This study was focused current knowlekge in early diagnostic procedure by TBLB in the discrepancy of its usefulness, compared to that by open lung biopsy. Consideration process by Mark's monograph is useful, if applied it to other clinical data as a working clinical diagnosis. In cases of pulmonary vascular disturbances, TBLB was useful for early detection of hyaline membranes in ARDS and microembolism of fibrin in Pre DIC. If we could not find specific features in the specimens, the next step was taken for step sections which increase to detect specific findings. In some cases of a peripheral small lesion and with the negative results by usual procedures, we tried to get specific findings by using a fiberoptic bronchoscope with catheters to get bronchial secretions or Mini-BAL at the nearest site of a lesion, and by such procedures with tests of tuberculostearic acid detection and others, we were able to reveal a good response for the treatment.

Bronchoalveolar lavage fluid (BALF) from 14 cases of sarcoidosis was analyzed for glycosaminoglycans (GAGs). It was found that the BALF from 3 of 4 cases of stage II sarcoidosis contained detectable amounts of GAGs (hyaluronic acid, chondroitin sulfate, dermatan sulfate and heparan sulfate). In one case, in which BALF contatined detectable amounts of GAGs, interstitial infiltrate and lymphadenopathy persisted even after treatment with steroids. The second case, in which the chest x-ray findings worsened, contained detectable amounts of GAGs. In the third case, in which BALF contained detectable amounts of GAGs, pleural effusion accumulated 2 months after admission. The BALF from 3 other cases, whose chest x-ray findings improved spontaneously or after treatment with steroids, did not detectable levels of GAGs. The BALF from 8 cases, whose chest x-ray films showed lymphadenopathy alone, did not contain detectable levels of GAGs. It was suggested that the amounts of GAGs in BALF from cases of sarcoidosis differ depending on stages.

Bronchial embolization using fibrin glue was performed in 10 patients suffering from prolonged pulmonary air leak. Six of the cases were spontaneous pneumothorax, and 4 were bronchopleural fistula following lobectomy. In 8 air leak was blocked but 2 were not. We could not identify the drainage bronchus with the air leak in one, and we failed to blow fibrin glue in the correct drainage bronchus in another. In 2 cases of 8 in which the air leak was blocked recurrence occured. In this method, the air leak may disappeare because the inflammation in the embolized lung accelerates the healing of the bronchopleural fistula, in addition to the effect of temporary obstruction of drainage bronchus. Bronchial embolization using fibrin glue is a technically easy and effective method for the treatment of bronchopleural fistula, with minimed side effects.

In a study on preservation of the circulation of extensive tracheal transplants, an eight-ring segment of trachea was resected and was orthotopically transplanted in 11 mongrel dogs. These transplanted tracheas were covered with latissimus dorsi muscle flaps. The transplanted tracheas were observed by bronchofiberscopy and fluorescent endoscopy, and dogs were examined microscopically after sacrifice. Revascularization of the transplanted trachea was comfirmed by injection silicon rubber particules or barium into the thoracodorsal artery. Seven dogs survived more than 1 month after operation and only 1 dog developed tracheal stenosis. Revascularization of the transplanted trachea from the both anastomotic sites and the latissimus dorsi muscle was observed from the 7th. postoperative day on. From the fourth or fifth week, the transplanted trachea showed complete revascularization and normalization. Moreover, regeneration of all around the entire transplanted trachea was shown from the eighth week on, microscopically. This experimental study suggested the this method was effective to preserve circulation in extensive tracheal transplants.

Between 1983 and 1988, we treated 11 children with foreign bodies in the tracheobronchial tree. All patients were under 10 years of age, and nine were boys. Ten of the foreign bodies (91%) were food (peanut : 7 ; noodles : 2 ; corn : 1) which were removed safely with a fiberoptic bronchoscope. A Fogarty balloon catheter was useful for removing peanuts. Foreign bodies were removed under general anesthesia. The first three patients were not intubated, and the fiberoptic bronchoscope was inserted directly with spontaneous breathing. The last eight patients were intubated, and foreign bodies were removed safely with forced ventilation. The fiberoptic bronchoscope is useful for removing tracheobronchial foreign bodies in children.

