    A cardiovascular epidemiologic study was performed in three different areas in Okayama prefecture in 1982. The incidence of hypertension in Sakuto-cho was higher than in the other two areas. However, the incidence was less than in previous studies. Conversely, the incidence of ischemic heart disease was higher in Sanyo-cho. In the group of essential hypertensions, IHD and obesity, HDL -cholesterol was low, and total cholesterol, triglyceride and LDL-cholesterol were high compared with the normal group. In women, with or without disease, total cholesterol and LDL-cholesterol were high compared with men, while the HDL-cholesterol/total-cholesterol ratio was low. Along with an increase in age, HDL-cholesterol decreased, and total cholesterol and LDL-cholesterol increased in men and women. HDL-cholesterol in Sakuto-cho was signifi-cantly high, and total cholesterol and LDL-cholesterol in Osafune-cho and Sanyo-cho were low.

    The effects of MPTP on non-dopaminergic neurons were studied in the striatum of ICR mice, by means of enzyme histochemistry for neurons containing acetylcholinesterase (AChE) or NADPH2 -diaphorase and of immunohistochemistry for neurons containing GABA, serotonin or enkephalin.
    The most prominent changes were observed in immunohistochemical staining for enkephalin, 3 days after the treatment with MPTP. Enkephalinimmunoreactive nerve fibers and terminals were increased both in number and in intensity, particularly at enkephalin-rich islands seen in the rostral striatum. In addition, enkephalin-positivepass-through fibers and cell bodies were also increased throughout the striatum. The highest density of these cell bodies was found in the caudal striatum. These changes in enkephalin immunoreactivity became less evident day by day, and 60 days later the staining result appeared almost the same as that in control mice. In contrast, serotonin-immunoreactive nerve fibers became sparse, particularly in ventrolateral parts of the striatum, from 7 days after the treatment with MPTP. Such a change was obsesved for as long as 60 days, so that it was suggested that the MPTP treatment caused irreversible degenerative changes in serotonin nerve terminals of the striatum.
    On the other hand, no significant change was recognized jn other striatal nerons containing AChE, NADPH2 -diaphorase or GABA.
    The present study indicates that some non-dopaminergic neurons, either intrinsic or extrinsic to the striatum, are affected by the MPTP treatment and therefore an important to the understanding of the etiology of Parkinson's disease.

    For studing aberrant cellular immunity in malignant digestive tract diseases, Helix pomatia A hemagglutinin (HP) binding activity, a T cell surface marker, to peripheral lymphocytes was studied in patients with digestive tract cancer. In this paper, basic experiments on an assay method based on 125I-labelled HP binding to neuraminidase-treated peripheral lymphocytes are described. HP was labelled with 125I using the chlormine-T method, and the purification of 125I-labelled HP was achieved by agarose-aminocaproyl-D-galactosamine affinity chromatography.
    The results show that 125I-labelled HP could specifically bind to neuraminidase-treated lymphocytes as cold HP. The optimum assay condition was determined to be: 1×106 neura-minidase-treated peripheral lymphocytes incubated with 0.4μg 125I-labelled HP in 0.5 ml of reaction mixture at 25°C for 90 min with gentle shaking.
    In addition, the effect of treatment of peripheral lymphocytes with neuraminidase on E-rosette formation was studied. Enhancement of E-rosette formation by neuraminidase-treat-ment was not statistically significant.

    Helix pomatia A hemagglutinin (HP) binding activity, a surface marker of peripheral T cells, to neuraminidase-treated peripheral T cells was studied in patients with digestive tract cancer or benign liver disease and in normal subjects. The HP binding activity (ng/106 T cells) to T cell subpopulations in peripheral lymphocytes was calculated as:
    amount of bound HP to total peripheral lymphocytes (ng/106 cells)/Ratio of T cells to total peripheral lymphocytes (%)×100. The calculated HP binding activity to T cells was significantly higher in digestive tract cancer patients than in benign liver disease patients (p<0.05) and normal subjects (p<0.01). These results suggest that either the subsets of peripheral T cells with high HP binding activity may be significantly increased or HP receptors of all peripheral T cells may be increased to the same degree in digestive tract cancer patients. This activity may be a useful index of cellular immunity in digestive tract cancer patients, although it seems to have no relevance to the progression of cancer.

    In order to clarify the pathophysiology of allergic disorders, it is necessary to study the reactivity of mast cells. Recently, evidence that mast cells have specificities in species and organs has been reported. For use in an investigation of bronchial asthma, I developed a suitable technique for the purification of mast cells using normal lung tissue from patients with lung cancer. The purification steps were: (1) dispersion of lung fragments into a single cell suspension with an enzyme mixture (pronase-chymopapain and collagenase-elastase), (2) centrifugation using a Percoll continuous gradient, and (3) exclusion of adherent cells. As a result, 2.1±1.1×105 mast cells per gram of lung tissue in purities of 38.8±7.2% were obtained. Isolated mast cells proved functionally and morphologically stable by the following findings. The cells showed: (1) dye exclusion exceeding 95%, (2) a histamine content of 4.3±0.6pg/mast cell, (3) a histamine release of 47.9±11.2% induced by calcium ionophore A 23187, and (4) almost no damage on scanning electron microscopic observation. These results indicate that the present technique for the purification of human lung mast cells is comparatively simple and useful. The isolated mast cells can be applied to pathogenetic and therapeutic studies of bronchial asthma.

