As is well known renal hemorrhage is evoked, other than by intrarenal lesions such as tumor, tuberculosis and stones, by extrarenal or general causative factors. With the present urological diagnostic method, as a rule, the source of the hemorrhage can be identified. However, in each individual case, the identification is not always possible. Such cases are designated as the so called essential hematuria.
In addition the genesis of this disease can not be unitarily proved by the histopathological examination of the extirpated kidney. Recently studies in this field are in progress and the nature of this disease is gradually been clarified. The allergy theory as the etiology and the focal infection theory which recommends the surgical treatments such as tonsilectomy and extraction of teeth greatly suggest the clue for the clarification of the nature.
The present author paid his attention to the relation between infection and other stresses and bodily responses, especially those in the autonomic nervous system and assumed that this disease was, at least partly, to be ascribed to irritation syndrome, Reilly, which took place in the kidney.
Therefore, hibernation artificielle was supplied clinically by administering chlorpromazine and promethazine as a “cocktail lytique” and good results were obtained. For a further experimental confirmation of this assumption the ganglion coeliacum and plexus renalis were stimulated and changes ranging between stasis and hemorrhage were evoked. To prevent the secondary changes due to exposition of the kidney and other operational procedures, the so-called jugular sac was made in the external jugular vein and the change in the kidney was followed-up on injection of various stimulants into it.
Furthermore, to study the renal histological changes which were evoked by the method stated above, the experiments of inhibition were made by means of the administration of “cocktail lytique” and the treatment to the renal hilus and the relation between the stimulation of the autonomic nerves and the renal circulation was studied by the infusion of Indian ink.
1. The rabbit's celiac ganglion and renal plexus were stimulated with toxin, croton oil, alcohol and others to evoke hematuria and renal changes ranging from stasis to hemorrhage.
2. Sac jugular method is most appropriate to prevent the secondary change in the kidney which might be evoked by exposition of the kidney and other operational procedures. Even by this method both microscopical and macroscopical hematuria can be evoked.
3. The volume of excreted urine was examined after the above mentioned sac jugular method, and the temporary stadium of oliguria was noticed. In spite of this stadium the total amount of intake was excreted within 4 hours.
4. As the renal change after nervous stimulation hemorrhage was slight in extent in the cortex especially in the glomeruli. The findings which reminded us interstitional nephritis were obtained most frequently.
5. When the nerves of the kidney were stimulated with staphylococcal toxin, a marked change in renal circulation, cortical ischemia, could be observed, though transiently.
6. In the kidneys on long-ranged observation and those which were stimulated repeatedly the main changes consisted in the interstitial hemorrhage and the hemorrhage, stasis and edema in the papillae. Cellular infiltration and necrosis were not observed.
7. The hemorrhagic change was more marked in the kidney on the side of which the renal hilus and ganglion coeliacum were treated with nicotin and carbolic acid solution than in the contralateral kidney.
8. The pictures of inhibition were obtained in the group of the rabbits which had previously been treated with the above stated “cocktail lytique”.
