ジャーナル フリー1970年晩秋, 県下F小学校に発生した急性腎炎流行の概況は次のようであつた. 月別の腎炎発生数は同年6月4名, 7月8名, 9月1名, 10月6名, 11月32名, 12月5名, 1月2名で累計58名であつた. 腎炎に先行して発熱, 咳, 咽頭痛などを主体とした上部気道疾患が認められた.学校の平常の欠席数は20名前後であつたが, 11月に入つて急激に増加し, 24日には94名に達し, 健康調査を行なつた11月27日の時点でも, 上部気道の異常を訴える児童を多数認めた.
そこで有症者を中心として児童176名について, 咽頭よりの溶連菌の分離, 酸加熱抽出抗原を用いた微量ゲル内沈降反応と毛管沈降反応による同定およびペア血清についての血中ASO価の測定を行なつた. その結果, 第1回目の保菌者検索でA群溶連菌保菌率が32.5%と著しく高く, しかもそのうち80%以上が12型菌の保菌者であつたことが判明した. 更に, これら12型菌保菌者群は保菌者群に比してASO価が一般に高く, しかも第1回目の菌検索で12型菌の保菌者でありながら, ASO価が166以下であつた者の殆んどが, 3週後に有意なASO価の上昇を示したことなども明らかにされた.
以上の事実は, この集団に発生した流行性腎炎が, 溶連菌A群12型による上部気道疾患にもとつくことを明らかに裏付けた.
微量ゲル内沈降反応による分離菌の同定結果は, 従来広く用いられている毛管沈降反応の成績と極めてよく一致したが, 未吸収型血清を用いて, A群とある型が同時に判定出来ること, 判定が容易且つ確実であること, 貴重な抗血清の必要量が, 毛管沈降反応よりも少なくてすむこと, どの利点があり, 特に本集団のようにA群12型といつたある特定の型を主として追及する場合には極めて好都合であつた.抄録全体を表示PDF形式でダウンロード (2155K) -

ジャーナル フリーPiromidic acid (PA), a pyridopyrimidine derivative developed in this country, is said to be highly effective to Gram-negative bacilli, showing no cross-resistance to other antibiotics except for Nalidixic acid.
The author applied this agent to 29 sporadic cases of infantile bacillary dysentery and 39 of dysenterylike germ-unidentifiable cases, and studied thereby the germ eradication and clinical effects.
Medication method is: 50-200mg/Kg in syrup, 100-150mg/Kg in powder, and 2-3g in capsules daily.
MIC measured with 25 Shigella strains turned out 3.12mcg/ml-25.0mcg/ml.
Therapeutical effects are summarized by the results obtained as follows:
1. The minimum effective dosis in syrup would be 100mg/Kg.
2. In capsules, 120mg/Kg or more yielded excellent results.
3. In children old enough for the administration of capsules, 2-3 g daily, divided 4 times, every 6 hours was proved excellent.
4. In such children as above, the combination of PA of 2g and KM (or KDM) of 1.0g was almost equivalent to PA 3.0g, but this combined use tends to delay the normalization of stool consistency.

ジャーナル フリーForty-three patients with temperature higher than 38°C due to acute infectious diseases were given indomethacin capsules or placebo to study the antipyretic effects of indomethacin.
The mean temperature reduction in patients treated with indomethacin was 1.95°C six hours after the drug administration. This antipyretic effect of indomethacin was statistically greater than that of those patients treated with placebo, who showed only 0.87°C temperature reduction in the same period.
Side effects observed during the study were confined only to the indomethacin group, and gastrointestinal upset, sweating, and hypotension were noticed in six patients.
The clinical symptoms of patients treated with indomethacin were ameliorated more rapidly than those treated with placebo.
The indications of combined therapy with antibiotics and anti-inflammatory drugs in acute infectious diseases were discussed.抄録全体を表示PDF形式でダウンロード (990K) -

ジャーナル フリーA case-report was presented as in incidence of bacteraemia due to S-typhi murium during the treatment of ulcerative colitis. The major symptoms consisted of thirst, vomiting, generalized fatigue and huskiness.
The diagnosis was made by bacteriological blood culture to find S-typhi murium. Generally, S-typhi murium may cause an acute gastroenteritis without any thphoid symptom in man, and in the past only several cases were reported in Japan.
