    Conservative procedure of treatment for a patient with chronic sinusitis was performed by means of inserting a drainage tube into a maxillary sinus through a wall of under nasal meatus and leaving it.
    It established drainage and ventilation of the maxillary sinus and insert such a medicine as Rinderon VG salve and antibiotics into maxillary sinuses. Drainage tube and instrument for inserting it were developed by us. This treatment was performed in 101 maxillary sinuses of 85 cases.
    Improvement in subjective symptoms was obtained in 86% of cases, and it in X-ray finding was 73% of cases.

  • Observation of maxillary membrane by ultrafine fiberscope (UFF)

    45 children with chronic sinuitis were treated by antral irrigation in our department from 1985 to August 1991.
    After 17 months of treatment, 11 (22.7%) of the 45 patients were cured, 2 (4.5%) had received operations, 19 (41.0%) were continuing with treatment, and 14 (31.8%) had discontinued treatment.
    The maxillary membranes of the 10 of the 45 children were examined using an ultrafine fiberscope (UFF) to determine the results of the treatment. The results of the examinations of the maxillary membranes by UFF were classified into three categories: normal or reticularvessel type, intermediate, including vasodilator and mixed vasodilator and edematous types, and edematous type.
    The results of examination by UFF correlate with both X-MFT and with the condition of the irrigation fluid recovered from patient.
    Examination by UFF was an effective method of observing the progress of chronic sinuitis.

    CO2 laser surgery apparatus was applied for the treatment of perennial nasal allergy, and long term results were evaluated for more than 2 years period after surgery, comparing the short time results.
    Among 38 patients who were observed long-term after surgery, 85.7% of cases showed good improvement, as compared with 86.1% improvement rate of 40 patients who were observed short-time.
    In both periods, the improvement of each symptom score for sneezing was exceed in that for nasal discharge, and that for nasal obstruction. Since there are no side effect throughout these periods, this surgery is considered as a safe and effective treatment for patients with perennial nasal allergy.

    Fibrin tissue adhesive was clinically applied in the field of rhinology and its irritant reactions were examined in animals.
    Clinically, we used this adhesive in 138 cases to bond the mucosa of the nasal septum during deviatomy. The amounts of tamponade required after the operation could be reduced in 93.5% of these cases (tamponade-reducing effect). In 20 cases, no tamponade was required after the operation. This adhesive was also useful in: (1) bonding the incised gingiva during surgical treatment of postoperative maxillary cyst, (2) stopping bleeding from bones during the operation on chronic sinusitis, (3) filling the cystic cavity during the operation on radicular cyst, and (4) filling bone defects during the operation on frontal sinuectomy. It was additionally useful in stopping nasal bleeding from the kiesselbach's area.
    In an animal study, several parts of the rat back were incised for subsequent wound closure with fibrin tissue adhesive, Aron-Alpha and suture.
    Inflammatory cell infiltration into the surrounding tissue was less marked after bonding with fibrin tissue adhesive than with Aron-Alpha, suggesting lower irritant reactions of fibrin tissue adhesive.
