    The dynamic property of glucose-induced insulin secretion was simulated with control theory, and the relationship between glucose concentration and insulin secretion was expressed in the transfer function of proportional plus derivative action to glucose concentration.
    By utilizing this system, the computer algorithm for the artificial beta cell was made.
    Microcomputer, insulin pump, and glucose analyzer were packed compactly in the size of 35cm W×25cm D×16cm H for bed-side use.
    The characteristics of the system recognized in clinical applications were; 1) Rate of insulin infusion was small enough to keep the plasma concentration of insulin physiological, then insulin requirements were reduced to around a half of those given subcutaneously, 2) Glucose or glucagon to restore hypoglycemia was not necessarily needed.

    Glucose sensor for implantable artificial pancreas has been developed in our laboratories. Glucose oxidase is immobilized on the surface of the Nylon membrane filter with glutaraldehyde. This type of sensor is much better than the sensor with GOD entarapping method, in the response range of the glucose concentration and in the length of the life.
    The exact relationship between sensor current and blood sugar level is recognized in animal experiment.

    Previous study has shown that the monitoring system using ultrafiltration method has improved on many points to shorten the response time and could achieved continuous In Viva Monitoring of Blood glucose level with bight accuracy.In this time, two Oxygen electrodes has taken up to measurement, and its output was obtained by difference of two electrodes. Flow-type electrodes were set in a line at the start and the finish point of GOD-enzyme reaction process.Linerity was obtained at least to 500mg/dl and sample volume could be in less than 20μl/min.

    Insulin therapy is now the only active and effective treatment for diabetic patients. Although it is difficult to regulate blood glucose into the narrow physiological range by conventional administration of drugs, ideal metabolic control should be developed.
    We are trying to synthetize an control system of blood glucose and to establish the artificial beta cell; a glucose sensor continuously measures the blood glucose level and this information feeds into a micro computer which energizes an infusion pump, and controlled amounts of insulin from a small resorvoir enter into the diabetic's circulation.
    In this study, the quadratic linearized model is employed as the mathematical model for the blood glucose regulatory system, and the modern control theory is applied. At present, this system has been developed as a bedsidee apparatus for the clinical treatment for hospitalized patients.

    Seven kinds of graftings for creation of hemodialysis blood access were used, and the comparison of graft materials and technics was made with their patency and utility. Graftings compared were as follow: 1) Autogenous vein graft, 2) Expanded-PTFE, 3) Modified bovine arterial graft, 4) Modified swine arterial graft, 5) Composite graft of bovine graft and expanded-PTFE, 6) Composite graft of swine graft and expanded-PTFE, and 7) Superficially replaced saphenous vein graft. Superficially replaced saphenous vein graft was the most satisfactory and remained patent for longer period of time. The advantages are; 1, ease of construction, only one anastomosis is necessary. 2. ease of canulating the fistula even in the obese patient. 3. allowing an immediate use of the fistula for hemodialysis after construction. We have found this method as the first procedure of choice in those patients who have no available site in the upper extremities for construction of a fistula.

    A new arterio graft was made by treating a swine carotid artery with chymotrypsin, proteiolytic enzyme, and by tanning it by glutalaldehyde. Chymotrypsin has no effects on elastines and a graft maintains its elasticity. Immunological assay and animal experiments confirmed the safety and function. The graft was clinically used in 102 dialysis patients at their limbs for blood access. The most cuccessful patient could use a graft for 24 months. The grafts of 40 of the 102 patients functioned or functioned with problems for more than 12 months. The remained cases were transplanted again. Clinical advantages of the graft are that it permits fast hemostasis and rarely develops aneurysm.

    We have tried to cotinuos monitoring of sodium ion, potassium ion concentrations and urea in the blood without sampling blood but connecting ultrafiltration cell to the patient's blood circulation. In this study measurment of chloride ion was added on this blood chemical monitoring system. Referance electrode of chloride ion was improved because of leakage of chloride ion from reference electrode. Basal problemes was examined by using these gel reference. And flow system of this blood chemical monitoring was improved, stability, the degree of accuracy and reproductivity were increased. There are size of electrode, shape of electrode and the methode to flow sample as improved points.

