    We investigated the relationship between physical and visual image quality of asymmetric screen/film systems. The physical image quality parameters included the characteristic curve, modulation transfer function (MTF), and noise Wiener spectrum. The visual image quality indices, such as sharpness and graininess, were determined using a method of paired comparisons of chest phantom radiographs. When the speeds of asymmetric systems increased, the visual evaluation for sharpness was lower but that for graininess was higher. Both visual evaluations for sharpness and graininess were lower with higher kV X rays. Visual graininess was closely correlated with the Wiener spectrum, but sharpness was only slightly correlated with the MTF. The sharpness of the asymmetric systems was inferior to that of the conventional system when the same tube potential was used. Therefore, a lower tube potential or a speed ratio of 1:2 for the front and back screens should be applied for clinical use of the asymmetric systems.

    To cope with problems resulting from a rapidly aging society with a low birth rate, effective methods must be established to support home care services through a welfare service system. Characteristics of 40 elderly women living in S City with their families who had stopped receiving “day service” (discontinued group) were compared with 40 age-matched elderly women selected among those regularly receiving the day service at the same facility. In addition, factors that influenced the women to stop receiving “day service” were studied. A questionnaire consisting of 20 items concerning various activities of daily living (ADL), instrumental ADL (IADL), health states, economical conditions, and reasons for quitting the service was completed by all subjects.
    The results showed that ADL, IADL and health states were significantly lower in the discontinued group than those of the control group. About half of the elderly in the discontinued group quit the services in order to receive medical care at medical institutes. These women were older in age and lower ADL and IADL than those who quit for other reasons. Time needed to care for their husbands and housekeeping, were also reasons for quitting the day service in the relatively younger group whose ADL and IADL were well maintained.
    Therefore, I concluded that “day service at welfare facilities” may become competitive with medical and health facilities already supporting home care services for the aged, if knowledge increases about social resources to support home care services.

    I studied the relationship between aging and exocrine pancreatic function by endoscopic retrograde aspiration of pure pancreatic juice (PPJ). Control subjects consisted of 65 outpatients presenting mild vague abdominal symptoms who fulfilled the following three criteria: 1) good general condition with no known organic diseases; 2) no abnormality in the liver, bile duct, pancreas, kidney or metabolism; 3) no alcohol consumption. Nineteen patients with definite pancreatitis were also studied. PPJ was collected from within the pancreatic duct by endoscopic retrograde catheterization of the papilla for 10 minutes after a bolus intravenous injection of secretin (Secrepan, Eisai Co., Ltd., 100 U/body). Exocrine pancreatic function was evaluated three parameters as follows: secretory volume, maximal bicarbonate concentration or bicarbonate output, and enzyme (amylase and lipase) output. The ordinary duodenal secretin test was also performed. Exocrine pancreatic function (both duct-cell and acinar-cell function) was significantly reduced in the elderly. The age-associated reduction showed a different pattern between duct-cell and acinar-cell function. The degree of duct-cell dysfunction was significantly higher than that of acinar-cell dysfuntion in the elderly. Influence of aging on maximal bicarbonate concentration was more clearly demonstrated by PPJ aspiration than by the ordinary secretin test. Elderly controls showed as much exocrine dysfuction as patients with chronic pancreatitis, making it difficult to diagnose chronic pancreatitis based on the exocrine function test alone. Therefore, clinical symptoms and findings as well as imaging tests should be considered for the correct diagnosis.

    An electroencephalographic (EEG) study with a multi-channel digital record was carried out to investigate EEG changes and their spatial characteristics accompanying hypothermia and cerebral ischemia during open-heart surgery in children.
    Subjects consisted of 10 children, ranging from 1 year 6 months to 4 years 6 months old, who underwent open-heart surgery with extracorporeal circulation and hypothermia. The EEG changes were evaluated visually and analyzed using a computer.
    (1) A discontinuous EEG pattern was noted during cooling in addition to commonly observed findings including a decrease in fast waves, slowing and amplitude reduction, and was considered caused by rapid and transient suppression of cerebral function. (2) Power spectral and topographical analyses revealed a prominent decrease in fast waves and a relative increase in slow waves during cooling. During cerebral ischemia, in contrast, an actual increase in slow waves and decrease in fast waves were observed. (3) We noted the characteristics of bilateral synchronous rhythmic high-voltage slow waves among various types of transient EEG abnormalities. (4) EEG changes induced by hypothermia differed by brain region. (5) EEG changes induced by hypothermia were influenced not only by body temperature itself but also by the rate of change in body temperature.

