  • ―精管結紮術前後のアミノ酸量からみた睾丸および副性器の機能―

    The author carried out analysis of free amino acids, obtaining samples from fifty six infertile patients. Analysis of free amino acids in normal human semen were also conducted, the samples of which were obtained from five healthy males before and after they were vasectomized.
    1. The samples were divided into three groups in accordance with sperm counts in semen and compared with each other.
    The first group : sperm counts-over 40×106/cc
    The second group : sperm counts-below 40×106/cc
    The third group : azoospermia
    In regard to the total quantity of amino acids, the second (593.3 mg/dl) and the third (610.6 mg/dl) groups showed lower value than the first group (826.3 mg/dl) with significant difference. As to the quantity of each amino acid, there were significant differences among sixteen kinds of amino acids in the first and the second groups, excepting Cysteic acid and Methionine. In the third group, twelve kinds of amino acids showed significant difference compared with the first group, excepting Lysine, Histidine, Arginine, Serine, Isoleucine and Tyrosine.
    2. The samples were divided into three groups according to percentage of motile sperm in seminal fluid.
    The first group : percentage of motile sperm-over 60%
    The second group : percentage of motile sperm-below 60%
    The third group : azoospermia
    As to the total quantity of amino acids, the second (548.8 mg/dl) and the third (610.6 mg/dl) groups held lower value with significant difference than the the first group (822.7 mg/dl) . As to the quantity of each amino acid, significant difference was recognized in sixteen kinds of amino acids of the second group except in Taurine and Methionine, compared with the first group. The third group also showed low value with significant difference in thirteen kinds, excepting Lysine, Histidine, Arginine, Cysteic acid and Methionine.
    3. The samples were divided into five groups according to sperm counts and percentage of motile sperm.
    The first group : sperm counts-over 40×106/cc, motility-over 60%
    The second group : sperm counts-over 40×106/cc, motility-below 60%
    The third group : sperm counts-below 40×106/cc, motility-over 60%
    The fourth group : sperm counts-below 40×106/cc, motility-below 60%
    The fifth group : azoospermia
    As to the total quantity of amino acids, the last four groups (2 nd group : 512.1 mg/dl, 3 rd group : 646.9 mg/dl, 4 th group : 561.1 mg/dl, and 5 th group : 610.6 mg/dl) showed low value with significant difference compared with the first group (872.2 mg/dl) . As to the quantity of each amino acid, twelve kinds of amino acids of the second group showed lower value with significant difference than those of the first group, except Lysine, Histidine, Cysteic acid, Taurine, Proline and Methionine. Eight kinds of amino acids of the third group also showed lower value with significant difference than those of the first group, except Cysteic acid, Taurine, Threonine, Serine, Valine, Methionine, Isoleucine, Leucine, Tyrosine and Phenylalanine. In the fourth group, seventeen kinds except Methionine, and in the fifth group, fifteen kinds except Lysine, Histidine, Arginine showed low value with significant difference, compared with those of the first group.
    4. Amino acids in normal human semen obtained before and after vasectomy were examined. The quantity of total amino acids before vasectomy was 667.6 mg/dl on the average, while it was 594.2 mg/dl after vasectomy. As to each amino acid, Methionine showed 5.1 mg/dl before vasectomy and 1.6 mg/dl after vasectomy, but others showed various values.
    As the results, these analysis seem to suggest that not only the function of testicle but also the function of accessory sex organs are incomplete in the abnormal human semen as far as amino acids are concerned.
  • 動物血清中のMAO活性測定法の知見補遺

    Monoamine Oxidase (MAO) in serums of beef, cat, pig, dog, rabbit, rat and human were studied by the method of McEwen with a minor improvement and the following results were obtained.
    MAO activities in beef, cat, pig, dog, rabbit and human serums were found to increase lineally in proportion to increase of serum in reaction mixture. However, MAO in rat serum was not detected with this method. It was also found that the activity units (u) of MAO in serum of human, dog, rabbit, pig, cat and beef were 30, 213, 555, 585, 1110 and 7950 u (m1a mole benzaldehyde product/3 hr/0.6 ml serum), respectively. The optimum pH of rabbit serum MAO was found at pH 8.0. Serum MAO activity was inhibited completely with addition of IIH at the concentration of 1×10-6M and NaCN at the concentration of 1×10-3M. When rabbit serum was incubated at 50°C for 10 min and at 60°C for 10 min, MAO activities were decreased to 92% and 50% of the control value, respectively. The incubation of rabbit serum at 70°C for 10 min resulted in complete inactivation of MAO.