    The sensitive method for the quantitative determination of all guanidino compounds in human serum was developed by the adaptation of fluorometric detection and high-pressure liquid chromatographic method.
    Our new analytical method for guanidino compounds was successfully applied for the evaluation of normal and uremic subjects.
    The cerebrospinal fluid as well as serum levels of methylguanidine, guanidine and guanidinosuccinic acid was found to be significantly elevated in uremic subjects.
    Methylguanidine was dialyzed more easily through a cuprophane membrane than a acetate membrane.
    Methylguanidine showed a similar curve of the removal rate during dialysis with creatinine and had the removal rate of 50-60%.

    It is useful to measure noninvasively the blood flow of arteriovenous shunt in the hemodialysed patients. Conventional devices have not been available for this purpose. The admittance plethysmography newly developed by authors has been successfully applied to measurement of the limb blood flow in the patients with arteriovenous shunt for hemod is lys is. The blood flow per 100ml of measured limb volume is obtained noninvasively and repeatedly without any discomfort to the patient. It will contribute to taking care ofthe hemod.ialysed patients.

    Conventional single needle system which has a few disadvantages as follows.
    1) It is difficult to settle blood flow and ultrafiltrate pressure constantly.
    2) Intravascular pressure changed in a wide range of punctured sites.
    3) This system with special equipment regulating blood flow is expensive.
    We have developed a new strategy with only advantages of single needle system. The principle is to control blood flow, pressing on and off the housing of blood line by a plate.
    Result of our clinical experiment with the system was satisfactory.

    Most of dispors ab le dialysers have been sterilized by Formalin or Ethylene Oxide Gas and supplied to clinical use. The dialyser using them should be rinsed out before clinical use. The rinsing procedure by hand needs time.
    We have developed An Automatic Rinsing Machine for dialyser and estimated the rinsing effects. Data for formalin concentration in rinsing fluid at an outlet of dialyser was satisfactory.

    We have devised a bleeding detector to safely carry out hemodialysis. If the intravenous cannula comes off in the middle of hemodialysis, it will result in discharges of blood from the vein and flows of air into the body. This is very dangerous.
    If you set this detector on either side of the part where the cannula is stuck, the alarm will be given and the blood pump will stop soon after bleeding, so that danger will be gone. As we have found this device extremely effective as a result of its practical use, we dare to make it public.

    We have continued to measure the edematous volume and circular length of the limb by admittance plethysmography and with a scale, in order to judge the effectiveness of the treatment by “HADOMER”.
    The results of the mea surments are shown as follows;
    1) The conductivity of the deseased arms are high and after massage by “HADOMER”, it aproaches to the normal range. (Fig. 3)
    2) The ratio of swelling of the deseased arms decreased in length of time. (Fig. 4, 5)

    The Combinations of artificial valves and the driving methodes for the pulsatile blood pump have been studied employing mathematical model and mock circulation to increase the pump efficiency with smooth valve action.
    These model analysis and mock tests revealed that the pulsatile pump has the optimal driving condition, under that the pump can eject the maximal output with smooth movement of the valves, and that the optimal combination of valves is a tilting disc valve for the inlet, and a ball valve for the outlet.
    The optimal driving conditions of the pump for clinical use are as follows;
    pulse rate=80cpm,
    systolic time=0.35sec,
    supply air pressure=0.6Kg/cm2,
    vacuum pressure=-100mmHg.
    These findings enable the pump output to increase up to 6L/min without any complications.

    The effects of pulsatile and nonepulsatile perfusion on cardiac function after open heart surgery were studied in the clinical and experimental materials. Improvement of cardiac function was discussed by cardiac functional curve which was documented every 10 minuts after relicing of aortic cross clamping.
    Pulsatile perfusion effected a significant improvement of cardiac function, because of it's diastolic augmentation and unloading of preload. It's maximum effect appeared 20 to 30 minuts after oneset of partial bypass perfusion.
    Significant improvement of cardiac function was not recognized in nonepulsatile perfusion.

