    The correlation of smoking with epithelial dysplasia and PCNA positivity was investigated in 27 patients with oral leukoplakia, for whom their smoking habit was known.
    The results revealed that Brinkman indexes in leukoplakias with epithelial dysplasia were higher than those without dysplasia, but there was no significant correlation between Brinkman index and epithelial dysplasia. PCNA positivity became higher with the grade of epithelial dysplasia, and PCNA positivity had an increasing tendency in accordance with the increase of Brinkman index. Further, PCNA positivities in leukoplakias that later developed into cancer were higher than those in the other leukoplakias.
    These results suggested that smoking maybe concerned with the progression of epithelial dysplasia by increasing cell proliferating activity in oral leukoplakia.

    The purpose of this clinical study on 62 cases of oral lichen planus (OLP) was performed to better understand its clinical features. The results were as follows:
    1. Patients were 22 males and 40 females with a 1:1.8 sex ratio. The disease predominantly occurred in middle-aged and elderly patients, and 87.1% of the patients were from 40-69 years of age.
    2. Contact pain was the most common chief complaint of the patients, followed by white lesions.
    3. 71.0% of the patients visited our department within one year.
    4. Systemic diseases were noted in 51.6% of the patients. 41.9% of the patients used medication for systemic disease.
    5. The positive percentage of metal patch test was 76.5%.
    6. 69.4% of the patients had multiple pattern lesions of OLP.
    7. The distribution of OLP by site of occurrence was as follows: buccal mucosa (66.2%) and lower gingiva (16.5%). 70.2% of the lesions of buccal mucosa were distributed bilaterally.
    8. Reticular lesions were most common, followed by plaque lesions.
    9. The clinical effects were classified into excellent (11.8%), good (35.3%), fair (35.3%), and poor (17.6%). The effective rate was 47.1%. The effective rates of vitamin A, steroid ointments, and biscoclaurine alkaloids were all about 50%.
    This study suggested that OLP may be caused by both local and systemic factors. Biscoclaurine alkaloids were useful for the treatment of OLP, considering that they had only slight side effects.

    A case of unusual shaped denture fibroma located on the floor of the mouth is reported. A 60-year-old female was referred to our department with the chief complaint of denture ill-fitness for past 2 years. A pedunculated, bank-like, firm tumor was on the right floor of the mouth extending from the midline to the retromolar trigone and was higher the alveolar crest. The tumor was excised at the base and alveoplasty was performed simultaneously. Microscopy revealed mature fibro-connective tissue containing perivascular chronic inflammatory cell infiltrations.

    Lesch-Nyhan syndrome, a rare inborn error of metabolism, is characterized by hyperuricemia, choreoathetosis, mental retardation and self-mutation behaviors. A case of a 13-year-old male with this condition, self-mutilation of the lower lip was seen. A splint for the maxillary dental arch was inserted to prevent lip biting. The splint was very successful in preventing lip biting.