    Many pathophysiological and therapeutic investigations on allergic pulmonary diseases have been reported using chopped lung tissue or rat peritoneal mast cells. However, there are some problems with these experimental systems. The major problem is the difference in the responsiveness of mast cells to stimuli among species or organs. In this study, reactivity of isolated human lung mast cells to immunological and non-immunological stimuli was examined. Isolated human lung mast cells showed a maximum histamine release of 47.9±11.2% at 10 μg/ml of calcium ionophore A 23187. The reaction was dependent on the concentration of calcium ionophore A 23187 and extracellular Ca2+. Compound 48/80 failed to induce histamine release from the mast cells below the concentration of 1000 μg/ml. The optimal condition for the passive sensitization of the mast cells was incubation at a final concentration of 1000 U/ml in IgE serum at 37°C for 2 hours. This treatment significantly enhanced immunological response. Maximum histamine release of 36.0±5.7% from the mast cells after passive sensitization was shown by anti-IgE. The reaction was dependent on the concentration of anti-IgE and extracelullar Ca2+. The mast cells from a patient with buckwheat allergy released histamine by treatment with anti-IgE, specific antigen, anti-IgG and anti-IgG4 in a concentration-related manner. Isoproterenol and nicardipine inhibited histamine release induced by anti-IgE in a dose-dependent manner, indicating maximum inhibition rates of 37.8±7.0% at 10-6M and 41.2±11.9% at 5 μg/ml, respectively. Scanning electron microscopic examination showed the absence of microvilli and degranulation of the intracellular contents after immunological stimulation. Acetylcholine induced significant histamine release from the mast cells at a concentration of 10-4M. These results suggest that human lung mast cells have specific reactivity to some stimuli. Further studies of human lung mast cells are required to clarify the pathogenesis of allergic pulmonary diseases.

    Cerebrospinal fluid (CSF) proteins, as resolved by microscale two-dimensional electrophoresis and visualized by silver staining, were analyzed quantitatively in cerebrovascular diseases (CVD) and other neuropsychiatric diseases. Some remarkable quantitative changes in CSF proteins were observed in patients with CVD, however, no change was detected in patients with schizophrenia and epilepsy. In CVD, patterns of CSF proteins appeared to vary during the course of the disease. In the first week from the onset, three neutral proteins of similar molecular weights (Mr 43, 000) were increased. In later stages, these proteins began to diminish to the previous level but several other proteins started to increase. Three of them were similar in size (Mr 37, 000) and had isoelectric points (pI) slightly more acidic than albumin. Proteins of Mr 39, 000-40, 000 highly represented in CSF were also increased in the late stage. These proteins are enriched in human serum, as well, and have been shown to decrease in CSF from aged subjects. Another subset of proteins increased in this period were very acidic (<pH 4.0) and consisted of 6 to 7 polypeptides. These polypeptides were barely detectable in CSF and serum of normal persons. It was suggested, from the proteins' molecular weight and charge, that these are proteolytic products of glial fibrillary acidic protein. After one month from the onset, the group of proteins at Mr 37, 000 were decreased to the previous level but the others remained unchanged.
    Among proteins which were detected in CSF from patients with CVD but not in normal CSF, Mr 40, 000 protein with pI similar to albumin was most significant. The protein was detected not only in CVD but also in schizophrenia, suggesting that some pathological changes in these diseases are common. In fact it has been reported that 40% of patients with CVD developed psychiatric symptoms. The occurrence of common CSF proteins in these apparently different diseases (organic vs. functional) may provide a clue to the pathogenesis of psychopathy.

    A questionnaire study on factors related to birth weight was carried out in a “Buraku” area and control area from 1975 to 1984. Altogether, 111 births were covered in the “Buraku” area, and 309 in the control area.
    The incidence of low birth weight was found to be significantly higher in the “Buraku” area than in the control area. Factors related to the high incidence of low birth weight were observed more frequently in the “Buraku” area than in the control area. Among these factors, the duration of gestation, the number of days-off before delivery, and the mother's physical structure and smoking habits were found to be strongly related to the incidence of low birth weight. Such factors as the number of days-off before delivery and mother's smoking habits were also found to have a high relation to socio-economical factors: These results demonstrate that the incidence of low birth weight is significantly higher in the “Buraku” area than in the control area, and that this phenomenon is brought about by poor health of the mother and socio-economical factors.