    Recently, the assisted extracoporeal circulation (AEC) is performed in clinics whereas survival rate of patients is unfortunately very low.
    Using 6 anesthetized dogs, AEC (mean flow, 50ml/kg/min) experiments were done during 12 23hr (mean 18hr) and 2 dogs survived more than 3 days after the weaning of the AEC and 4 dogs died within 2 or 3 hours later of the weaning. In both survival and dead groups, P and Q decreased, V and P increased, P and PaO decreased and PaCO increased just immediate after begining of AEC, and the survival group showed less changes of these measurements than the dead group. During PaO decrease and increase of PaCO and PaCO -P were continuously observed in both groups. Six hours later of the bigining of AEC, D increased about 83% of control and this phenomenon suspected pulmonary congestion.
    In survived 2 dogs, P changed gradually. Rapid and huge changes of P appeared in dead group. The meaning of this difference of P between survival and dead group is very important. Extreme changes of P may induce pulmonary damages.
    In order to use of AEC on pulmonary diseases patients, of P should be kept at constant value and we must aboid extreme changes of P as possible as we can.

    An echocardiographic evaluation of right and left ventricular function during synchronous and asynchronous left heart device pumping was carried out in 20 mongrel dogs to establish the long term monitoring system as a non-invasive method.
    The echo transducer was placed on the right ventricular epicardium directry or 1.5-2cm away from it, as the immersed method. The effects of LHB on heart rates, LVDd, LVDs, RVDd, RVDs and LVSV were measured and following results were obtained; 1) almost no changes of heart rates and RVD during sync. and async., 2) a significant decrease of LVDd and LVSV during both sinc. and async. with good correlation to the bypass ratio, 3) no signifcant changes of these determinants between async. and sync.
    The results of this study demonstrate the usefulness of echocardiographic monitoring in the evaluation of a left heart assist device as a noninvasive, repeatable and reliable method.

    Takagi's pump (Double-chambered alternate pumping paracorporeal device) has been tested in 50 dogs for last one year.
    Results obtained in left heart bypass with this device were as follows.
    1) Bypass flow rate was always maintained at the higher level than usual cardiac output, such as 170ml/kg/min up to 3 months. 2) In sever left heart failure induced by the ligation of almost all major branches from anterior descending, circumflex, and posterior descending coronary artery to the left ventricular wall, dogs survived up to 5 day with almost normal appearance.
    These results suggest that our device may guarantee the survival even in the sever left heart failure of almost no cardiac output.

    Experimental studies on left ventricular assist circulation (LVAC) using our pulsatile flow roller-type pump were carried out. During left atrio-ascending aortic bypass, the effect on hemodynamics was examined under different bypass flow from 30 to 60ml/kg/min.
    With the relatively low bypass flow of 40ml/kg/min or less, left ventricular and systemic pressure decreased about 50 per cent on the initiation of LVAC but recovered the control value within a few minutes as left ventricular pressure recovered. With bypass flow of 45 to 55ml/kg/min, remarkable diastolic augmentation was observed on central aortic pressure tracing. Left ventricular pressure decreased about 20 per cent and drop of central aortic pressure was not observed. With the bypass flow of 55 to 60ml/kg/min, cardiac output became zero and systolic segment of aortic pressure tracing disappeared and central aortic pressure was maintained well with only the blood pump action. But the decrement of left ventricular pressure remained 50 to 55 per cent of control value.

