    The outcome of operations on 68 ears, which we performed between 1986 and 1988was reviewed.
    We carried out pure-tone audiometory in these cases. Surgery was done for cholesteatoma in 44.1%; for chronic suppurative otitis media in 55.9%; and for the second. time in 11.8%. Especially 8.8% had both operations done by us. Thirty two ears underwent intact canal wall tympanoplasty; 30 ears, classical tympanoplasty; and 6ears, radical mastoidectomy. Postoperatively hearing was improved in 52.5 % of the ears in which the tymponoplasty was carried out.

    The patient was a 26-year-old male clinical laboratory technician who was admitted to our hospital complaining of right intractable otorrhea and hardness of hearing. After tympanoplasty under a diagnosis of chronic otitis media, tuberculous otitis media was diagnosed from the results of the histopathological examination. He was administered antituberculosis drugs (SM, RFP, INH) postoperatively and he is presently being treated as an out-patient with a favorable course. This case is considered to be of primary tuberculous otitis media in which infection with the tubercle bacillus occurred via the external auditory meatus while the patient was working in hospital.

    The therapeutic effects of L -V therapy for sudden deafness from March,1983 to July,1987 were studied; and the following results were obtained. Twenty of the 78 cases in which this therapy was used resulted 20 cases in “complete recovery”; 12 cases in “marked recovery”; 21 cases in “recovery”; 25 cases in “no change”. The marked hearing recovery was observed in the patient who began this therapy within 7 days from the onset. The degree of hearing loss at the beinning of the therapy bore almost no relation to therapeutic effect. We had many cases showing “complete recovery”among the cases without vertigo and in those treated with steroids. Many cases with positive SISI test showed “complete recovery”, and the prognosis was related to the result of the SISI test. In 69.5 % of the cases, tinnitus diminished or disappeared. Therefore, this therapy had a good effect on tinnitus.

    Two cases of psychogenic deafness are reported.
    One patient was 16 years old, and the other was 24 years old. Their deafness occurred suddenly. Although both cases showed highly damaged sensorineural deafness of flat type on pure-tone audiometry, they had no disturbance for speaking. The results of auditory brainstem response or stapedial reflex were normal. The contradictions between the sujective and objective data suggested a psychogenic deafness. They were treated at both departments of otolaryngology and psychology, but deafness did not change. Auditory brainstem response and stapedial reflex are importnat to diagnose such deafness.

    The Cornell Medical Index (CMI) test was given to 72 patients with peripheral labyrinthine disorders. CMI types III and IV were noted in 31.9% of all, in 18.0% of those with Ménière's disease, in 36.4 % of those with BPPV and in 41.7% of those with otogenic vertigo. The scores for psychological symptoms of BPPV and otogenic vertigo were more than those of Ménière's disease. Otogenic vertigo with Schellong's test positive revealed many with CMI types III and N. The number of vertigo episodes was related to CMI type in Ménière's disease and BPPV. We had many CMI types III and IV cases in BPPV and otogenic vertigo with a long course, and in Ménière's disease with a short corse. Psychological factors appear to be involved not only Ménière's disease but also BPPV and otogenic Vertigo.

    A case of cholesteatoma otitis with vertigo is reported. The direction of nystagmus was changed with the passage of days. Thus, the judgement of the semicircular canal condition is important to observe the nystagmus. Concerning hearing, its level decreased and improved. Infection might have affected the inner ear. After recovery of infection, its function recovered.

    Three cases of laryngeal injury were reported. One patient was a 29-year-old male who had sandwiched his neck between the chassis of a dump truck. The second patient was a 44-year-old male who stabbed himself in the throat to commit suicide. The third patient was a 22-year-old male who bumped into the rear of a car, and struck his neck with a steering wheel. The first and second cases were open injury and had severe fracture of thyroid or cricoid cartilage. Because two patients had continued misswallowing still after several months, they underwent total laryngectomy.
    The third case was a closed injury, but emergency trach e otomy was done because of dyspnea. We kept the stent in his larynx for two weeks, to maintain the airway but he had slight hoarseness.

    A case of a laryngeal foreign body (needle) in a 64-year-old male is reported. The patient fell down from a ladder and hit his neck. The laryngeal foreign body was found incidently by cervical X-ray photographing. Then it was judged to be a needle and he was introduced to our department. Twenty-five years ago, he had severe pain in his throat when heate a fish. The abnormal feeling in his throat and the slight hoarseness seemed to have continued thereafter, as the needle had been included in the fish. We could completely extract the needle surgically.

    Thirty-one cases of acute epiglottitis presented in an 8-year period were clinically studied. The ages ranged from 15 to 80 years, there was no difference in sex, and the onset tended to be frequent from fall to winter.
    Typical clinical symptoms were sore throat, dysphagia and dyspnea, and emergency tracheotomy or endotracheal intubation were performed in 2 cases. The causative organism by pharyngeal culture was mainly indigenous bacteria, and H. influenzae, which is generally considered to be the causative organism of this disease, was detected only in 1 case by blood culture. All the hospitalized cases were treated with intravenous infusion of antibiotics and steroid inhalation, and steroids were intravenously injected in 21 cases. The average admission period was 7.6 days. The hospitalization period was obviously shorter for the patients who were concomitantly given steroids intravenously than those not given steroids and a significant difference was noted. All cases were successfully treated by maintaining the airway, and administering antibiotics, steroids, and replacement fluids.

