    Treatment for maxillofacial trauma accounts for large part of oral and maxillofacial surgery. Many treatment modalities for zygomatic fracture and condylar fracture have been introduced so far. This paper describes the technique that the author routinely applies for open reduction of zygomatic fracture and condylar fracture.
    For zygomatic fractures, exposures of the fracture lines are achieved by lateral eyebrow incision, subtarsal incision and intra-oral vestibular incision. The important aspect of the surgery is sufficient elevation of the periosteum of temporal surface of the fractured zygoma. This makes the precise reduction of the bone segment easy especially for the cases that have medially collapsed zygoma.
    For the cases of condylar fracture, exposure of the fracture line is achieved via retromandibular approach. In this procedure, identification of the posterior edge of the parotis and the fascia of sternomastoid muscle is the most important surgical step. After the identification of these anatomic landmarks, exposure of the posterior edge of the mandibular ramus is easily achieved by anterior retraction of the parotis.

    Before independence of the USA, diagnosis and treatment, including surgical operations of oral and maxillofacial lesions were performed by medical doctors of European emigrants and their American disciples.
    The first American dental school was founded in 1840 and exodontia was established to be the core of most educational programs in oral surgery for almost 100 years. The earliest American dental schools did not contribute to the development of oral surgery. The Journal of Oral Surgery, which was the first specialized journal of oral surgery in the USA, was published in 1943 by the American Dental Association.
    Many emigrants played an important role in upgrading educational programs in oral and maxillofacial surgery, including AJ Asgis (Russia), RH Ivey (1881 UK), VH Kazamjan, (1879 Armenia), CW Waldrom (1887 Canada), and K Thoma (1883 Switzerland). In 1938 Asgis emphasized the importance of including basic sciences in American dental education programs. The same opinion was asserted by Thoma in 1957. These opinions stressed that the educational programs of American dentistry should include basic sciences. Lack of knowledge about basic sciences would be unsuitable for dental medicine and might impair the development of oral and maxillofacial surgery.

    To our knowledge, a case of primary oral malignant melanoma (MM) invading mainly the jaw has yet to be reported. We report a case of MM that spread widely in the mandibular medullary cavity with limited involvement of the mucosa.
    A 40-year-old woman visited our hospital because of mobility of the lower right second molar. Initial examination showed that the second molar floated, and the gingiva was pale purple-blue. An X-ray film showed a diffuse radiolucent area from the premolar to the ramus of the right mandible. The diagnosis of a biopsy specimen was MM, and no distant metastasis was detected on whole body examination. A wide tumor resection with radical neck dissection and immediate reconstruction were performed. Histopathological examination of the specimen revealed MM invading mainly the mandibular bone marrow; however, the primary site remained unknown. DAV-Feron therapy was given postoperatively. Three months later, left cervical lymph node metastasis was detected, and neck dissection was performed. Three years after the first visit, a metastatic lesion in the pleural space was excised by thoracosopic surgery. Ten courses of DAV-Feron therapy were completed, and the patient is now under observation.

    Kimura's disease is a rare benign chronic granulomatous lesion of soft tissue or lymph nodes of the head and neck region and is characterized by eosinophilia and an increased serum IgE level in peripheral blood. Kimura's disease is reported to be associated with renal disturbance, but the pathogenesis of this association remains obscure. We report a case of Kimura's disease associated with nephrotic syndrome.
    Two years before the first visit, the patient complained of a mass with painless swelling in the left cheek and on both sides of the neck. Remission and relapse of the mass occurred several times in association with the dose of steroids administered for nephrotic syndrome. Laboratory investigations revealed peripheral blood eosinophilia and an elevated serum IgE level. The mass in the cheek was surgically enucleated for esthetic purposes and definitive diagnosis. Histopathological examination indicated eosinophilic lymphfolliculoid granuloma; thus, Kimura's disease was finally diagnosed. The patient has received continuous pharmacotherapy with steroids and immunosuppressive drugs; the mass shrank on both sides of the neck, and good clinical status has been maintained.

