    Poly (A+) rich messenger RNA were isolated from rat livers and translated in a cell-free synthesizing system derived from wheat germ S30 and the following results were obtained.
    By this translation assay, the ferritin mRNA on polysomes from iron loaded rats gave approximately twice as much incorporation into ferritin as the mRNA from controls. In addition, the peptides corresponding to H- and L-subunits of carrier ferritin were obtained by immunoprecipitation using anti-rat liver ferritin antiserum and SDS/polyacrylamide gel electrophoresis, suggesting that these two subunits were primary gene products of ferritin synthesis.
    Comparing with the results of in vivo experiment (1st. of our paper), the preferential induction of L-subunits were not so remarkable and it was suggested that iron facilitates the translation efficiency of L-subunit messenger RNA and the assambly of L-subunits rather than H-subunits into ferritin molecule.

    ADCC of peripheral blood lymphocytes from thirteen normal subjects was investigated in a assay system of 51Cr-labeled chicken red blood cells (CRBC) coated with rabbit antiCRBC serum. Results were as follows; 1) ADCC of unfractionated mononuclear cells was 66.1±9.5% (M±SD). 117±0.6% decrease of this activity was demonstrated following monocyte-depletion from the peripheral blood cells. 2) Maximal effect was obtained under condition of effector to target cell ratio of 25:1 and antiserum dilution of 10-3. 3) Investigation of time kinetics of ADCC revealed that target cell lysis progressed rapidly until 4 hours and reached to the maximal effect at 6 hours. This effect was perpetuated until 36 hours after the culture. 4) Fractionation of lymphocytes demonstrated that effector cells were non T-lymphocytes. 5) Pretreatment of lymphocytes with heat-aggregated IgG at concentration of 1-10mg/ml suppressed cytotoxicity completely. Slight suppression of the cytotoxicity was also found at concentration of 0.01mg/ml.

  • II. Relationship between ADCC activity of peripheral blood lymphocytes and mononuclear cell infiltration in the liver

    In order to clarify the participation of ADCC in the mechanism of liver cell injury, ADCC activity of peripheral blood lymphocytes from patients with acute viral hepatitis and chronic liver diseases was investigated and compared with mononuclear cell infiltration in liver tissues obtained by needle biopsy.
    ADCC activity was decreased significantly in initial stage of acute viral hepatitis, chronic hepatitis (active), chronic hepatitis (inactive), persistent hepatitis and primary biliary cirrhosis. This activity was normalized at the recovery stage of acute viral hepatitis.
    ADCC activity was compared with degree of momnuclear cell infiltration in portal tracts and hepatic lobules. The grade of mononuclear cell infiltration in hepatic lobules was in inverse proportion to ADCC activity. Therefore, it is suggested that K cell was mobilized into the liver and ADCC was participated in liver cell injury.
    In normal subjects, ADCC activity was suppressed almost completely with heataggregated IgG as a model of immune complex. On the other hand, because the suppression was incomplete in patients with chronic hepatitis (active), chronic hepatitis (inactive) and persistent hepatitis, it could not be denied that Fc-receptor of K cell was blocked by immune complex.

  • III. Relationship between ADCC activity of peripheral blood lymphocytes and circulating immune complex

    The presence of soluble immune complex (IC) in the sera from patients with chronic liver diseases was examined by Platelet aggregation method and immunofluorescence method using neutrophils. Furthermore, relationship between the presence of IC and ADCC activity of peripheral blood lymphocytes was investigated.
    The results of these two methods for IC were almost the same. The frequency of IC was high in patients with primary biliary cirrhosis (75.0%) and hepatic cirrhosis (46.7%). It was higher in HBsAg negative cases of chronic liver disease than in HBsAg positive one, though the difference was stastically not significant.
    Singificant correlation between the presence of IC and reductio nof ADCC activity of peripheral blood lymphocytes or inhibition of ADCC by heat-aggregated IgG as a model of IC could not be recognized. Blockage of Fc receptors of peripheral blood K cells could not be attributable to these phenomena. It is likely that number of peripheral blood K cells was decreased or the functions of these cells were abnormal.

    The ability of human serum to inactivate vesicular stomatitis virus (VSV) was studied using of 169 patients with liver diseases and 100 healthy adults. The results indicated that each geometric mean titer of VSV inactivation in four groups (acute hepatitis, liver cirrhosis, chronic hepatitis, asymptomatic HBsAg carrier) was significantly higher than that in three groups (primary biliary cirrhosis, alcoholic liver injury, drug-induced liver injury) and healthy adults group. Under these observations, the measurement for serum antiviral activity against VSV appears to be useful for diagnosis of viral liver diseases.