    Kaufman's urethral incontinence prosthesis has been implanted in 3 male patients. The prosthesis, made of silicon rubber, is to be applied to the anterior urethra transperitoneally and elevates the urethral resistance.
    Case 1; 9 year-old-boy. Stress incontinence due to the congenital malformation of external urethral sphincter. Improvement by implantation, 70%.
    Case 2; 27 year-old-male. Urinary incontinence and retention due to scar formation at posterior urethra followed by automobile accident. Transurethral resection of posterior urethra was intentionally performed to eliminate the state of retention and to make the state of incontinence. Improvement by prosthesis, 40%.
    Case 3; 42 year-old-male. Urinary incontinence due to traumatic urethral injury. Elevation of anterior urethra with abdominal rectus sheath resulted in failure previously. Improvement by prosthesis, 50%.
    No foreign body reaction has been observed so far. When the further compression effect is desired, the prosthesis can be inflated with contrast medium via a small needle. One cannot expect a complete continence with this type of prosthesis at least in those cases presenting a severe scar formation in posterior urethra.

    The purpose of this study is to elucidate the velocity of O2 and CO2 uptake and release of perfluorochemical (PFC) emulsion, which is a candidate for an artificial erythrocyte substitute because of its high O2 and CO2 solubility.
    The velocity was measured with a polarographic continuous flow method and aa spectropnotometric stopped flow method.
    These results were as follows:
    1. The O2 uptake and rtlease of 20% PFC emulsion finished within 26msec., whose rate was more rapid than that of human erytnrocyte.
    2. With a small quantity of carbonic anhydrase, the CO2 uptake and release of 5% PFC emulsion finished within 8, 5msec.
    These results suggest that PFC emulsion may be useful as artificial O2 and CO2 carrier in circulation, i. e. artificial erythrocyte substitute.

    Mixed venous oxygen tension was as well rerated to cardiac index as to arterial oxygen tension after open cardiac surgery. In an anesthetized dog, relationship between mixed venous Po2 and arterial Po2 at different levels of cardiac output with right chest open, or of artificial pump output under pulsatile total cardiopulmonary bypass with a closed perfusion system containing membrane oxygenator, was evaluated. Continuous monitoring of respiratory and blood gas tension with two mass spectrometers demonstrated that, when arterial oxygen tension was increased gradually, the higher was the level of mixed venous Po2 and the more pronounced was its rise, as cardiac output or artificial pump output was maintained at the higher levels. Calculated venous admixtures were lowest when inspired oxygen fractions were between 0.35 and 0.6.

    The activation rates of Factor X in the intrinsic coagulation pathway and by RVV were measured in the stationary condition and in a concentric cylinder device in a shear field. The activation rates in the intrinsic pathway decreased with the shear rate. On the other hand, the rate of activation by RVV did not vary in the shear field.

    A disposable humidifer (artificial nose) was developed for the purpose of keeping heat and moisture of inhalated gas, thereby preventing postoperative respiratory complications due to dried airway, Animal experiments and clinical results showed that the preservation of respiratory function was attained by using the disposable humidifier in terms of physiological shunt in the lung and alveolararterial Pot difTerence.In addition, the patients with DH could cough up easily imn.ediately after operation. Electromicroscopic study indicated better preservation of the mucoci l iary epithelium of the bronchus in the group with DH. It was concluded that DH was effective for preserving respiratory function during and after operation.

    Circumferencial defect 7 rings in length-were created in the cervical trachea of 16 mongrel dogs and replaced with tube of open heavy Marlex mesh covered with pericardial flap. “Sling suture” technique was used for alleviating tension at the anastomotic portion.
    There was one anesthetic death. Two dog died from complication of the procedure. One dog disappeared 53 days after operation.
    Six animals have been killed for study. All cases were followed bronchoscopically.
    Neither infection nor significant air leak was noted. Migration and dehiscence of the graft did not occur. Significant stenosis was found in three dogs at the anastomotic portion. In the majority of animals, this prosthesis has continued to function as a satisfactory graft for segmental defect in the cervical trachea for periods up to 6 months.
    We are working to make better prosthesis in terms of the lining of ciliated colummer epitherium and of the prevention of stenosis at the anastomotic portion.

    The roller type micropump driven by the pulse motor was developed as the insulin infusion pump for artificial cell. Flow rate of this pump is able to control by output signals of microcomputer. The volume displacement for one pulse was about 0.006μl. The pump output was increased in quite linear fashion from 0.003 to 0.1ml/min as increase the pulse number. The reproducibility was better than 2 % and pump flow rate was independent of load pressure.
    As the next version of this type of pump, smaller one was also developed. Dimensions and weight of this pump are almost same as those of an usual wrist watch, so that it can be put on the patient's hand.