    A case of Ewing's sarcoma of the mandible in a young child is presented. A 12-year-old boy was referred to our hospital because of swelling of the cheek, which he noticed 2 months previously. Clinical examination revealed diffuse swelling of the cheek, and a fistula of the gingiva. CT examination revealed a sun-ray apiculated image. MR showed a contract-enhancing lesion in the left mandibular angle. FDG-PET and Ga-scintigraphy revealed no metastasis. Based on the results of histopathological analysis, Ewing's sarcoma was diagnosed. The patient received combined chemotherapy, surgery, and radiotherapy. The patient has shown no evidence of recurrence or metastasis for 24 months.

    Sebaceous carcinoma arising in the oral cavity is extremely rare. We report the case of a 76-year-old woman with delayed cervical lymph node metastases from sebaceous carcinoma of the soft palate, developing 4 and 9 years postoperatively. The primary tumor was 20 mm in diameter. On the basis of the imaging and biopsy findings, a malignant tumor of the soft palate (cT2N0M0) was diagnosed, and the tumor was excised. Delayed cervical lymph node metastasis was detected on the right side 4 years 11 months postoperatively and on the left side 9 years 8 months postoperatively. Dissection was performed on the right and left sides. The postoperative course was uneventful, and there was no evidence of recurrence or metastases 13 years after the first resection.

    We report two cases of alveolar osteonecrosis and tooth loss caused by trigeminal herpes zoster infection.
    A 69-year-old man and a 73-year-old man were referred to our hospital after the onset of herpes zoster infection of the third ramus of the right trigeminal nerve. The first patient had alveolar osteonecrosis and spontaneous teeth exfoliation (5 4 3) 15 days after the onset of herpes zoster infection. He was given a diagnosis of mandibular carcinoma by his dentist. The second patient had teeth mobility (4 3) 4 days after the onset of herpes zoster infection. He was given a diagnosis of acute necrotizing ulcerative gingivitis by his dentist. In both patients antibiotic therapy and local irrigation were performed, and the separated necrotic bone was removed step by step. In the second patient the right lower canine, which had grade III tooth mobility, was extracted 55 days after treatment. After final removal of the necrotic bone, cure was obtained.
    Although both patients showed a typical clinical course of herpes zoster infection, their dentists could not make a correct diagnosis. Therefore it is necessary to be aware that alveolar osteonecrosis and tooth loss can be caused by trigeminal herpes zoster infection. Minimum surgical treatment may be effective for such complications.

    To study the characteristics of sialolithiasis in children, we examined 16 patients with sialolithiasis younger than 10 years who were treated in the Department of Oral and Maxillofacial Surgery, Nagano Red Cross Hospital during a period of 27 years 3 months from October 1983 to December 2010. The 16 patients with sialolithiasis had salivary calculi in the submandibular gland. The main initial chief complaint was swelling in the salivary gland or its aperture (7 patients, respectively). About 70% of the patients visited our department within 6 months from the onset of initial symptoms. The main initial clinical finding was swelling in the aperture of the salivary gland (12 patients). All patients had a calculus, and most of the calculi were less than 10 mm in diameter. Calculi were removed through an intraoral approach in 14 patients (87.5%).
    The results of our examinations and previous studies indicated that the most common region of sialolithiasis in children was the aperture of the submandibular gland. The calculi usually caused swelling in the aperture and salivary gland, and their size was 10 mm or less in diameter.

    We report a case of bisphosphonate-related osteonecrosis of the jaw (BRONJ) complicated by rapidly progressive glomerulonephritis (PRGN). A 68-year-old woman was referred to our department because of right submandibular swelling. She had received methotrexate, inflimaxive, and arendronate sodium hydrate for rheumatoid arthritis. After extraction of the right mandibular first molar, she received steroid therapy for a leg ulcer, and osteonecrosis of the mandible occurred. During conservative treatment, her creatinine level became 3.34 mg/dl. She was hospitalized to control renal function and given a diagnosis of rapidly progressive glomerulonephritis associated with mandibular infection. We performed segmental resection with mandibular reconstruction using a titanium plate. Renal dysfunction started to improve after opertion, and BRONJ was cured.