    The antiviral activity of human serum to inactivate vesicular stomatitis virus (VSV) was studied using sera of 13 patients with liver diseases, one with agammaglobulinemia, two with C3 deficiency, one baby cord blood and one healthy adult. The antiviral activity in each serum tested was not parallel with the concentration of IgG, IgM, IgA, C3c, C4 and the titer of complement hemolytic activity. Furthermore, antiviral activity was detected in sera treated with zymosan and at 56°C for 30min to inactivate complement. These results indicate that the activity of human serum to inhibit VSV infectivity is not associated with immunoglobulins and complement.

    An enzyme immunoassay was developed for the detection of the HBsAg using β-Dgalactosidase/antibody conjugate and silicon rods or glass beads as a solid phase. The lower level of the detection was approximately 1ng/ml for HBsAg, and the sensitivity was about the same in EIA and RIA. Three groups were studied for the detection of HBsAg. The results are as follows:
    1) Ninety two nurses were studied by EIA and RIA, and all nurses showed negative HBsAg in both methods.
    2) In 451 staffs in our university, 11 subjects (1.6%) were positive for HBsAg by EIA and 10 out of 11 were positive by RIA and 5 out of 11 by Au-plate. It seemed interesting that a case out of 11 positive cases by EIA was demonstrated negative by RIA, whose serum was inhibited over 50% by the confirmatory test using anti-HBs.

    A noticeable reduction in the disappearance of indocyanine green (ICG) from the plasma was observed in rats received a single intraperitoneal injection of colchicine at a concentration of 0.3mg per 100g body weight, 3hours prior to the application of dyes.
    On the other hand, disappearance of bromosulphophtalein (BSP) from the plasma remained unchanged even after colchicine treatment.
    Excretion of ICG into the bile was also inhibited by colchicine, while that of BSP remained normal. Cytoplasmic microtubules in hepatocytes were reduced significantly by colchicine treatment.
    These results seem to correspond to the findings observed in a case of constitutional ICG excretory defect in man, suggesting that hepatocyte microtubular system is engaged in the intracellular transport of ICG. Colchicine treated rats may be served as an animal model for constitutional ICG excretory defect, pathogenesis of which has not yet been clarified.

    2, 4, 6-Trihydroxy propiophenone (THPP) has been reported to induce a choleretic effect in the papillary region of biliary tract by the smooth muscle relaxing activity. However, its influence to the hepatocyte and biliary epithelium is poorly unknown. Therefore, bile flow was estimated in the aminal model having tube cannulation into the common bile duct. After intravenous THPP injection, the levels of the bile flow increased, and it was dose dependent. To clarify the bile productive effect of this drug, the concentration of bile acid, bicarbonate and chloride in bile was determined after infusion of THPP. Moreover, canalicular bile flow was measured using the biliary clearance of erythritol.
    Concequently, each compornent of bile did not change even THPP injection. On the other hand, the level of erythritol clearance elevated in comparison with control group. From the relationship between erythritol clearance and bile acid secretion, the increment of bile acid independent canalicular flow was estimated. Above evaluation cleared that THPP enhanced bile excretion from hepatocyte by means of the induction of the bile acid independent canalicular bile flow.

    Sixty autopsy liver of the aged persons (over 65 years of age) were studied histologically to elucidate the characteristic changes of the liver in the aged. The subjects sellected had no liver diseases or diseases might affect the liver. Among twenty-five parameters examined, fibrous boadering of the portal area with slight cellular infiltration were canspicious findings. Sclerosis of the hepatic artery was not marked and was less severe than that of renal artery. The volume of each hepatic lubule was decreased, with the corresponding decrease in the number of liver cells. Variation in the size of the cells and nuclei were marked along with a prominent increase in multinuclear cells. Fibrous thickning of liver capsule was commonly found.

    To clarify the epidemiologic features of occupationally acquired hepatitis B virus (HBV) infection among medical personnel, the authors carried out a seroepidemiologic survey of the 1642 employees in the Nihon University Hospital. The frequency of the HBs antigen in the personnel and blood donors was not differ significantly. However, the HBs antibody frequency was highest in surgeons (20.9%), obsterics-gynecologist (20.4%), ophthalmologist (22.2%), nurses (17.4%) and radiologic technicians (18.4%) in comparison to the controls (10.4%) and those in the practices of Internal medicine (7.5%) and pediatrics (5.9%). Thus, it would appeal. that exposure to blood and blood products rather than patient contact without such exposure might be the important risk factor for HBV infection. Although the prevalence of the HBs antibody in hospital personnel rose at a significantly higher rate with advancing age than did such prevalence among the comparison group, there was an accelerated phase of rising HBs antibody prevalence during the early years of clinical practice and training for hospital employees, after which the rate of increase appeared to plateau. Our results suggest that HBV infection constitutes a significant occupational risk for medical personnel.