    Non-protein N substances such as urea and ammonia are converted to amino acid and protein in the rumen of ruminant by virtue of microorganisms. Considering the possible application in hepatic and uremic patients, converting activity of micrococcus glutamicus of urea and ammonia to L-glutamate was examined. Fresh, lyophilized or sonicated micrococcus glutamicus reduced urea concentration and produced L-glutamate in the medium. Oral or ex vivo application of the treated. microorganism is suggested to be useful after sophistication of methodology in hepatic and uremic patients.

    High ammonia concentration of blood was seen ingiving L-asparaginase and L-arginine. So that the necessary equipment to eliminate ammonia was needed. On account of high rapid elimination of ammonia electro-dialyser was used. The menbranes of CMV and AMV as electrodialyser were used and electrified between them. Electro-dialyser to use ultrafiltration has high point of rimiting current density. So that the effects on electro-dialyser were quantitatively analysed in vivo experiments. With increasing electrical current, the rapidity and efficiency of blood ammonia removal was increased. With maintaing acid base equilibrium, electrolyte (Na, K and ci) and osmotic pressure, ammonia was eliminated rapidly.

    The effect of intra-aortic balloon pumping onthe coronary blood flow in hypothermia was studied in mongrel dogs.
    IABP increased the coronary blood flow by 7% with 300 msec of deflate time in 3790, while it increased by 48 to 50% with 300 msec of deflate time in hypothermia.
    These results were drived from a relative increase of the coronary blood flow in hypothermia.
    It was concluded in our experiment that IABP was more effective in hypothermia than in normothermia.

    It is quite possible that inflation of an intra-aortic balloon during intraaortic balloon pumping (IABP) results in a marked increase in electrical impedance in the aorta and, therefore, an increase in transthoracic electrical impedance. No remarkable change in electrical impedance of the thoracic organs and tissues excepting the aorta results from IABP, as compared with various types of assisted circulation with ventricular bypass technique. Therefore, the difference of transthoracie (total) electrical impedance, ΔZB, defined by the following equation, must be due to inflation of an intra-aortic balloon.

    Assuming that the aorta is compatible with a Windkessel model (time constant=, R/K), and that the aorta is oceluded. by an intra-aortie balloon at the time (α) after closure of the aortic valve, ΔZB is related to the balloon volume ( ΔV) by the following equations (Fig. 6.):
    Experiments were performed using Minnesota Impedance Cardiograph MDL-304A, AVCO IABP Model-7, animal balloons (10ml and 20ml), a human balloon (30ml), etc., in mongrel dogs.
    As a result, there was convincing evidence to support the conception that there existed a quantitative relation between ΔZB and ΔV, The actual volume of a balloon inflated during IABP was obtainable using calibration curves, and prompt detention of malfunctioning of the balloon was possible.
    It may be concluded that impedance cardiography is an useful method to monitor cardiac output, intrathoracic fluid volume, and the performance of an intra-aortic balloon continuously.

    Since 1974, IABP was applied to thirty patients with severe left heart failure following acute myocardial infarction or cardiac surgery. By reviewing of our experience, we studied the early hemodynamic response to IABP and prognosis. What have been obtained from our experiences are follows: 1. Even if systemic arterial pressure, left atrial pressure and endocardial viability ratio were maintained satisfactorily, it was the cardiac index to determine the prognosis. The prognosis is very severe if cardiac index is unable to exceed over 2.0 1/mira/M2 within 12-24 hours after IABP applied.
    2. Cardiac index in patients with cardiogenic shock following myocardial infarction was increased by IABP. Since several cases deteriorated after 24-48 hours, it is desirable to perform surgical treatment at latest within 24 hours.