A case of a 69-year-old male who aspirated a fragment of peanut into his bronchus was complicated by pyothorax. When he presented with dyspnea, he was treated for bronchial asthma because he had previously experienced bronchial asthma. Though fever and chest pain were added to his complaints a few days later, these were thought to be due to acute upper airway inflammation. However, as the chest radiogram showed right pleural effusion, he was admitted. We diagnosed this case as pyothorax from the results of the examination. A fragment of peanut was found in the orifice of the right basal bronchus by bronchofiberscopy. It was considered that this case had been aspiration pneumonia complicated by pyothorax caused by the peanut. However he did not remember having aspirated the peanut. Attention must given to such cases of bronchial foreign bodies without aspiration history because they are often diagnosed from the secondary symptoms (for example, pneumonia, atelectasis, emphysema, etc.) due to the foreign body presence.

We reported a successful case of bronchoscopic therapy for postoperative bronchial fistula. A 51-year-old man underwent right pneumonectomy for adenocarcinoma. Five months after the surgery bronchoscopy and bronchogram revealed a fistula at the surgical stump. At this time we attempted to close the fistula with cyanoacrylate (Aron Alpha A) administered through the bronchoscope. Four weeks after the transbronchial treatment, the closure of the fistula was confirmed by bronchoscopy. Unfortunately the patient died of metastasis to the brain and pancreas 10 months after surgery. This type of transbronchoscopic therapy for postoperative bronchial fistula can be effective for small and early fistula without infection.

A 61-year-old man had undergone bilobectomy of the right middle and lower lobes because of squamous cell carcinoma that developed in B^7 10 months previously. Six months after the surgery slight protrusion in the left upper trunk was detected by routine bronchoscopy. Endoscopy was repeated 3 times at 3-week intervals and atypical squamous cell metaplasia was revealed by biopsy specimens and brushing cytology. He was readmitted, complaining of dyspnea, 10 weeks after the last bronchoscopy and the left main bronchus was almost completely occluded by a polypoid squamous cell carcinoma that developed from the left upper trunk. After the tumor almost totally vaporized by YAG laser treatment, sleeve resection of the left upper trunk was successfully performed.

Three cases of pulmonary aspergillosis, Productive aspergilloma on the inner wall of a cavity, fungus ball type aspergilloma and allergic bronchopulmonary aspergillosis, were admitted to our hospital. The patients were treated with intravenous infusion of miconazole, an anti-fungal drug. Two aspergillomas were treated with CT-guided percutaneous and bronchofiberscopy guided endobronchial intra-cavitary infusion of miconazole. They had no significant side-effects, symptoms and chest X-ray findings also improved. We consider these methods of treatments to be very effective.

Flexible fiberoptic bronchoscopy without intubation was performed in a total of 8 children under 7 years of age. The youngest patient was 29 days old and weighted 3, 150g. The indications for bronchoscopy were stridor, suspicious of foreign bodies, respiratory infection, and dyspnea. Thirteen procedures were performed, 11 of which were without intubation. Three types of bronchoscopes (Olympus BF type P10, 4B2, 3C10) were used. All examinations were performed under general anesthesia. There were on severe complications during or after examinations. Transnasal insertion of the FFB was better than transoral insertion because of good stability and easy manipulation. It was not difficult to perform maks ventilation during examination with the BF type 3C10 even in 1-year-old babies. When the bigger bronchoscopes (BF type P10, 4B2) were used, 100% oxygen supply through the channel was useful to maintain adequate O_2 saturation. Each insertion should be finished within 1 minute. Our experience suggested that the flexible fiberoptic bronchoscopy was useful in the observation and treatment of pediatric bronchial cases and also could be safely performed even without intubation.