    Seven patients with infectious mononucleosis admitted to National Shimonoseki Hospital during recent one year are reported. All cases had characteristic signs and symptoms of infectious mononucleosis as described by Evans, and were complicated by severe hepatitis. The diagnosis was comfirmed by the tests for various antibodies against Epstein-Barr virus. Five of seven patients had significantly high titer of IgM and or IgG antibodies to viral capsid antigen.
    Patients with EBV antibody positive infectious mononucleosis tended to have a high levels of serum IgM and patients with EBV antibody negative infectious mononucleosis tended to show high level of serum transaminase.
    In infectious mononucleosis hepatitis of all 4 cases studied, focal necrosis was observed as Well as portal and intralobular infiltlates of inflammatory cells, predominantly lymphocytes. The present study indicates that infectious mononucleosis is not so rare in Japan and sometimes complicates with severe hepatitis.

    Coagulation and fibrinolytic systems were investigated in acute hepatic failure of rabbits induced by the intravenous administration of D-galactosamine HCl (1 g per kg). Almost all rabbits died within 60 hour (mean±SD 34.2±15.1 hour) following a typical course of fulminant hepatic failure. The hemolytic activity of plasma complement (CH50) markedly decreaSed within 15 hour and endotoxemia thereafter appeared in 14 of 15 rabbits, Serial coagulation studies reflected the occurence of disseminated intravascular coagulation (DIC) prior to death in a high percentage. Coagulation factors and CH50 were valuable prognostic indicies in the course of fulminant hepatic failure. It is suggested that in acute hepatic failure endotoxemia and changes in complement systems are closely related to the development of DIC, which may finaly lead to lethal complications such as acute renal failure and gastrointestinal bleeding.

    During hepatocarcinogenesis of rats fed with 3'-Me-DAB, possible roles of ligandin and glutathione s-transferase (GST) were investigated. Ligandin and GST levels of liver cyto sol were found to decrease transiently until the 4th week of feeding with 3'-Me-DAB, and increased gradually after 8th week and was elevated markedly at the 10-12 weeks. In later stages, both ligandin and GSH levels gradually decreased until the liver carcinoma appeared. Electrofocusing column fractionation was employed for the qualitative analysis of GST from rat liver exposed to dietary 3'-Me-DAB. The GST obtained from normal rat livers were fractionated into three heterogeneous peaks, major peaks with pH's 7.6and a 9.2, minor peak with pH 8.3. On the other hand, GST from rat liver exposed to dietary 3'-Me-DAB showed unique increase on the peak with pH 8.3. A transient elevation of this peak was observed at 4-8 weekssafte r feeding the animals with 3'-Me-DAB. The second transient elevation occured after 12th week. The time of first elevation of this peakc orresponded to appearance of oval cells histologically and the time of second elevation corresponded to appearance of hyperplastic nodules. Therefore, the pH 8.3 peak may have a immportant role on chemically induced hepatocarcinogenesis.

    The specifc localization of radiolabeled antibody to AFP was investigated in rats bearing subcutaneous transplants of AH-7974 ascites hepatoma.
    Total body scintigrams 48 to 168 hours after injection of 125I-anti rat AFP horse antibody showed remarkable localization of radioactivity on the tumors, as a control, a faint outline of tumor was observed by 125I-normal horse IgG. The tumor/blood radioactivity ratio in the antibody group exceeded 1.0 and was approximately four times higher than that in the control group 7 days after injection, and suggested an active accumulation of radioactive antibody in the tumor tissue. In the subcellular distribution of radioactivity in the tumor tissue, about 30 to 60% of the total radioactivity were found in a fraction of the cell membrane plus nucleus. Furthermore autoradiograms of the fixed tissue sections in the antibody group revealed the specific localization of the radioactiVity on the tumor cell surface as black grains.

    Two cases of hepatic injury induced by cefalexin were reported. In case 1, 45 year old male, serum bilirubin, alkaline phosphatase, leucine amino peptidase, γ-glutamyl transpeptidase and Lp-X were elevated after taking cefalexin. Histological findings of the liver biopsy revealed several bile plugs and spotty necrosis of the liver cells in the center of the liver lobule. But reduction of the interlobular bile ducts in number and chronic nonsuppurative destructive cholangitis were not observed. Acute intrahepatic cholestasis as this case has rarely been reported. In the other case, 18 year old male, remarkable changes after taking cefalexin were recognized neither biochemically nor histologically except for slight elevation of transaminase. In these two cases, the drug-induced lymphocyte transformation was positive for cefalexin. It seemed that the same drug caused different clinical and histopathological pictures in individuals.