    To clarify the action of IgG antibody on basophils in intractable asthmatics, morphological changes, density and redistribution of IgG antibodies on basophils were observed by immunoscanning electron microscopy.
    Pear-shaped basophils appeared more frequently in severe asthmatics than in non-severe asthmatics when anti-IgG was added. The density of IgG antibodies bound to basophils was significantly higher in severe, but steroid-independent, asthmatics than in non-severe asthmatics and in steroid-dependent asthmatics (p<0.001). Cap and patch formations of IgG receptors on the cell were more frequent in severe asthmatics than in non-severe asthmatics.
    These findings suggest that the reactivity of basophils mediated by IgG antibodies is enhanced according to the severity of asthma, indicating that IgG antibodies act as anaphylactic antibodies in severe asthmatics, although IgG antibodies bound to the cell decrease during long-term steroid therapy.

    The action of IgG antibody on basophils was studied during hyposensitization therapy in bronchial asthmatics. IgE and IgG antibodies bound to the surface of basophils, and the redistribution of IgE receptors were observed by immuno-scanning electron microscopy.
    The ratio of IgG to IgE antibodies bound to basophils increased significantly during hyposensitization therapy (p<0.01). The occurrence of cap and patch formations of IgE receptors on the surface of basophils diminished during hyposensitization therapy. The effects on the passive sensitization of basophils with IgG4 myeloma protein were also studied in vitro. The results showed that the ratio of IgG4 to IgE antibodies on the cell increased, and binding of anti-IgE antibody significantly diminished (p<0.01). The percent of reactive basophils decreased when anti-IgE antiserum was added.
    These findings indicate that IgG4 antibodies affect the binding between IgE antibody and anti-IgE or antigens on the surface of basophils as a blocking antibody in hyposensitization therapy.

    Studies of 2216 24-hour Holter electrocardiograms of 1549 patients revealed bradyarrhy-thmias in 33 cases of advanced atrioventricular block, 169 cases of sick sinus symdrome and 32 cases of atrial fibrillation with slow ventricular response. The underlying diseases were mostly idiopathic, but included ischemic heart diseases and cardiomyopathy as well. The findings of Holter electrocardiography showed a significant difference between the bradyarrhythmia group and control group as to the minimum heart rate, maximum heart rate, the longest ventricular pauses and total number of QRS's during a 24-hour period.
    There were 55 cases of bradyarrhythmias with longest ventricular pauses of 3.0 seconds or longer. The incidence of atrial fibrillation with slow ventricular response was significantly different from other bradyarrhythmias. There were 64 cases of bradyarrhythmias with the total number of QRS's during a 24-hour period equaling 70, 000 beats/day or less. The incidence of sick sinus syndrome group I was significantly different from other bradyarrhythmias. As regards severity of bradyarrhythmias, these two findings were inconsistent.
    Comparative studies of paced and unpaced groups showed that the patients were older and had more subjective symptoms in the paced group. In all Holter findings, the total number of QRS's during a 24-hour period and the longest ventricular pauses were significantly different from other findings. No correlation was observed between the longest ventricular pauses and the maximum sinus node recovery time (or maximum automaticity recovery time).
    In consideration of subjective symptoms and underlying diseases, severity and pacemaker indication of bradyarrhythmias should be determined by longest ventricular pauses and total number of QRS's during a 24-hour period shown on 24-hour Holter recordings.

    One hundred twenty-four patients with unipolar permanent pacemaker implantation (advanced atrioventricular block, 62 cases; sick sinus syndrome, 51 cases; and atrial fibrillation with slow ventricular response, 11 cases) were studied by 24-hour Holter electrocardiography. Analysis of pacemaker function and arrhythmias with various pacing modes (VVI pacing, 70 cases; AAI pacing, 19 cases; and DDD pacing, 35 cases) and measurement of hemodynamics revealed the following.
    In all pacing modes, atrial contraction was observed in cases of advanced atrio-ventricular block. However, in the VVI pacing mode with sick sinus syndrome, atrial contraction was often lost. In sick sinus syndrome group III, pacemaker non-mediated arrhythmias were often recorded. Pacing failure was not noted with any atrial or ventricular pacing. Undersensing was observed in 26 % of the AAI pacing group and 7 % of the VVI pacing group. Myopotential inhibition was observed in 11-24 % of all pacing modes, but only two patients complained of subjective symptoms. DDD pacing with atrial sensing yielded effective increase of heart rate, but caused pacemaker tachycardia and pacemaker mediated tachycardia. In making these diagnosis and managing of pacemaker mediated arrhythmias, 24-hour Holter recordings were the most effective. In comparison of hemodynamics in any pacing mode, physiological pacing was significantly different from VVI pacing as to blood pressure, left atrial pressure and cardiac index. Patients who require improvement of hemodynamics should be treated with physiological pacing. As for the optimal choice of pacing modes, patients with advanced atrioventricular block without underlying disease do not always require physiological pacing. However, the patients with sick sinus syndrome should receive physiological pacing.

    The authors reported the results of a field survey of acatalasemia and hypocatalasemia conducted from 1949 through 1985. Six families and eleven cases of acatalasemia and twelve hypocatalasemia cases were added to the statistics reported in 1976. As a result, the total number of reported acatalasemias became 108 cases in 53 families as of the end of 1985.
