom y should be combined with UPPP for maximum effectiveness.

    We examined the applicability of the single radial complement fixation test to serological diagnosis of respiratory virus infection. The viruses used for the examination were influenza A virus, influenza B virus, RS virus and mycoplasma pneumoniae. We could confirmed an epidemic of influenza A virus infection in our trial period. This test is considered useful for routine serological diagnosis of viral infection.

    A case of vascular leiomyoma of the parotid gland is reported. The patient was a 37-year-old male with a left parotid tumor. The tumor was removed under general anesthesia. Histological examination revealed vascular leiomyoma. No recurrence has been noticed for 9 months after operation. Leiomyoma is a benign tumor which commonly occurs in the smooth muscle of the uterus, digestive organs or skin. This is the first case of leiomyoma of the parotid gland in Japan.

    Two cases of parathyroid cyst are reported. In case 1, a 61-year-old female had a left anterior cervical mass. On the chest X-ray film, the trachea was deviated toward the right by the mass. In case 2, a 48-year-old female also had an anterior cervical mass which had appeared for two years. Both cases were diagnosed as thyroid cyst or thymoma and the patients underwent operation. Upon operation, both masses were found to be solitary cysts, which were situated not in the thyroid gland but in the cervical region adjacent to the lower pole of the left thyroid lobe. In case 1, the cyst had a duct on the inferior part, which was thought to be connccted with the thymus gland. In case 2, high values of not only parathyroid hormone but also calcitonin hormone were detected in the contents of the cyst. Histologically, in both specimens, the inner surface was lined by the flattened cuboidal or columnar epithelium and by the connective tissue contained scattered islands of the parenchymal parathyroid cells. The final diagnosis of both cysts was parathyroid cyst.

    The distribution of hematoporphyrin derivative (HPD) in mice with transplanted dermal carcinoma was compared with that in normal tissues such as liver, tongue, spleen, kidney and skin on 1,2,3,4,7 and 9 days after HPD i. p. injection. The samples prepared from each tissue and tumor were used to measure the HPD content by a fluorescence spectrophotometer. HPD concentration in the dermal carcinoma was still higher than that in the normal skin from 4 to 9 days after HPD injection. The dermal carcinoma with necrosis showed higher HPD concentration than that without necrosis. HPD accumulation in tissue is probably due to host factors like vascular formation, vascular permeability, lymphatic clearance, and route of HPD administration.

    The muco-dermal tracheoesophageal shunt was successfully performed on 77%of 13 patients. Aspiration was well controlled by the three step method, finger press, silicon injection and voice prosthesis. A comparison of this method with previously experienced Komorn's modification showed almost the same success rate. This method is a simple primary one-stage radical method for every type of laryngeal cancer.

    The serum levels of several tumor markers were measured in 77 patients with head and neck cancer. The markers we measured were CEA, ferritin, TA-4, sialic acid, TAP, -AT. Statistical analysis revealed that these items could be divided into three patterns, accordind to the prognosis and tumor extent. This combination assay indicates the efficacy of treatment. Therefore, the combination assay was useful to determine the patients' condition with head and neck cancer.

    Cases of malignant tumors of the head and neck during a period of 4 years were analyzed with respect to the relationship of double cancer between its onset in terms of age of onset, family history, smoking and immunological competence was examined. Distribution by site was low in the nasal, paranasal and lingual areas, and was high in the larynx and thyroid gland. Incidence of double cancers was fairly high (8.3%)and age at the onset was about 10 years older than that of the primary tumors of the head and neck. Of 7 patients with double cancers,5 had a family history of cancer and 2 of the 4 patients with laryngeal cancer had a smoking history.
    Immunological competence showed decreased activity in 2 of the 3 cases examined. One patient with triple cancer (the stomach, larynx and thyroid gland) had a history of smoking, but had no family history of cancer or decreased immunological competence.

    We experienced a case of leukemic change of malignant lymphoma difficult to distinguish from acute lymphocytic leukemia. However, it was diagnosed as malignant lymphoma or leukemia from immature B-cell origin by retrospective immunohistochemical examinations (MT-1, MB-1, LCA, M1) along with investigations of cell surface markers. Immunohistochemical tests made it possible to differentiate T or B-cell classification, retrospectively. It is important to reduce the numbers of examination of cell surface markers and to select an effective combination.