    A patient, 52 year old woman, with chronic intrahepatic cholestasis, presumably due to drug, was reported. She complained of jaundice with Stevens-Johnson syndrome followingerythromycin and sulpyrin therapy against common cold-like symptom in Ocotber, 1976. Jaundice continues for longer than 3.5 years.
    Wedge biopsy specimen of the liver taken 8 months after the onset of jaundice revealed bile plugs in the centrilobular areas and accumulation of bile pigments in the liver cells and Kupffer cells with feathery degeneration of hepatocytes. Portal tracts were slightly enlarged with fibrous tissue. The most characteristic findings of her liver specimen was reduction of the interlobular bile duct (less than 74μ in diamter) in number, as compared with corresponding hepatic arterial branch. However, chronic non-suppurative destructive cholangitis was not observed.

    Using Percutaneous Transhepatic Portography (PTP), spontaneous reversal of portal blood flow was shown in two patients with liver cirrhosis, in whom hepatocellular carcinoma and Budd-Chiari syndrome were excluded. The characteristic X-ray findings in these two cases were as follows. 1) Intrahepatic portal vein branches were not visualized after the injection of contrast medium at the porta hepatis, 2) Blood flow in the portal trunk was seen retrogradely, 3) Caliber of the portal trunk was narrower than normal, and 4) Huge collateral veins were recognized. In the pressure measurement, portal trunk had the highest pressure among portal vein tributaries. IRI level in the portal trunk was lower than in the peripheral vein. Blood NH3 level in the portal trunk was the lowest among the portal vein tributaties. Following injection of 131 I-MAA is the portal trunk, 88.1% of the colloid was trapped in the lungs. From these results, it was suggested that the direction of the portal blood flow waS reversed (hepatofugal) in these patients. The frequency of this phenomenon demonstrated in PTP was only 1.3%, 2 out of 150 cases of portal hypertension.

    A 44 year-old woman who had been followed up since 1975 under the diagnosis of primary biliary cirrhosis (PBC) with Sjogren's syndrome recognized colic pain at the left upper part of the abdomen and back radiating pain in February of 1979. The chemical analysis revealed high titers of serum and urinary amylase and serum Iipase. In addition, the amylase band of the pancreas was predominant in the serum amylase isozyme study. Complication of pancreatitis was most suspected, and the dietary limitation and administration of 100, 000 U to 200, 000 U of Aprotinin per day were done with no significant response to the symptoms and labolatory data. However, the following treatment with 30 mg of prednisolon per day showed marked improvememt of them. Although the complication of PBC with Sjogren's syndrome and pancreatitis is rare, the possibility of common etiologic factors among these three diseases in this patient is discussed.

    A 50 year old man was admitted to the Gifu Prefectural Gifu Hospital because of right hypochondralgia. Physical examination revealed hepatosplenomegaly and the hard enlarged cervical lymph nodes. Laboratory findings on admission was as follows: HBsAg negative by RIA and RPHA, anti-HBs negative by PHA, anti-HBc positive 212 by IAHA, AFP 320 ng/ml. A 99mTc-phytate liver scintigram showed large filling defects in the right and left lobe of the liver.
    The diagnosis of hepatoma was considered and intravenous administration of Adriamycin was started with a daily dose of 10mg/day. The patient clinically improved transiently, and then gradually exacerbated and died. Pathological diagnosis by the autopsy of the liver was hepatoma. After administration of 300mg ADM, HBsAg in serum was detected by RIA, and then the HBsAg titer increased to 213 by RPHA. It is suspected that immunosuppression by administration of ADM may permit an increase in viral replication sufficient for expression of HBsAg in the serum.

    The patient was a 58 year-old male admitted on Jan. 9, 1979 because of ascites, hepatomegaly and abdominal mass with hematemesis and tarry stool. Slight elevation of alpha-fetoprotein was found and celiac angiography demonstrated that tumor vessels corresponding to the mass in the right abdomen arise from the right hepatic artery and the left gastroepiploic artery. In April 18, the patient died of hematemesis resulting from rapture of esophageal varices. An autopsy showed liver cirrhosis with a large pedunculated hepatoma arising from the inferior surface of the right lobe of the liver.