    Acid-base and energy metabolism associated with extracorporeal circulation and surface cooling ad perfusion rewarming for open heart surgery was discussed.
    Sixty five patients operated upon for correction of a serious cardic anomary were studied in order to evaluate changes that occured during, post cardiac bypass and post operation.
    Acid-base occured in under 3 years, 3-15 years cases showed respiratory alkalosis during to 90 minutes post cardiac bypass, post operation changed into slight metabolic alkalosis, over 15 years showed slight metabolic acidosis, post perfusion changed into metabolic alkalosis, in extracorporeal circulation. Deep hypothermia group showed metabolic acidosis during low temperatur but post operation changed into respiratory alkalosis and metabolic acidosis.
    Blood sugar increased by the cardiac bypass, however under 3 years, years 3-15 cases were greater over 15 years case, these changes recovered to normal with in 12 hours, in extracorporeal circuration. Deep hypothermia group increased out of surface cooling, and the high level lasted 5 hours after operation.
    Insulin level could not be secretively suppressied however over 15 years case were recognized more than other cases, in extracorporeal circulation. Deep hypothermia group showed monotoneous for 60 minutes post operatively.
    NEFA increased by the cardiac bypass, and which is reasons something to do with insulin secretion, in extracorpoleal circulation. Deep hypothermia group showed high level from surface cooling to perfusion rewarming.

    It is widely known that various electrocardiographic changes are observed in patients suffering from various cerebral diseases. However, there still remain many unknown facts about how and why such changes occur.
    In this study, the effect of electrical stimulation of the thalamus (nucl. medialis dorsalis : S group) and the hypothalamus (preoptic area : P group) of the dog brain on the cardiac function was investigated.
    The parameters of the cardiac funtion studied were blood pressure, ECG, coronary boot flow, cardiac output along with the index of contractility, Vmax and max dp/dt.
    A change in the ST·T segments on the ECG occurred in most cases just after the stimulation. The blood perssure, cardiac output and coronary blood flow appeared to change at about the same time. These changes were thought to be primary to stimulation for most cases. The appearance of the premature beats infrequently observed in both groups, esecially, in P group.
    The heart rate increased after stimulation in S group, while there was a tendency for the heart rate to decrease in P grotp. The degree of change was slight in both groups. The blood pressure, cardiac output, Vmax and max dp/dt increased upon stimulation in S group.
    However, with stimulation of P group, the parameters of these heart function tended to decrease. There seemed to be a close relation between Vmax and Vpm.
    Therefore, it was thought that Vpm is the index of choice for the cardiac contractility.

    Six patients with combined valvular disease are descrived. Although the presence of both lesiosn was evident by auscultation, the patients' symptoms and the radiographic and hemodynamic findings were primary those of mitral stenosis. One (case 2) was treated by closed commissurotomy, one (case 4) was closed commissurotomy and tricuspid annuloplasty, one (case 5) open commissurotomy, one (case 3) mitral valve replacement and two (case 1, 6) : had double valve replacement.
    One (case 1) died with low cardiac-output syndrome postoperative day and one (case 5) remained class II symptom, but 4 patients was striking symptomatic improvement which has been reflected in the results of postoperative hemodynamic evaluation. The importance of preoperative assessment of patients with combined valvular disease and the choice of the operative method employed in each cases are discussed.

    A 10-year-old boy was bited by a snake (natrix tigrina), raised as his pet. After 13 hours epistaxis and gingiva bleeding started, and in additon ecchimoses and hematuria were noted.
    Examination of blood plasma revealed fibrinogen 14 mg/dl and makedly prolonged prothorombin time.
    In 1 hour there was no clot retraction. Plasmin activity was very accerelated, but platelets were normal and counted 200000/cu mm. We gave transamin with satisfactory effect because it brock to fibrinogen degradation.