    Sapporo IABP system was made. It is composed of electronic controller and pump unit (Cordimat IV) Triggering is with the R-Wave, filling interval depends upon deflation interval. Skip circuits and automatic deflation time circuits are added. The device was used in 9 patients with acute myoca rdial infarction and cardiogenic shock, 6 patients of them were improved generally and 3 were temporarily weaned from the IABP.

    Natural history of ventricular septal defect (VSD) due to acute myocardial infarction (AMI) is very poor. To decrease the mortality rate in those severe patients, we have attempted IABP in dogs associated with VSD or VSD c AMI which was produced by ligation of the left anterior descending coronary artery. And hemodynamic studies were done in detail.
    In this study all.dogs were divided into S groups such as group 1:QP/QS 1.0-2.5, group 2:QP/QS 2.5-4.5, group 3:QP/QS over 4.50. Hemodynamic changes due to IABP were evaluated from the degree of QP/QS above described. Consequently, the best result of effects of IABP was recognized in Group 2. No effect of IABP could be obtained in Group 3 indicated more than 4.5.

    To know the artificial heart(AH) pump function in-vitro is very important for the AH control after its implantation in animal. In general, the pump function is evaluated by the venous pressure sensitivity of pump output similarly to the Starling's law in natural heart and many mock circulatory system with changeable inflow water head has been developed. But in these system, no attention was paid for socalled venous return change and so, it is often experienced that the pump function tested by these system revealed different characteristics when it is implanted in animal.
    Then, it is thought by authors that the response of pumping out ability for the venous return change instead of the venous pressure change i s the most important for the evaluation of AH pump function. A new in-vitro evaluation method of AH pump function was deyelped which is made up of open reservoir tank, roller pump, collapsible tube and sac, AH pump to be tested, compliance tank and peripheral resistance. In this system, the venous return is simulated roller pump output, and collapsible tube and sac simulate vein and right atrium respectively. The pump function is evaluated by the response of pumping out ability for the roller pump output change and it well simulate the characteristics of the pump in-vivo implanted state.

    Highly reliable pneumatic artificial heart (AB) driving system with complete redundant system for failure free operation was developed and used in 13 left ventricular (LV) bypass experiments without any failure. It has the following features besides normal counterpulsation; (1) When the contraction of LV is detected during pump systole, AH is quickly switched to diastolic phase. In tachycardia, if systolic time of AH becomes too short to eject enough blood flow, the next ejection begins earlier and continues longer. (2) When the diastole of LV continues for longer than 1+ seconds, AH starts the pumping. (3) In fibrillation, AH is asynchronously pumped.

    According to our total artificial heart experiment, the long survival can be achieved by maintainig of cardiac output in normal rate, which is 80 to 100ml/kg.
    When an experimental animal is made exercise, its cardiac output should be increase. The amount of increased cardiac output is seemed to relate venous pressure, peripheral resistance or partial O2 pressure of blood. The relation between load of exercise and such physiological data, as CVP, TPR and O2 consumption, were analysed quantitatively in this report.
    The results were; 1) By CVP, the cardiac output of artificial goat during exercise could not be determined. 2) TPR has some relation to the cardiac output but we need more precise study. 3) O2 consumtion is seem to be most useful information to control total artificial heart.

    The differentiation of cardiac and vascular effects of cardiovascular drug is not an easy task. The total artificial heart implanted animal is a good model, in which vascular effect of drugs can be evaluated under the awake unanesthetized condition, since any drug will not affect the artificial heart directly. This paper shows the pure peripheral effects of isoproterenol, dopamine, glucagon and acetylcholine during total artificial heart pumping in the awake, unanesthetized post operative calves.

    Although the experimental results of the total artificial heart implantation have rapidly improved and several calves have been reported recently as surviving longer than 100days, the respiratory failure after the implantation surgery is still one of the major problem for the cause of death. If the pumping work of the right side heart is much stronger than that of the left side heart, respiratory failure will happen.
    Cardiac function curve of both side heart should be checked before the implantation by mock circulation. The relationship between right and left atrial pressure of the TAH implanted calves has been studied in this report.