    This is a report of an 81-year-old man with recurrent aphthosis of the oral cavity and uncontrolled diabetes mellitus (Type II). When he was 71 and 76 years old, he entered the Department of Oral Surgery of our hospital due to recurrent aphthosis of the oral cavity. It was suspected that the case was Behget's disease, but no specific findings were obtained.
    When he was 80 and 81 years old, he entered our department three times because of acute epiglottitis, chondritis of the epiglottis and pharyngeal ulcer, respectively. Although the patient was given Cepharanthin, no remarkable change was noted. Some treatment to control the blood glucose was performed, and his ulcers healed gradually.

    We report a case of laryngeal pseudolymphoma a 38-year-oldman. His chief complaints were hoarseness and dyspnea. At the third biopsy since presentation, pseudolymphoma was diagnosed. In the process of formation of the pseudolymphoma, granulomalike tissue probably formed temporarily. The tumor was removed with a CO2 laser. There is no sign of recurrence at the present 2 years after treatment.

    A case of benign tumor (lipoma) of the hypopharynx in a 53-year-old male is reported. The tumor we extracted was not a pendulous tonsil; at had arisen from the right lateral wall of the hypopharynx. It recurred two months later, and was resected via an external approach.

    We report a case of von Recklinghausen's disease (VRD) with a tumor in the left parotid region. The patient was a 3-year-old female. Swelling of the left parotid gland. showed a tendency of gradual enlargement from about 8 months after birth. A primary benign tumor of the parotid gland was considered from an imaging diagnosis. Further, the presence of nevus complicated by VRD was also considerd, and an operation was performed, Neurofibroma was diagnosed, the tumor in the parotid region was judged to be a symptom of VRD, and a cosmetic approach was taken. The family history, should be obtained due to different physical findings and chief complaints. At present there have been no complications in other areas and the course is good.

    We report a 25-year-old man with recurrent oral ulceration. In October 1981, he entered our hospital because of a traumatic carotid aneurysm and thrombophlebitis of the legs. Angiography showed an aneurysm of the right common-internal carotid artery at the level of bifurcation. A diagnosis of Vasculo-Belaçet's disease was made. In september,1985, surgery was performed.
    After operation, right H orner's syndrome and a nasal obstruction appeared. He suffered from this nasal obstruction until January,1986. In April,1986, bilateral conchotomy was performed. After operation, there were no complaints of nasal obstruction.

    An odontogenic skin fistula is a secondary disease from an infected tooth. An 18year-old woman was admitted with a fistula on the right submandibular area. The diagnosis was external dental fistula resulting from the root inflammation of the right mandibular 2nd molar. The results of tooth were good.

    In 10 patients with head and neck tumors chemotherapy by intra-arterial infusion was performed, chiefly using cis-diamminedichloroplatinum. The drugs for concomitant use were peplomycin, methotrexate, and 5-fluorouracil.
    A complete response was obtained in 4 cases, and partial response in 5 cases. The response rate was 90%. The therapy was concluded to be effective, and to be a useful treatment preoperatively or before radiotherapy.
    The total and free platinum concentrati o n in the blood were similar for both intraarterial infusion and intra-venous injection. The platinum concentration in tissues tended to be higher with intra-arterial infusion.

    We investigated the radius and width of tumor invasion as well as the sites of metastasis in cases with malignant melanoma, by scintigraphy using N-isopropyl-P-(123I) iodoamphetamine (IMP) (referred to as 123I-IMP).
    Simultaneous administration of L-Dopa with intra-arterial drug infusion therapy, significantly reduced the accumulation of radioactivity from 123I-IMP in the tumor lesion.
    This indicated that L-Dopa might have an affinity for melanoma cells, as it replaced 123I-IMP in the tumor lesion. Consequently, L-Dopa might be effective in the treatment of malignant melanoma.
    Furthermore, wereported the results of the combined administration of L-Dopa and 5-(3,3'-dimethyl-I-triazeno) -imidazole-4-carboxamide for the treatment of malignant melanoma.

    We performed intra-arterial infusion therapy on a case of carcinoma of the maxillary sinus (T3N0M0, undifferentiated carcinoma) for 162 days.
    Urokinase (UK) combined with cepharanthin were give n for 162 days. Thus CP therapy (CDDP: 75 mg + PEP: 25 mg) and sensitization of radiotherapy by 5-FU (a total dose: 3,500 mg) were performed. Gross Denker operation and additional radiotherapy (a total dose of 70 Gy) were carried out, and thereafter a complete response was observed.
    We recommend long-term intra-arterial infusion via an indwelling catheter for achieving good therapeutic results.

    Bacteriological studies and observation by the scanning electron microscope of the tips of the intraarterial catheters were made on patients with malignancies of the head and neck who were treated by intraarterial injection. Seven of the 28 samples (25 %)were positive bacterial contamination, but no clinical symptoms of infection developed at all. There were two achromobacter xylosoxidans,2 staphylococcus epidermidis,2 propionibacterium acnes and one pseudomonas cepacia. Positive bacterial contamination (Pseudomonas cepacia) was seen in ony 1 of the 9 patients who were compelled to remove the intraarteral catheters. Scanning electron microscopic findings of the tips, and the inside catheters showed granular and fern-like materials, but none showed bacteria. Predominant peaks of Na, Cl, P, S levels were detected by an X-ray microanalyzer.
    In conclusion, no relationship was observed between the trouble of tubing and the bacterial contamination, but care should be paid to avoid the contamination.
