A 68-year-old man was admitted with a chief complaint of dyspnea. Chest radiography showed rapidly progressing interstitial pneumonitis. Under a diagnosis of acute exacerbating interstitial pneumonitis, high dose methyl prednisolone and cyclophosphamide were given. The interstitial lung shadows gradually improved. A month later, his leucocyte count decreased and several patchy shadows with cavities appeared in both lung fields. Because aspergillus fumigatus was detected from bronchial lavage fluid, we diagnosed chronic necrotizing pulmonary aspergillosis. After treatment with Amphotericin B and 5-flucytosine, these patchy shadow with cavities were gradually resolved except for one lesion. High dose steroid and cyclophosphamide administration should be carried out with care in the treatment of interstitial pneumonitis.

A 37-year-old woman had fever, heliotrope erythema, an erythematous and scaly rash with Gottron's papules over the knuckles, and proximal muscle weakness. Creatine kinase was elevated at 199 U/L. An electromyogram showed myogenic pattern and fibrillation. Chest roentgenogram showed diffuse bilateral patchy infiltrates in the periphery of the lungs. Videoendoscopy revealed diffuse multiple small nodular lesions in the tracheobronchial wall. Uveitis was also noted. Transbronchial biopsy specimens from the bronchial mucosa revealed noncaseating granulomas with occasional giant cells, while lung parenchyma specimens revealed interstitial pneumonia. These findings suggested the coexistence of sarcoidosis with dermatomyositis.

A 21-year-old man was admitted to our hospital because of fever, cough, bloody sputum and dyspnea. He had been treated for depression and other psychosomatic diseases since the age of 20 years. The night before admission, he came home very drunk and inhaled domestic insecticide which he mistook for oxygen spray. On admission, he was hypoxic and chest X-ray revealed infiltration in both lower lobes. Bronchoscopic lung biopsy was attempted but cytologic diagnosis was not possible because of poor patient cooperation. Gas chromatographical analysis of the bronchial lavage fluid revealed an oil component (kerosene) contained in the inhaled insecticide. Consequently, extrinsic lipoid pneumonia was strongly suspected. During hospitalization, pleural effusion and nodular shadows were observed by chest X-ray, although these conditions improved with steroid treatment.

A 32-year-old woman with cervical bronchogenic cyst diagnosed by resection is described. The patient was under treatment for hyperthyroidism. Computed tomography and ultrasonic echogram examination revealed bilateral cystic masses in the lower neck. The patient was operated on via transverse neck incision. Operation revealed that the left cyst had no connection with the thyroid. The left cyst was approximately 4cm diameter, the wall was thin and contained white turbid mucus. Histologically, the cyst consisted of ciliated epithelium, smooth muscle, mucus glands and this evidence established the final diagnosis of bronchogenic cyst. The right cyst was diagnosed adenomatous goiter. Surgical excision is indicated for cervical bronchogenic cysts because of their compression of contiguous structures and tendency to become infected. The postoperative course was uneventful and the patient was discharged 7days after operation. This is the 10th reported case of a cervical bronchogenic cyst in the Japanese literature.

A 78-year-old man was admitted to our hospital with dysarthria due to a transient cerebral ischemic attack (TIA). A chest X-ray film on admission revealed organizing pneumonia in the right upper lobe. Subsequent bronchoscopic examination revealed a granulomatous polyp in the trachea. Pathological findings of the biopsied specimen revealed that it had been caused by inflammation. Bronchoscopy revealed that 11-month of beclomethasone dipropionate inhalation therapy 800μg/day had no effect. We performed high frequency current snare polypectomy and subsequent Nd-YAG laser therapy because it was difficult to distinguish from a malignant tumor. We conclude that the combination of high frequency current polypectomy and Nd-YAG laser therapy may be useful for tracheal polyps.

A 57-year-old man with metastatic mediastinal lymph-node carcinoma of unknown origin invading the trachea complained of bloody sputum and dyspnea. Bronchoscopic examination revealed tracheocarinal stenosis caused by the malignant tumor. Transbronchoscopic electrosurgery was performed four times to eliminate the dyspnea. A dynamic stent was inserted under general anesthesia. The electric scalpel and forceps designed for the flexible bronchoscope were useful for treating the neoplastic stenosis. The dynamic stent was useful for improving the quality of life in this case.

Three cases of airway stenosis caued by granulation were successfully treated by bronchoscopic intralesional injection of triamcinolone acetonide (TA). The first case was a 48-year-old man in whom bilateral vocal cord paralysis developed after thyroidectomy for thyroid cancer. Tracheostomy was done and a T-tube was inserted. After 1 year, T-tube removal was attempted but it was not possible due to the presence of subglottic stenosis caused by granulation. Bronchoscopic intralesional injection of TA was performed, which reduced the stenosis and allowed T-tube removal and stomal closure. The second case was an 80-year-old woman with tracheal stenosis due to granulation caused by intubation. Bronchoscopic laser irradiation alleviated the stenosis, but 1 month later granulation recurred at the stenotic site. After multiple intralesional injections of TA, the stenosis did not recur and the patient was discharged. The third case was a 70-year-old woman who had undergone sleeve segmentectomy of right S^2. She was found to have stenosis of the right upper lobe bronchus just proximal to the anastomosis due to granulation. The stenosis subsided after intralesional injection of TA. Intralesional injection of TA performed under bronchoscopy is technically feasible and is minimally invasive. So far, no apparent side effects or complications have been reported. It can be considered as an alternative to airway reconstruction or stenting in the treatment of airway stenosis caused by granulation.

A 66-year-old man was admitted to our hospital due to recurrent hemoptysis. On bronchial arteriography, an aberrant artery originating from the celiac artery via left gastric artery and communicating with the pulmonary circulation was pointed out. He received embolization therapy of the aberrant artery. Chest CT suggested the existence of intralobar pulmonary sequestration in the right lower lobe. He had no recurrence after the embolization therapy. This was a case of intralobar pulmonary sequestration with a rare type of aberrant artery.
