    In an attempt to elucidate the clinical significance of serum NSE level in patients with advanced NSCLC, I determined the pretreatment serum NSE levels in 154 patients who received chemotherapy. Of them, 45 (29%) showed an elevated level of ≥ 10ng/ml. Patients with elevated NSE level were significantly more frequent in stage IV than in stage III. The median duration of response to chemotherapy was significantly shorter in patient with an elevated level than in those with a non-elevated level (4.9 months vs 6.7 months; p<0.05). Patients showing an elevated NSE level survived significantly shorter than those showing a non-elevated level (6.2 months vs 13.1 months: p<0.001). A multivariate analysis revealed that the pretreatment NSE level, clinical stage, and pretreatment serum albumin level were independent prognostic factors in patients with advanced NSCLC receiving chemotherapy. Additionally, I stained 46 tumor specimens histochemically using the anti-NSE antibody. However, the stainability did not correlate with the serum NSE level. These findings indicate that the elevated NSE in the sera did not originate in the expression of neuroendocrine nature of NSCLC tumors but in the bulk of tumors and cell kinetic characterisitics, e. g., rapid growth with large cell death.

    Both NG-monomethyl-L-arginine (1.0×10-5mol/kg), which is the specific inhibitor of nitric oxide formation, and D-arginine (1.0×10-4mol/kg) or L-arginine (1.0×10-4mol/kg) were administered intravenously to albino rabbits under local anesthesia, and choroidal blood flow was measured by the hydrogen clearance method until 60 minutes after administration.
    The effect of NG-monomethyl-L-arginine, which caused vasoconstriction of the choroid, was partially reversed by the addition of L-arginine. These findings suggest that the endothelium-derived relaxing factor from L-arginine participates in the regulation of choroidal blood flow.

    Integrins are a family of adhesion molecules which mediate adhesion and interaction between cells and extracellular matrix. The renal glomerulus is composed of cellular components and extracellular matrix. To clarify the role of integrins in the progression of glomerulonephritis, we examined the expression of β1 integrin and fibronectin, which is a ligand of β1 integrin, in the glomeruli in patients with IgA nephropathy and membranous nephropathy by an indirect immunofluorescence study and immunoelectron microscopic study using specific antibodies. β1 integrin was expressed on the surface of mesangial cells, glomerular epithelial cells, endothelial cells and Bowman's capsullar epithelial cells in normal control glomeruli. The expression of β1 integrin and fibronectin was increased in the mesangial area in patients with IgA nephropathy showing mesangial proliferation and mesangial matrix increase. β1 integrin may play an important role in the construction of the glomerulus and also in the deposition of immunecomplex, proliferation of mesangial cells, and increase in extracellular matrix in IgA nephropathy.

    In a rat model, halothane causes liver injury by interaction with microsomal cytochrome P450 (P450) under a hypoxic condition. Rats that were pretreated with phenobarbital and exposed to halothane under reduced oxygen tension for 2 hours developed hepatic centrilobular necrosis with marked elevation of serum alanine aminotransferase (ALT) 24h after exposure. The elevation of ALT was significantly suppressed in the rats pretreated with a 10mg/kg dose of zinc (Zn) 24h prior to the exposure in comparison with the rats pretreated with saline (control rats). The region of necrosis was also significantly reduced in Zn-pretreated rats. The P450 content of the hepatic microsomes was slightly decreased before the exposure in Zn-pretreated rats in comparison with that in the control rats. It was noteworthy that the aminopyrine demethylation (AD) activity of hepatic microsomal P450, which was markedly decreased after the exposure in control rats, was maintained after the exposure in Zn-pretreated rats. These findings indicate that Zn-pretreatment has some protective effect against halothane-induced liver injury and suggest that this protective effect of Zn involves not only the decrease of P450 before exposure but also the preservation of AD activity during exposure, which result in reduced interaction between P450 and halothane in the microsomes.

    Four cases of dissected human coeliac and other mesenterial arteries are presented. Typologic discussion of these cases indicates that the arterial system of human mesentery vessels basically consists of left gastric, splenic, (common) hepatic, superior mesenteric and inferior mesenteric arteries, and that these arteries, like the inferior phrenic and inferior rectal arteries, are originally paired vessels (left and right). The left gastric, (common) hepatic and superior mesenteric arteries are thus derived from the right-sided vessels, while the splenic and inferior mesenteric arteries arise from the left-sided vessels. The aberrant accessory colica media artery is a typical left-sided vessel. The celiac trunk is also left-sided vessel.
