    The present study was designed to investigate the role of AMPS in the formation of collagen fibers in experimental hepatic fibrosis induced by CCl4 and the suppressive effect of crude papain on hepatic fibrosis. In the early stage of hepatic fibrosis the space of Disse and a portion of sinusoids were filled with ruthenium red-positive material (AMPS). Filaments, microfibrils, and immature collagen fibrils were observed around fibroblast-likecells in the space of Disse filled with AMPS. An increase of AMPS and collagen fibers in perisinusoidal spaces is slight in the rat liver treated with both CCl4 and crude papain in comparison to that treated with CCl4 alone. Besides, ruthenium red-positive granules, usually present attached to the mature collagen fibrils with regular intervals, were hardly observed in case of the simultaneous administration of CCl- and crude papain. Portions of those immature collagen fibrils were fragmented with disappearance of periodic bands. The maturation arrest of collagen fibrils due to the digestion of AMPS by crude papain is supposed to be one of the major factors for the suppression of hepatic fibrosis.
    The above findings will support the important role of AMPS in the formation of mature collagen fibers.

  • Energy Metabolism and Replication of Mitochondrial DNA

    In order to clarify the energy metabolism in regenerating rat liver the rate of mitochondrial respiration, the ATPase activity of inner membrane mitochondria and the level of adenine nucleotides were studied serially in the liver from partially (70%) hepatectomized and shamoperated Wistar rats. The replication of mitochondrial DNA was also investigated by the incorporation of thymidine and by the sedimentation behavior on sucrose gradients. The level of phosphorylation and respiratory control were higher and the level of ATP and energy charge were lower in regenerating liver than those in the control. No significant change was found on the activity of ATPase. It was observed that mitochondrial DNA existed predominantly as an open circular form (27S) in regenerating liver. From these results it was inferred that consumption of ATP for regeneration predominates over increased synthesis of ATP by mitochondria and further mitochondria themselves are destined to devide in regenerating liver cell.

    The proliferation of the so-called oval cells is quite interesting for elucidating azo-dyeinduced hepatocarcinogenesis.
    The purpoSe of this study is to investigate the fate of proliferated oval cells in the early stage of 3'-Me-DAB-induced hepatocarcinogenesis by light microscopic, electron microscopic and autoradiographic method.
    Ultrastructurally, the cells possessing characteristics intermediate between oval cells and small basophilic hepatocytes were frequently observed.
    Furthemore, it was concluded that some oval cells were able to transform into hepatocytes autoradiographically. Therefore, it was suggested that some oval cells were transformed into small basophilic hepatocytes and further differentiated into mature hepatocytes. Meanwhile, other oval cells take part in the formation of cholangiofibrosis.
    After all, oval cells play the leading part in the course of 3'-Me-DAB-induced hepatocarclnogenesis.

    Sulfated and nonsulfated bile acid in urine of 39 patients with cholestasis (26 cases with acute hepatitis and 13 cases with obstructive jaundice) were analyzed by gas chromatography.
    There was no correlation between daily excretion of urinary bile acid and the results of liver function tests. The percentage of sulfated bile acid in urine was inversely correlated to total serum bilirubin concentration and alkaline phosphatase. These results suggested that the sulfation of bile acid in the liver of patients with severe cholestasis might be deficient.
    Prognosis of patients with acute hepatitis, was not correlated with daily excretion of urinary bile acid. The percentage of urinary sulfated bile acid in 19 of 26 patients with acute hepatitis was above 50%, and transaminase test in this group showed normal after the treatment for 22-180 days. However, the percentage in 7 cases was below 50%, and 5 cases of these patients progressed coma and died within one month. Therefore, it was presumed that there might be high relationship between prognosis of patient with acute hepatitis and the percentage of urinary sulfated bile acid.

  • Phosphatase in Three Patients of Gastric Carcinoma with Liver Metastasis

    Three autopsied cases of the gastric carcinoma with liver metastasis were described. Abnormal levels of both carcinofetal proteins and carcinoembryonic isozymes of alkaline phosphatase (ALP) were found in their sera. All 3 patients had elevated α-fetoprotein (AFP) levels of 9, 100, 3, 025, 000 and 5, 525ng/ml in case 1, 2 and 3 respectively. Carcinoembryonic antigen (CEA) was also found in case 2 and 3, elevated to levels of 6, 000 and 5ng/ml, respectively.
    The properties of their isoenzymes were almost identical to variant ALP (Warnock) in case 1 and 2, while characteristics of isoenzyme in case 3 were almost the same as the Nagao isoenzyme except for electrophoretical slow moving.
    Microscopic examinations of both the primary and metastatic tumors disclosed moderately differentiated adenocarcinoma tubulare of gastric origin and their histological findings were similar in all 3 cases but no histological similarity of their metastatic carcinoma to hepatocellular carcinoma was observed.

    The patient was a 42-year-old woman, whose earliest symptom was generalized pruritus, and was admitted to the Keiyu Hospital in December in 1967. Liver function tests at that time reflect cholestasis with markedly elevated alkaline phosphatase (max. 115 K.A.U.) and cholesterol levels (max. 622mg per dl.), whereas serum albumin levels remained normal.
    Then she admitted to our hospital on the 4th of September in 1973, complaining of severe pruritus and jaundice. After admission, the pathologic fractures of the right 9th rib occurred. In endoscopic pancreatico-cholangiography, no obstruction in extrahepatic bile ducts was found. She died of acute adrenal failure on the 21st of March in 1974. In autopsy, the liver was 1, 440g and histological diagnosis was primary biliary cirrhosis.

    A case of primary hemangioendothelioma of the liver with histopathological features characteristic of an infantile hemangioendothelioma of the liver, in a 24-year-old male, was reported.
    The patient was found to have marked hepatomegaly and ascites over a prolonged period of time and succumbed with hematemesis and melena about 2 years after the onset of his illness or about 16 months after the development of ascites. Multiple tumor nodules of 0.5 to 1.5cm in diameter, reddish-purple in the center and greyish-white at the periphery, were found throughout the liver with pronounced cicatrization on limited area of the left lobe at autopsy.
    Hemangioendotheliomas of the liver in infants are congenital, and many of the patients develop symptoms ranging from 36 hours to 4-1/2 years of age. Although these tumors are histologically benign, most patients die within 6 months following the onset of their illness because of a rapid growth of the neoplasm. The findings noted in the case described herein would suggest that, in rare instances, hemangioendotheliomas of the liver in infants may grow very slowly or remain latent over a prolonged period of time.
