Psychosomatic medicine iuvestigates the physiological, psychological, social, and environmental factors of diseases. Is psychosomatic tailor-made prevention and therapy achievable? To answer this question, it is necessary for us to have genomic data associated with diseases effectively treated with psychosomatic approach. In this paper, we presented our data on four functional polymor-phisms. A polymorphism in the promoter region of the dopamine D_4 receptor gene is associated with novelty-seeking trait of human personality. A promoter polymorphism in the serotonin transporter gene is associated with harm avoidance trait of human personality, antisocial traits, smoking, and cardiovascular diseases. A missense polymorphism of the methylenetetrahydofolate reductase gene is associated with cardiovascular diseases and schizophrenia. An intronic polymorphism of the angiotensin converting enzyme gene is associated both with cardiovascular diseases and with affective disorders. In addition, to identify associated gene mutations systematically, genome-wide linkage search for atopic asthma and schizophrenia including QTL analysis are presented.

At the end of the last century, Genetic analysis found that type 2 diabetes is a disease associated with thrifty genes or genes resistant against starvation, although the disease is known for a disease for rich people. Type 1 diabetes as well has been found to be associated with pancreatic specific autoantibodies and the autoantigens related to the autoantibodies was found at the end of the last century, which lead to the way to prevention of type 1 diabetes. Such circumstance around both type 1 and type 2 diabetes provides us with a fundamental therapy to diabetes instead of a conservative therapy. The therapy will resolve a difficulty of the treatment of diabetes.However, we have to consider that diabetes is a mental disease related closely to self-esteem ability. The major part of the treatment of diabetes depend upon self-management of the disease. Assessment of the self-management is just evaluated by the level of glycemic control or the development of diabetic complications. The self-management will be continued for their whole life. Poor glycemic control or the development of diabetic complications sometimes leads to the lowering of self-esteem or the self-destruction, which also leads to poor glycemic control, and so on. Even if an ideal drug is developed, such self-esteem is necessary for the mass-screening or treatment of diabetes.Here, psychosomatic medicine is more necessary for the treatment of diabetes.

Substantial changes have been effected in the clinical diagnosis for low back pain. The first one is an introduction of EBM. The second is a conceptual change for low back pain. The third is reconsideration of the standard for evaluation of therapeutic results. These changes have taught us the importance of considering low back pain as multifactorial biopsychosocial pain syndrome, not a biomedical "injury". A clinical system complied with by the new conception is based on a multidisciplinary approach. Medical care providers for low back pain should pay attention to the knowledge or techniques not only in their own special area but also in other areas.

The purpose of this study was to assess longitudinal change of self-efficacy and its relationship with psychological adjustment in terminally-ill cancer patients. The sample was consisted of 31 (15 males and 16 females) patients with terminal cancer who were outpatients under hospice care or in hospice ward. The interviews were conducted on two points which had at least two weeks duration, and with some measurement scales, which included self-efficacy scale for terminal cancer, and Hospital Anxiety and Depression Scale (HADS). Patients' performance status also evaluated using Karnofsky Performance Status scale (KPS). Repeated measures ANOVA revealed no differences between the first interview (time 1) and the second interview (time 2), except for subjective assessment of sleeping status. Repeated measures MANOVA with controlling KPS revealed that time procedure had significant main effects and interaction effects with KPS on ADL efficacy, depression, and anxiety (p<0.05). Structural equation modeling compared structures between time 1 and time 2,which represented the relationships among three self-efficacy subscales and anxiety and depression in HADS. No equality constrains were rejected and the first model had a comparatively high fit (Chi-square [37] = 51.3,p = 0.06). These results showed no significant difference in corresponding paths between time 1 and time 2. In conclusion, self-efficacy and psychological adjustment of terminally-ill cancer patients longitudinally changed according to each performance status. However, a relationship between self-efficacy and psychological adjustment was stable in spite of progression of physical counditions.

To examine the relationship between commuting time and atherosclerotic risk factors, we conducted a cross-sectional study at Jichi Medical School Health Care Center. The subjects were 3,615 men and 1,236 women, and the study was carried out with self-recording questionnaires in multiphasic screening in 1998. In the total subjects, means of body mass index, systolic blood pressure, diastolic blood pressure, and triglycerides increased with commuting time, a mean of high density lipoprotein cholesterol decreased with commuting time. After making adjustment among age, sex, alcohol drinking, smoking and exercise, however, multiple regression analysis showed commuting time of over 60 min was an independent factor associated with body mass index. In both men and women, a proportion of alcohol drinkers increased with commuting time, and in the total subjects, a proportion of smokers increased. As for men, a proportion of regular exercise decreased with commuting time. In order to prevent atherosclerotic disease, it is necessary to educate long time commuters on better life style.

The Present study reported an application of the solution-focused approach (SFA) to a patient with spasmodic torticollis. Although a decrease in the base EMG level of sternocleidomastoid muscle was observed during relaxation training (EMG biofeedback and Autogenic training), symptoms of spasmodic torticollis remained intact in daily life. In order to explore a possibility of the muscle control in daily life, we applied the SFA to the patient. While the therapist focused on the patient's solution image during SFA sessions, the patient not only described its future goal but also performed various activities with muscle control. After four sessions of the SFA, the symptoms improved. We discussed some implications for the effectiveness of the SFA on the case of spasmodic torticollis.
