    It has been widely known that subperiosteal implant or endosseous-subperiosteal implant is applied to the serious alveolar bone loss patients. Though those conventional implants made of metal alone assure bioaffinity as biomaterial, however, soft-tissue is generated at the interface with bone.
    The author and his colleagues have been conducting extensive R&D work for the past seven years on the possibility of using Round Blade made by plasma spraying of H.A.P. on titanium material. As a result of animal experiment, the interface was observed to be united with bone in terms of histochemistry by the use of H.A.P. coated Round Blade.
    Based on the above, subperiosteal implant and endosseous-subperiosteal implant both treated with H.A.P. plasma spraying were evaluated through clinical study, and the best progress has been observed in such case as only the parts where such endosseous subperiosteal implant were buried in bone was treated with H.A.P. plasma spraying.

    We performed scanning electron microscopic(SEM)observation of ceramic implant materials after cutting and polishing, and the diamond points after cutting. Also, we measured surface roughness of materials after cutting and polishing. Three ceramic implant materials(Zirconia, Alumina, Hydroxyapatite), core composite resin and 12% Au-Pd-Ag alloy were used in this study.
    In SEM observation of ceramic implant materials after cutting and polishing, each material showed characteristic surface textures and these surface textures after polishing did not recover before cutting.
    Surface roughness of each material before cutting was varied. But after cutting and polishing, the materials showed almost the same surface roughness. In SEM observation of the used diamond points,
    chips were not observed on the diamond points which cutted the 12% Au-Pd-Ag alloy, Zirconia, Alumina and Hydroxyapatite, although chips were observed on the diamond point which cutted the core composite resin.

    We examined the fundamental frequency of the percussion sound of I T I implants(K type and F type)and natural teeth beaten with the plastic bar. The fundamental frequency of I T I implants(max.1.17KHz, min.0.97 KHz)was almost 1 KHz as well as that of natural teeth(max.1.22 KHz, min.0.95KHz).

    Various kinds of dental implant treatment are in force as a result of defects and prosthesis. Under these conditions, I have come across a way called conscious-sedation. It has clear superiority over methods which make patients suffer surgical defects,long periods spent in dental chairs, and mental burden. Using intravenous sedation treatment when patients are anesthetized has good results in several cases. The patients don't have restraints on their respiratory system or interference with their circulation. They also don't feel nausea. It makes outpatient treatment safe and easy, especially for the patients who have difficult conditions. From this point of view it is a big help for implant operations.

    Dental implant operations have rapidly progressed these days through the good, results of fundamental research and the development of new materials for them.
    Clinically speaking, the demand for the operations has been increasing from the patients'side. But the patients, who are applied to the operations, are in most cases categorized into the middle-aged the advanced age by comparison. Not a few of them suffer from medical troubles. Local anaesthetic agents, medication, and doses of it are limited due to that and at times the operations as well, when hypnotic therapy, electric anaesthesia, acupuncture,and SSP therapy, which are possibly administered to self pain control system in a living body in oriental science, are regarded as safe and highly important treatments to take advantage of in clinic medicine.
    SSP anaesthesia, without local anesthetic agents, has resulted in the successful endosseus implant operation on the patient who suffers from kindney trouble.
    That is reported as follows.
    It is effective to postoperative and get along good result and also effective to the patients with another generalized disease.

    To evaluate the effect of the SL-type Bioceram screw implant on the masticatory function of lower complete denture wearers, six complete denture wearers were selected. The lower denture was anchored onto the implants which were inserted into the bilateral canine regions three months prior to the experiment. The subjects were instructed to chew a piece of gum, under the following three conditions, and jaw movement by means of MKG and surface EMGs from the bilateral masseter and posterior tempolaris muscles were recorded and analyzed. Mucosa support(stage 1), implant-mucosa-support(stage 2), and implant-mucosa-support with anesthetizing the surrounding structures of the implants(stage 3)were set up as experimental conditions.
    It might be pressumed that retention and support of the dentures in stage 2 were markedly improved by anchoring onto the implants, as evidenced by the most clearcut and rhythmical chewing motions kinesiographically and electromyographically.

    Even if homogenous or allagenic tooth transplantation is successfully carriedout, transplated teeth are resorpted in time after operation. The fact is well known that the transplanted teeth are replaced by new bone while the long time observation. Using this phenomenon, clinical trial of grafting using frozen tooth paticles was preformed in the three caces with periodontal bone destruction. After grafting the frozen tooth fragments into the periodontal and periapical bone defect, radiopacity in the grafted region was gradually increased from the 3 th month to the 6th month.

    At present when working with dental implant we diagnose the cases in a variety of ways:oral inspection, palpation, study model, dental radiography and panoral radiography. However each of these diagnoses is not definite, still being objective and total technique, so at present, dentists have to rely on their intuition.
    We have been trying to use X-ray CT with the ways mentioned above for several years in order to make diagnoses more reliable and objective.
    That is, using X-ray CT, we can find the exact measurement for the highness, width density of jaw bones, and the shapes, locations and distances of maxillary sinus, incisive foramen, anterior nasal spine, nasal cavity, zygomatic arch, maxillary tuberosity, inferior alveolar canal, mental foramen, and soon which are in need of implant.
    Moreover we could get a three dimensional analysis from the reconstruction of the portraits.
    The method of using X-ray CT with dental implant were first introduced in Japan by Matsuzawa,Nakazima and others at the 16th meeting of the Japan Dental Implant Society.
    We would like to report that our method has been effective although we need to conduct more examinations.

    We used often various attachment about the superstructure of Subperiosteal Implant. We had Reagel experienced hinge attachment, blockmatic attachment, mini presomachic attachment, IC attachment, soft attachment and so on. They have both merits and demerits, but they don't have special problems with all symptoms of examples till now.
    But they have an only problem which their periods are various. Example some long period is then years or some short period is two years.
    Therefore I think they are needed to more work.The thoughts of superstructure have flexible type, inflexible type, removable type, unremovable type,occlusal type. We used the fix type and the inflexible type before, but now we use usually the fix type and the inflexible type. At this time we'll report of using the regulex attachment of the fix type and the inflexible type with these two symptom of examples which used their type.

    The purpose of this study was to obtained basic information on the strength of the bone to retain the porous implant, which were inserted into the defective part of mature dog's mandible intramedullary.
    After sacrificing dogs periodically, new bone formation at the interface between bone and implant was investigated histo-pathologically by SEM and the contact force was calculated by push-out test.
    Results:
    Following the experimental study produced interesting results clinically.
    At the time of 4 weeks after operation new bone had been formed and ingrowth into the pore spaces.
    With the passage of time, the pore was filled with new bone. The contact force between bone and porous implant averaged 41.2kg 4 weeks,44.0kg 8 weeks,133.5kg 12 weeks and 123.9kg 16 weeks after operation.
    The highest interface strength value obtained was 188kg.
