    Binding ability of ICG to the serum proteins was examined in 7 patients with hepatitis whose ICG test were much more impaired in comparison with BSP test (group H), 7 patients with familial type of abnormal ICG retention (group F), and 9 healthy adults (group N). The serum proteins which were bound with ICG were divided into 3 fractions by means of Sephadex G-200 column chromatography.
    The difference of binding ability of ICG to the serum proteins influence for the abnormal retention of ICG in all groups, particularly in the group H. A qualitative difference in binding of ICG to the serum proteins was noted between the both groups of H and F. The appearance of abnormal retention of ICG could be due to different physico-chemical mechanism in between two groups.

    The incidence of hepatoma developed on the ground of liver cirrhosis has been observed at high incidence frequency, particularly in those cases with history of viral hepatitis, and such association has been regarded important clinically. This time, we performed a prospective study of 59 cases with liver cirrhosis during the period from April 1973 to August 1975, and investigated the role of Hepatitis B Virus (HBV) on the development of hepatoma.
    The cases were divided into 3 groups i.e. HBs-Ag positive (16 cases), anti HBs positive (10caSes)and bothegative(33 cases). We followed up serum α-Fetoprotein periodically by RIA method. In 4 cases with positive HBs-Ag, we observed gradual increase of α-Fetoprotein, and tumor stains were demonstrated with celiac angiogram. The high rate of hepatoma in this group is significantly different from those of the other two groups. The tumors were solitary and multiple in each two cases and surgical resection could be done in each one of them. We affirmed two cases of small hepatoma of histologically Edmondson Group II, all were less than 2 cm in diameter and might be early hepatoma.
    In all of the four cases, the presence of HBs-Ag in serum and hepatocyte were found before the development of hepatoma. It implicates a close relationship between the incidence of hepatoma and HBV or HBs-Ag.

    A tendency of high serum complement level (CH50) in patients with primary cancer of the liver was confirmed, and its clinical significances were discussed.
    The serum complement levels in patients with hepatocellular carcinoma without cirrhosis were significantly high as compared with that of normal subjects, and mean serum complement level in patients with hepatocellular carcinoma complicated with cirrhosis was also increased about two fold compared with that in patients with cirrhosis.
    In some of the cases that serum complement levels were examined for a long period of time, the serum complement levels elevated in parallel with AFP since relatively early stage of its clinical course, but in general, a definite correlation between the two was not found.
    The serum complement levels were also high in the cases of hepatocellular carcinoma with low AFP production which was histologically well differentiated, and this seems to be a great help to establish early diagnosis for hepatocellular carcinoma.

    In view of the high rank of liver diseases (5th) in the cause of deaths among the employees of Japan National Railways, the prevalences of hepatitis B surface antigen (HBsAg) and anti-HBs were studied in 2, 985 subjects who had reached the ages of 40, 45 and 50 in 1973, together with major liver tests such as SGOT, SGPT, alkaline phosphatase and zinc turbidity test. HBsAg was determined by immune adherence hemagglutination test and anti-HBs by passive hemagglutination test. It was found that the antigen was positive in 1.9% of the examinees and the antibody in 16.8%. The frequency of abnormal liver tests among these subjects was low, and no differences were noted regarding liver tests between those positive for antigen, positive for antibody and negative for both. Although these data suggested lack of correlation between HBs antigen and liver function tests among these asymptomatic carriers, long follow-up studies are necessary before any conclusion is drawn as to the prognostic significance of the antigenemia without overt liver disease.

    The low caloric intake test was performed in a case with Dubin-Jonson syndrome who did not demonstrate the secondary rise in the BSP plasma clearance test. Following the fasting test the total plasma bilirubin increased by almost 100% (mainly direct type), and a a remarkable improvement of ICG clearance was noted. Interestingly, there occurred the secondary rise in BSP in plasma after fasting.
    The physiological alteration which causes the secondary BSP rise fbllowing the fasting seems to be dependent on the basic metabolic defect (characteristic in Dubin-Johnson syndrome) involving excretion of conjugated BSP into bile canaliculi.
    Furthermore, the demonstrated concomitant improvement in ICG uptake by hepatocytes may provide additional support to the concept that uptake and excretion of anionic dyes in hepatocyte are relatively independent steps on which certain selectivity is superimposed.

    A woman, aged 44, complained of abdominal pain. She was disquiet. She went into grade III deep hepatic coma in two days. Plasma exchange was applied; under total heparinization, 400ml of her blood was removed from her radial artery, of that blood cells were cleaned with 5% fructose solution, and patient's plasma was depleted and replaced with frozen plasma, after then the clear blood was returned to her cubital vein. These procedures were repeated to 4500ml under mild general anesthesia (GO). She recovered within one month.
    Mcrits of this procedure may be as follows:
    1. This can be performed anywhere at any time, if astaff is able to take plasma or frozen plasma.
    2. Patients receive only homologous plasma, and not a large amount of fresh blood.
    3. Doctors and nurses are in safety, because lavage of toxic blood is made in the pack, namely, it is not made by centrifuge, and patient's blood flows through the complete closed system.

    A 60-year-old woman had been known to have a portal hypertension and hepatosplenomegaly with no complaints for the past 3 years. She was admitted because of the sudden bleeding from esophageal varix. Esophageal transection and splenectomy were carried out under 4, 500ml of blood transfusion.
    Surgical biopsy of the liver revealed "chronic non-suppurative destructive cholangitis" Serum examination showed strong positive mitochondrial antibody, raised Al-Pase & IgM and negative HBsAg.
    After the operations she was referred to our department because of jaundice, anorexia and general fatigue, associating with remarkably elevated serum transaminase and positive HBsAg & anti-HBc. Needle biopsy of the liver showed superimposed acute viral hepatitis with marked cholestasis.
    Administration of azathioprine for 97 days demonstrated no significant biochemical, immunological and histological improvements.
