    Portal hypertension is related with not only liver cirrhosis, but also liver fibrosis. However, causalgenesis of liver fibrosis with portal hypertension is still obscure. Portal fibrosis, as terminal stage of various portal inflammation, might be caused by viral hepatitis, chronic cholangitis, toxic hepatic injury and other unkown etiology.
    An attempt was made to clarify the role of viral hepatitis, especially hepatitis B virus, to development of such portal fibrosis. Surgical edge biopsy materials, at the various kinds of surgical operation for portal hypertention, from 63 cases of liver fibrosis, 18 cases of precirrhosis and 66 cases of liver cirrhosis, were re-examined by orcein staining for HBsAg. HBsAg was found in the liver tissues of 9.5% in liver fibrosis, 11.1% in precirrhosis and 28.8% in cirrhosis, respectively. Accordingly, even in portal fibrosis with portal hypertension, hepatitis B virus take a role in part for their causalgenesis.

    Hepaplastintest (HPT), thrombotest (TT), prothrombin time (PT) and blood coagulation factors II, V, VII, VIII, IX and X were measured in patients with liver disease, in mice with MHV-2 hepatitis and in dogs with Eck fistula. The following results were obtained.
    1) The activities of the blood coagulation, except for factor VIII, showed a decrease in liver diseases as compared to healthy subjects, especially in fulminant hepatitis and in liver cirrhosis with coma.
    2) In liver diseases, there was a correlation between the decrease of the activities of blood coagulation, except for factor VIII, and the severity of the disease.
    3) The PIVKA-inhibitor showed a higher level in fulminant hepatitis than in hepatitis and in liver cirrhosis.
    4) In mice with MHV-2 hepatitis, the activities of the blood coagulation decreased with the progression of hepatic necrosis. This tendency was particularly noted in factor VIII.
    5) In Eck fistula dogs without meat intoxication, the changes of the blood coagulation factors were within the normal range. But in Eck fistula dogs with meat intoxication or with CCl4 hepatic damage, the activities of the blood coagulation, except for factor VIII, greatly decreased.
    Therefore, it is believed that the determination of blood coagulation tests in liver diseases is useful to determine the diagnosis and prognosis of the diseases, and that involvement of DIC was suggested in fulminant hepatitis from the aspect of PIVKA-inhibitor and factor VIII.

    An attempt is made to develop a method to measure the relative difference of the ultrasonic echo level between the normal and malignant tissues so to be able to contribute in the diagnosis of the malignant diseases of the liver.
    In 21 patients with malignant tumors of the liver, the sensitivity graded tomographic ultrasonic examination was done and the method revealed differences between the malignant tissues and surrounding normal tissues to be measurable ranging 9 to 15 dBs.
    Direct cytological aspiration of the echographically suspecious malignant area was done in 9 patients with the malignant diseases of the liver. In 7 cases out of 9, the aspiration biopsy was positive for cancer cells. Thus, the ultrasonic scanning method contribute in the diagnosis of the malignant diseases of the liver and especially the combined aspiration cytology is a practical approach to the liver tumor.

    Spontaneous hydrothorax complicating hepatic cirrhosis is not very infrequent, but the exact cause has not been fully elucidated except for the cases with overt perforation of the diaphragm. This is an account of a 43-year-old male with cirrhosis who developed rightsided pleural effusion of a moderate degree when he had a massive ascites.
    Postmortem examination disclosed several striated blackish nodules consisting of collagen fiber with surrounding infiltration of lymphocytes and macrophages; they represented silicosis of the pleural lymphatics. No defect or bleb was seen in the diaphragm. These findings strongly suggest that in this patient, the lymph drainage via the intercostal lymphatics had been impeded because of a block due to silicotic granulomas, resulting in lymph stasis and subsequently hydrothorax.

    A case of hepatocellular carcinoma, associated with an occulusion of inferior vena cava caused by fibrous membrane, was reported. Patient was 52 years old, female, with whom ascites as well as edema located in lower extremities were conspicious at the time of admission in Tonami Welfare Hospital.
    Celiac arteriography revealed numerous neovascularization and arteriovenous fistulae in the entire hepatic region. Of particular interest was that portal tracts appeared during the arterial phase of angiography. It was observed, however, that no obstruction in the vessels was present. These findings indicated an occulusion of either hepatic veins or inferior vena cava at the site of hepatic vein entrance.
    Other laboratory data showed an elevation of serum enzyme activities (alkaline phosphatase 29.5 K.A. units, LDH 753 units), retention of BSP, and positive HBsAg as well as α1-Fetoprotein.
    Despite the intensive treatment with anticancer drug and diuretics, patient died of hepatic failure after 64 days of admission.
    Autopsy findings revealed that the entire liver was occupied by moderately differentiated hepatocellular carcinoma. Other part of liver showed liver cirrhosis. In addition, complete occulusion of the inferior vena cava was observed just above the entrance of bilateral hepatic veins; the cause of the obstruction was fibrous membrane which might be originated in congenital malformation.