    We investigated the curent status of home nursing in our institution, and visiting nurses' station “chime”, to clarify current problems and possible improvements in home care service. A large number of the subjects were aged, and had various diseases usually involving cerebrovascular disorder. Patients requiring medical treatment comprised less than half, and it was considered that support was strong in the current home nursing siuation, particuulrly. We examined the age status of persons who looked after the patients, and tce burden on women who provided care. Furthemore, problems with the prenent visit nursing syntem were evaluated by an anonymous questionnaire survey of patients and their attendants. Many of the subjects were worried or troubled about what to do about exacerbation or sudden changes of he patients' condition, the health of the caretakers, and the tight schedule of the caretakers' life. Based on the results of this questionnaire, we would like to improve the cooperation among members the visiting nurse staff including doctors, and physiotherapists so that each of them may better play their roles and provide more appropriate nursing based on the wishes and needs of individual patients and their caretakers rather than those of the uniform service.

    Purpose. To test whether encagement and volume load as stressors influence the intraocular pressure in rabbits.
    Method. Rabbits were encaged for an hour in a horizontally-plastic tube with an internal diameter of 13.2 cm and a length of 33.2cm. After the encagement, rabbits received a rapid intraveonus drip infusion of 5% glucose solution, 20 ml per kilogram of body weight, for 5-6 minutes. All experiments were performed at the same time of day.
    Results. The intraocular pressure after encagement (11.2±3.0mmHg as a mean and standard deviation) was significantly higher compared with that of control rabbits without encagement (9.2±1.0mmHg, Student t-test, p=0.046). This difference became significantly larger when volume load was exerted on both groups of rabbits (p=0.0007). Serum levels of cortisol, adrenaline, and nordrenaline were significantly elevated afte encagement (n=6, p=0.0002, p=0.027, p=0.030, respectively). Venous pressure of the ear tended to increase in rabbits after encagement (15.8±3.1mmHg) compared with that of control rabbits (8.5±2.3mmHg, p=0.11), and the difference became significant when volume load was placed on each group of rabbits (p=0.021).
    Conclusions. Encagement, especially in combination with volume load, increased the intraocular pressure in rabbits. The elevation of peripheral venous pressure caused by an increase in serum catecholamines would underlie the elevation of intraocular pressure probably through the increased outflow resistance.

    DNA topoisomerases are enzymes that catalyze topological changes of DNA by transiently cleaving one (topo I) and a pair of complementary DNA strands (topo IIβ), participating in cellular transactions of DNA. Expression of both enzymes in the developing cerebellar plate in rat embryos was studied immunohistochemically. Neuroepithelial cells in the ventricular layer expressed topo I and topo IIβ on embryonic day 15 (E15) and E16, respectively. On E 16-22, cells in the intermediate layer (IML) are known to undergo extensive differentiation to become Purkinje cells and deep cerebellar neurons. Many IML cells, immunostained with not only anti-topo I and- IIβ but also-calbindin antibodies, were differentiating and migrating to form the Purkinje cell layer. Cells in the external germinal layer (EGL) on E17, originating from the germinal trigone and spreading over the surface of cerebellar plate, expressed topo I on E17. Topo IIβ immunoreactive cells, however, were not readily recognized in EGL untill E22. The topo IIβ immunopositive cells resided in the ventral half of the layer, corresponding to the premigratory zone of postnatal EGL. These date suggest the involvement of topo I and IIβ in DNA metabolism associated with cerebellar development in embryos, especially that of topo IIβ in the processes of neuronal differentiation.
