JOURNAL FREE ACCESSWe examined the utility of the echo-guided axillary approach to brachial plexus block (B.P.B.) in 50 consecutive patients undergoing surgery of the forearm and hand. The success rate of complete sensory blockage of radial, ulnar, medial, antebrachial and mus-culocutaneousnerve was 90%, 96%, 94%, 98%, and 70% respectively. These results were comparable to those when using the nerve stimulator technique, which has become more popular in recent years. With the echo-guided axillary approach, side effects can also be avoided such as nerve injury and intravascular injection associated with B.P.B. because it can be performed visually. We conclude the echo-guided method is a safe and useful technique for the B.P.B. at axilla.View full abstractDownload PDF (2118K) -

JOURNAL FREE ACCESSThis report is on a 48-year-old man with an invasive thymoma located in the superior vena cava and the right atrium. He had progressive dyspnea and superior vena cava syndrome, and an emergency resection of the tumor was scheduled under extra corporeal circulation.
Special care was needed at induction of anesthesia because of possible complications such as deviation of the trachea by tumor and increased intracranial pressure. Only transeso-phageal echocardiography could be used for monitoring circulation. Careful anesthetic management was required during induction of the anesthesia. It was such an emergency operation, that we did not have the time to prepare enough for it. However we would result in a good anesthetic course by using transesophageal echocardiography during the opertion. This is an extremely rare case among patients with invasive thymoma as far as its growht is concerned.View full abstractDownload PDF (1670K) -

JOURNAL FREE ACCESSWe report a case of a 55-year-old male with status asthmaticus associated with a highly elevated creatinine kinase (CK) level after treatment with volatile anesthetics. The patient required mechanical ventilation with sevoflurane and halothane to release bronchoconstric-tion. After completion of this successful treatment, serum CK level was elevated up to 15, 800 IU/l three days later. Rhabdomyolysis was diagnosed. We could treat this complica-tion by mandatory aggressive volume replacement, mannitol, bicarbonate and acetazor-amide without blood purification. We discussed whether rhabdomyolysis resulted from asthma myopathy and/or volatile anesthetics. We must be aware of rhabdomyolysis in the management of status asthmaticus.View full abstractDownload PDF (516K) -
