Auricular defects are often caused by animal-bite injuries; however, few reports have described comprehensive management, including initial treatment and subsequent reconstruction, in such cases. We present the case of a 23-year-old woman with partial auricular loss due to a dog bite. Approximately three-fourths of the right auricle was lost, with associated cartilage exposure. Initial management included irrigation, fixation of the avulsed cartilage, and coverage with a postauricular flap. Definitive reconstruction was performed four weeks later using costal cartilage grafting, a temporoparietal fascial flap, and split-thickness skin grafting. A sterilized plaster model of the contralateral ear was used intraoperatively to optimize cartilage shaping and minimize the volume of harvested cartilage. All grafts survived completely, resulting in a satisfactory contour that allowed the patient to wear eyeglasses and face masks. Planning from the outset enabled successful one-stage reconstruction with good aesthetic outcomes.
