    This paper contains
    I. A historica l review and general indications for surgery of synovectomy in rheumatoid arthritis.
II. General indications of joint surgery for RA with various methods of surgery applied to various joints as stages progress.
III. Masuta Mori's knee joint synovectomy
    a) precise description of technique.
    b) Postoperative treatment
    c) One example for deciding the limitation of Dr. Mori's surgical treatment relating to the unstable knee joint.
    IV. Discussion
    A. Summarizing review of Rheumatoid surgery up to the present including Swett, Ghormley, SmithPetersen, London, Vainio
    B. Several surgical experiments made by several surgeons in order to enlarge the surgical indications of the Mori surgery technique.
    V. Conclusion
The Mori surgery technique is obviously less atraumatic in intervention, though there is a greater amount of surgical removal of morbid rheumatoid tissue from the knee than is the Sweet's technique. The Mori technique should be called "Pericapsular Capsulosynovedctomy, " by which the realcause of flexion contracture is found in the anterior compartment morbid synovia and morbidly swollen supra and infra patellar fat pads of the Knee joint.

From the extensor compartment a total excision of the joint (please refer to Figure 1-a) capsule is made in the 4 block dissection of the extensor compartment of the knee,2 block dissection in the extension compartment of the wrist, elbow and ankle joints. As for the flexor side compartment, the total joint capsule rem-oval in 2 block dissection pattern in the flexion side of the ankle and approximately one total joint capsule on the radial side in addition to the resection of the radial head. Thus a large window is created in the extensor side, giving very good view for the dbridement of the affected joint. In late stage synovectomy quite a procedure to eliminate the established contracture becomes needed. You can realize the deepness and extension to be of the close correlation between the results and the amount of the joint capsule removal.

To specify which brain structures generate ste ady-state visual evoked potentials (SSVEPs) to flash stimulation, source derivation (SD) technique and coherence analysis were carried out in rabbits. SD technique is sensitive to current activity generated in the superficial cerebral cortex. In our study, P21 as a representative of the 30Hz SSVEPs recorded from diffusely over the head using liked-ears reference (LER) was hardly detected at any leads by SD, whereas the wave P57 as a representative of the 10Hz SSVEPs was recorded from over the visual cortex contralateral to the stimulation using LER, while its distribution was more localized by SD. Coherence between the signals from the left lateral geniculate nucleus (LGN) and cerebral surface electrode (P3) was measured using 30 and 10Hz stimulations delivered to the right eye. With 30Hz stimulation, the coherence value at the fundamental frequency was large (0.88). With 10Hz stimulation, the coherence value at the fundamental frequency showed a small peak (0.66). Interpretation of SD and coherence study suggests the location of neural generators as follows: 1) The 30Hz SSVEPs originate from far-field, possibly the contralateral LGN.2) the 10Hz SSVEPs consist of potentials from two sources: the near-field potential from the visual cortex and far-field potential probably originating from the contralateral LGN.

Our previous study on spike topography wit h source derivation method showed formation of mirror foci in both cerebral cortical and ventral hippocampal (V-HPC) kindled rabbits. To evaluate the contribution of the corpus callosum (CC) and corpus anterior (CA) to the genesis of mirror foci in rabbits, the effects of electrolytic lesions of CC and CA on the spike amplitude ratio in the mirror/primary sites were studied. Lesions even in CC or CA alone produced significant attenuation in the amplitude ratio. In VHPC kindled rabbits, combined CC and CA lesions produced more significant post-lesion attenuation compared with the CC alone lesion group, whereas there were no significant differences between solitary and combined lesions in cortical kindled rabbits. These results suggest that in seizures of cortical origin, CC is the main pathway for interhemispheric propagation and CA is an additional pathway, while CA has a major role and CC plays a secondary role in seizures of V-HPC origin.

We analyzed the expression of CD9 antigen and GPIIb/IIIa-like protein by activated human umbilical vein endothelial cells (HUVECs) incubated with tumor necrosis factor-alpha or lipopolysaccharide using flow cytometry and two anti-CD9 monoclonal antibodies (NNKY1-19 and MALL-13; raised using platelets and ALL cells, respectively) as well as an anti-GPIIb/IIIa monoclonal antibody (NNKY2-6). Using the anti-CD9 antibodies, it was found that the fluorescence intensity of activated HUVECs was higher than that of untreated control cells. However, fluorescence for NNKY2-6 showed no significant difference between activated HUVECs and untreated controls.
These findings suggested that CD9 antigen ex pression by HUVECs increases in response to activation. On the other hand, there was no significant difference of GPIIb/IIIa-like protein expression between activated HUVECs and control cells. It is well known that anti-CD9 antibodies can induce platelet activation, but the actual function of CD9 antigen is still unknown. The results of this study suggest that it plays a role in the adhesion of hematopoietic cells to ECs or in the procoagulant activity of ECs.
