    A 69-year-old housewife with chronic eczema was treated by Kampo-medicine. For about 20 years, she had suffered from episodes of general eruptions every summer. At our outpatient clinic, skin lesions, chiefly consisted of papules with erosions, were noticed on bilateral external surface of the nose, the back of right hand and the back of the neck.
    At first, decoctions of Keishibukuryo-gan-ka-Daio was prescribed for one month, and afterwards, Keishibukuryo-gan-ka-Daio Yokuinin for the rest. The lesions was completely improved after two or five months of the treatment, and the other skin lesions appeared on bilateral forearms, disappearing also after two or three months of the same design of the treatment. After the therapy by Kampo-medicine, no skin lesions were noticed at least for more than two years.
    The therapeutic mechanism of the skin lesions was briefly discussed from both the stand point of “Cell pathology” and that of “Whole body pathology”.

    It is well known that aconitine-alkaloids is effective for treatment of rheumatoid arthritis (RA). However, the clinical use is limited in part because of its side effects. In this study we examined the therapeutic effects of newly processed “bushi” (Shujibushi) in which the contents of original forms of aconitines were reduced to aboid their undesirable effects. The effectiveness of the “Shujibushi” was recognized in 65.8% (45/67) for clinical and definite RA and in 80.7% (21/26) for probable RA. There were not any adverse side effects.
    In order to obtain possible explanations for the high effectiveness of the “Shujibushi”, we analized ingredients of the “Shujibushi” in comparison with “Uzu” another type of aconitine-alkaloids medicine. The “Shujibushi” contained the significantly larger amounts of total alkaloids compared with “Uzu”. The satisfactory effectiveness of the “Shujibushi” might be brought on by the high content of total alkaloids which seem to be composed of aconitine-metabolites and its original forms.

    The changes of contents of dehydrocostuslactone (I) and costunolide (II) in the extract of Saussureae Radix were investigated during decoction. The contents of I and II in infused solution were reached to maximum after about 10min of the decoction, respectively. However, when the decoction was continued until the volume becomes to half, the contents of I and II in the solution decreased significantly compared to maximum content in the decoction. The content of II also decreased rapidly during the reflux in water. These results indicated that these small availabilities cause not only by volatilization but also by decomposition.
