JOURNAL FREE ACCESSBecause of the risk of infections such as hepatitis and AIDS, and GVHD, it has been suggested recently that homologous blood transfusion be avoided as much as possible during elective operations.
We studied rates of blood transfusion during operations at our hospital in the years 1986-1991.
In 1986, homologous blood transfusion was performed in 32.4% of 1, 687 cases, but decreased to 13.6% of 2, 535 cases in 1991.
The main reason homologous blood transfusions decreased is that autologous blood transfusions were introduced in 1987, when our blood transfusion service was estab-lished. Since then, predeposit autologous transfusions have increased year by year, and were performed in 219 patients in 1991.
In conclusion, the rate of homologous blood transfusion decreased markedly because of the increase in autologous transfusion, and changes in blood transfusion criteria and surgical techniques.View full abstractDownload PDF (711K) -

JOURNAL FREE ACCESSModified electroconvulsive therapy (m-ECT) was tried in seven depressive and three schizophrenic patients who did not improve with drug treatments. Before m-ECT, they were given 100% oxgeneration for four deep breaths. After spontaneous breathing for 3 minutes, they were injected with thiopental 5mg•kg-1 and succinylcholine 0.6mg•kg-1 intravenously in order to perform 100 volt electroshock for 5 seconds without convulsion.
Blood pressure, heart rate, and serum CK levels did not significantly change as compared with those in conventional ECT. Spontaneous respiration reappeared in 121±40 seconds after m-ECT. Seven of ten patients did not need assistant respiration.
In conclusion, m-ECT with preoxygeneration was beneficial for patients because of its hemodynamic stability and easy respiratory management and unremarkable changes in serum CK levels.View full abstractDownload PDF (558K) -

JOURNAL FREE ACCESSOptimal dose of midazolam for sedation during spinal anesthesia was investigated. One hundred and fifteen patients for spinal anesthesia (analgesic area below Th4), 30 to 70 years old, were divided into five groups according to midazolam dose: 0.025mg/kg (27 cases), 0.05mg/kg (25 cases), 0.075mg/kg (23 cases), 0.1mg/kg (25 cases), and 0.125mg/kg (15 cases). Responses to verbal command and ciliary reflex were signifi-cantly more depressed in the 0.05, 0.075, 0.1, and 0.125mg/kg groups than in the 0.025mg/kg group. The sedative effect of 0.025mg/kg of midazolam was therefore considered to be weak. In more than 25% of cases in the 0.075, 0.1, and 0.125mg/kg groups, respiration was depressed by dropped tongue. Respiratory rate increased significantly in the 0.1 and 0.125mg/kg groups. The numbers of cases who required vasopressor, who had body motion or nausea and vomiting did not differ much among the five groups. Blood pressure was more stable in the 0.025 and 0.05mg/kg groups than in the other three groups. The stabilities of circulation and respiration were better in the 0.025 and 0.05mg/kg groups than in the others. It was concluded that the optimal dose of midazolam for sedation during spinal anesthesia in patients aged 30-70 years is 0.05mg/kg.View full abstractDownload PDF (509K)
