    Retrospective study on clinical efficacy and safety of the Cepharanthin long-term use for the patient with oral lichen planus (OLP) was performed at 12 institutes as a multiple institution collaborative project. We examined the degree of the severity of the disease, improvement rate and the side effects of the Cepharanthin use at 4 weeks and 6 months after the prescription and at the endpoint of the Cepharanthin use for eighty OLP patients given the Cepharanthin for more than 4 weeks within a period from July 2008 to June 2013.
    More than 50% improvement rate was observed in almost all clinical types of OLP and the location of the lesions following to the Cepharanthin use. The improvement rate showed higher value at the endpoint of the Cepharanthin prescription, and long-term use revealed much higher efficacy for improving OLP. On the other hand, the dosage of the Cepharanthin showed no significant different effect for the OLP improvement. In addition, we recognized high improvement rate of the Cepharanthin use even in the case without steroid use. As a result, eighty percent of the OLP cases showed significant improvement by means of the Cepharanthin use, in addition, there was no significant adverse events following to the Cepharanthin use. From the above results, it can be concluded that the Cepharanthin has a significant efficacy for the treatment of OLP.

    Burning mouth syndrome (BMS) is a painful syndrome characterized by continuous burning sensation of the oral mucosa; it is one of the causes of non-odontogenic sharp pain.
    In European countries, it has been reported that local and systemic administration of clonazepam alleviates the burning sensation in the oral cavity. However, no study on clonazepam use in BMS patients has been conducted in this country. Nine patients with BMS presented with burning sensation in the oral cavity in the Dentistry and Oral surgery, Tokyo Dental College Ichikawa General Hospital, between April 2010 and March 2013. We initiated treatment with systemic administration of clonazepam and examined the response rate for these patients. In addition, we performed clinical evaluation for determining the appropriate dose and time to recovery following clonazepam administration.
    All patients were women, with an average age of 72.3 years. The main site of pain was the tongue. The initial dosage of clonazepam was 0.5mg, and an effect was observed in six cases in two weeks. The dosage was modified in the remaining three cases, following which an effect was observed within six weeks. Hence, a complete response was obtained in all nine cases. This study suggested that a low dosage of clonazepam administered systemically could be expected to provide relief in BMS patients.

    Non-bullous congenital ichthyosiform erythroderma (NBCIE) includes hereditary dermatosis caused by dyskeratosis along with hyperkeratosis and erythema. A 22-year-old woman with NCBIE was treated with orthognathic surgery under general anesthesia. Deliberate perioperative considerations are needed before anesthesia equipment can be attached to the skin and the body temperature of such patients must be appropriately managed. A normal body temperature was maintained throughout the procedure and no perioperative complications developed. We concluded that a concomitant forced-air warming system with an underbody blanket help to maintain the body temperature of patients with NCBIE during general anesthesia. Silicon tape is also useful for fixing tracheal tubes and intravenous catheters in place when patients with NCBIE undergo surgical procedures.

    Pleomorphic adenoma is the most common tumor arising in salivary glands. However, pleomorphic adenoma of the sublingual gland is extremely rare. Furthermore, sublingual tumor is difficult to diagnose, because the most cases are malignant. We report on a man with pleomorphic adenoma of the sublingual gland. His chief complaint was a mass in the left side of oral floor. The patient underwent removal of the tumor. The pathologic diagnosis was pleomorphic adenoma. There has been no evidence of recurrence for one and a half years after the operation. It is necessary to treat as malignancy, if the sublingual tumor is suspected.

    Amyloid deposition may occur in the oral cavity as a local symptom of systemic amyloidosis.
    We report a case of multiple myeloma with tongue amyloidosis. The patient was a 70-year-old female complained of swelling and pain of tongue. An ulcer in the tongue margin was observed and the biopsy specimen showed amyloid deposition. After systemic examinations, the patient was diagnosed as having multiple myeloma and so the lesion was a local systemic amyloidosis due to multiple myeloma.

    Recently Mikulicz’s disease has been considered to be the novel disease entity IgG4-related dacryoadenitis and sialoadenitis (IgG4-DS). We herein report a case of IgG4-DS associated with minor salivary gland swelling. A 26-year-old female complained of bilateral lower lip minor salivary gland, submandibular gland and lacrimal gland swelling. Severe IgG4 positive plasmacyte infiltration was detected in the minor salivary gland, and the serum IgG4 level was markedly elevated. In addition, autoimmune pancreatitis was detected, so the patient was diagnosed with IgG4-DS. IgG4-DS occasionally associates with the minor salivary gland. Therefore, in patients with glandular swelling, IgG4-DS is an important differential diagnosis.
