    We describe the preoperative use of limited cone beam computed tomography (CT) with a dental CT scanner for the assessment of mandibular third molars before extraction. Cone beam CT provides 42.7-mm-high and 30-mm-wide rectangular solid images, with a resolution of less than 0.2mm.
    The positional relationship between the mandibular third molars and the mandibular canal was examined by dental CT. Sixty-eight lower third molars of 62 patients whose teeth were superimposed on the mandibular canal on periapical or panoramic radiographs were studied. Dental CT scans clearly demonstrated the positional relationship between the mandibular canal and the teeth. The mandibular canal was located buccally to the roots of 16 teeth, lingually to the roots of 27 teeth, inferiorly to the roots of 23 teeth, and between the roots of 2 teeth. The presence of bone between the mandibular canal and the teeth was not noted in 7 of 16 buccal cases, 24 of 27 lingual cases, and 10 of 23 inferior cases on dental CT scans, suggesting that the cana was in contact with the teeth.
    Fifty-nine of the 68 mandibular third molars were surgically removed, and postoperative transient hypoesthesia occurred in 4 patients. Dental CT scans showed no bone between the mandibular canal and the teeth in all 4 patients. Hypoesthesia was not related to the bucco-lingual location of the mandibular canal or to the extent of bone loss between the canal and the teeth. However, hypoesthesia did not occur in patients with bone between the mandibular canal and the teeth. Thus, information on the distance between the canal and teeth on dental CT scans was useful for predicting the risk of inferior alveolar nerve damage.
    Because of its high resolution and low radiation dose, cone beam CT was useful for examination before mandibular third molar surgery.

    Liposarcornas of the tongue have rarely been described. We present a rare case of myxoid liposarcoma of the tongue. A 63-year-old man presented with two slowly enlarging masses arising in the tongue. One, measuring 25×30mm, was found on the lateral surface, and the other, measuring 15×10mm, on the dorsal surface. Biopsy was performed, and the lesions were diagnosed as liposarcomas with low grade malignancy. We therefore performed partial glossectomy with a wide margin. Microscopic examination revealed a myxoid liposarcoma invading muscle. After 3 months, a small mass was noted on the opposite dorsal apex of the tongue. The histopathologic findings of the resected specimen were similar to those of the primary tumor. This rare case of myxoid liposarcoma of the tongue was associated with local recurrence. As of 2 years after the second operation, there has been no evidence of recurrence or metastasis.

    Hyalinizing clear cell carcinoma is a minor salivary gland tumor, proposed as a new entity by Milchgrub et al in 1994. A review of the literature indicated that this tumor is rarely associated with recurrence and lymph node metastasis, but lymph node metastasis has not been reported in Japan. This report describes a case of metastasis to the cervical lymph nodes that appeared 2 years 7 months after surgical excision of a hyalinizing clear cell carcinoma.

    An 82-year-old woman was referred to Tokai University Hospital because of swelling of the upper jaw. Biopsy was performed, and the diagnosis was squamous cell carcinoma. Because she had coronary artery disease, percutaneous transluminal coronary angioplasty (PTCA) was performed before surgery for the upper gingival cancer. Consequently, surgery was able to be performed without perioperative cardiac complications. However, coronary restenosis occurred 1.5 months after PTCA, and an intracoronary balloon-expanded stent was inserted at the site of restenosis.
    Currently, there has been no local tumor recurrence and no angina pectoris.

    We report a case of pseudolymphoma of the palate. A 55-year-old woman was referred to our hospital because of swelling of 9 years' duration on the left side of the palate. Intrao ally, a well-circumscribed lesion measuring approximately 30mm was noted. Radiographically, computed tomography revealed resorption of the maxillary bone, and 67Ga scintigraphy showed abnormal uptake on the left side of the palate. Microscopically, the tumor showed hyperplasia intermingled with lymphoid follicles and interfollicular lesions. The latter consisted of lymphocytes, plasma cells, eosinophils and larger histiocytic cells, none of which showed atypical or neoplastic findings. Immunohistochemically, the lesion consisted of an admixture of cells that were positive for CD45RO, CD20, and S-100 protein.

    Glandular odontogenic cyst (GOC) is a rare jawbone cyst of odontogenic origin, first described by Gardner et al. in 1988. GOC is characterized by typical histopathological features, a gradual increase in size, and a high rate of local recurrence if not adequately treated. We report a case of GOC occurring in the anterior region of the mandible. The patient was a 62-year-old woman who presented with swelling of the anterior region of the mandible. Panoramic radiography indicated a multilocular radiolucent lesion in the anterior region of the mandible. Enucleation of the cyst was performed. Postoperative healing was satisfactory, and no recurrence has been noted on long-term observation. We reviewed 41 cases of GOC that had beenreported previously. Among these cases, 35 were located in the mandible and only 6 in the maxilla. The male: female ratio was 1.28 (23: 18), and the mean age was 47.8 years. Radiographically, among a total of 37 cases, 18 were unilocular and 19 were multilocular. The overall recurrence rate was 19.5%.

    We describe a melanin-pigmented calcifying odontogenic cyst that arose in the right side of the mandible in a 12-year-old boy and review previously reported cases of similar melanin-pigmented lesions. The patient was referred to our department for evaluation and treatment of gingival swelling. A computed tomographic scan showed a calcifying lesion with in a cystic lesion measuring 31×22mm. The lesion was excised under general anesthesia, and the pathological diagnosis was a melanin-pigmented calcifying odontogenic cyst. Areas of melanin pigmentation were stained blackish-brown with hematoxylin-eosin stain and were most common not only inside but also around ghost cells. Nearly all reported cases of melanin-pigmented calcifying odontogenic cysts have occurred in Asians and blacks, and there have been only a few cases in whites. Racial factors are reported to play an important role in melanin pigmentation.

    A case of trigeminal herpes zoster infection associated with alveolar bone necrosis of the mandible and facial palsy is reported. A 72-year-old woman was referred to our hospital by her dentist because of right-sided facial pain. Physical examination revealed vesicles on the lower-right side of the face and vesicle-like erosive lesions on the mandibular gingiva (5-1). Blood examination indicated the high levels of antibodies against varicella-zoster virus. An antiviral agent (acyclovir) was administered, and the vesicles and erosions gradually resolved. However, facial palsy (Ramsay-Hunt syndrome) and alveolar bone necrosis developed 2 weeks later, and then the right lower canine spontaneously fell out. Steroid therapy, stellate ganglion block, and local irrigation were done. Tooth extraction and removal of necrotized bone were finally carried out, and cure was obtained.
