    Histological features of 112 liver biopsy specimens from 106 patients with alcoholic liver injury were studied.
    Alcoholic hyalins were found in 6 specimens (5.4%). Spherical giant mitochondrias were seen in 9 specimens (8.0%). Fatty metamorphosis, polymorphonuclear cell infiltration, pericellular fibrosis were observed in many specimens. Zonal or submassive necrosis were also observed in 20 specimens (17.9%). Cholestasis was seen in 32 specimens (28.6%).
    Histological changes of interlobular bile ducts were investigated in 103 specimens containing interlobular ducts. The changes observed in interlobular ducts revealed the inflammatory cell infiltration into the epithelium and the epithelial cell vacuolation in most specimens. Neutrophils in the lumen, swollen or flattened epithelial cells, karyopyknosis and eosinophilic degeneration were observed in considerable specimens. Among 31 specimens with cholestasis, the changes of interlobular bile ducts were detected in 19 specimens (61.3%), whereas the specimens without cholestasis showed the changes in only 4 of 72 (5.6%).
    These results suggest that cholestasis in alcoholic liver injury seems to related to the changes of interlobular bile ducts.

    Karyometry of hepatocytes was performed on 68 autopsy cases including cirrhosis with and without hepatoma and control, in order to relate the premalignant state of liver to the corresponding cellular changes. On thick liver sections, nuclei of hepatocytes or of hepatoma cells were subjected to measurement of nuclear diameter, in which the following parameters were employed; the mean D and deviation s of D, the numerical density of nuclei Nv and N/C ratio, tne eccentricity e2 of nuclear shape was also calculatied by simulating nuclear sections with dllipses. Hepatoma itself was characterized by elevated s, e2, Nv and N/C, of which the former three denote size unequality, transformation into an elongated shape, and elevated density of hepatoma cell nuclei, respectively. Comparison of hepatocytes between cirrhotic and control cases disclosed that in cirrhosis, nuclei were rather more uniform and less atypical with even lower N/C than in control, a change that was remote from the findings in hepatoma cells. This is not consistent with the morphology of premalignant cells in an ordinary sense. On the other hand comparison between cirrhotic groups with and without hepatoma revealed, a significant increase in Nv and N/C of hepatocytes in the group with hepatoma, suggesting that canceration was preceded by hepatocellular hyperplasia which produced mass of unusually small cells.

    To clarify the clinical significance of minute hepatocellular carcinoma, data on 12 patients with tumor less than 5cm in size seen in our clinic during the past 20 years were evaluated. All of these patients were male, and average age was 61 years old. Of these patients, 6 revealed positive HBs antigen or antibody in serum and 10 positive alpha-fetoprotein. Most of them exhibited a decreased functional reserve in the liver. Ten patients (38%) underwent hepatic resection, and 2 had conservative treatment. Operative mortality rate was 27%. Survival rates of these patients, excluding operative death, were 67% at one year, 33% at 3 years respectively. After treatment, 2 patients (40%) died of the recurrence of tumor, and 3 died of liver cirrhosis. Of overall patients, 64% had a capsule around tumor, and 92% associated with liver cirrhosis.
    From these findings, it is suggested that some of patients with minute hepatocellular carcinoma reveal a clinical feature of advanced tumor, and that the survival time of patients is fairly influenced by the prognosis of accompanied liver cirrhosis.

    We tried to prognosticate the course of liver diseases by using the multivariate analysis and 16 routin laboratory data.
    Two hundred forty seven patients with liver disease were classified into 4 groups on the basis of liver biopsy findings; acute hepatitis 78 cases, chronic hepatitis 40 cases, liver cirrhosis 74 cases and hepatoma 55 cases.
    The canonical discriminant analysis using 16 laboratory tests was examined on each group.
    The following conclusion may be established from the experiments:
    1. The results of this multivariate analysis showes the similar pattern to the clinical processes of acute hepatitis.
    2. This analysis is able to estimate the course of liver diseases and to discriminate between hepatoma and the other liver disease.

    The neurological status and activities in beagles with portcaval anastomosis (PCA) were graded from prolonged observation. Significant correlation between the grades and the valuse of blood chemistry was found and specific abnormal electroencephalograms (EEG) were appeared as the grades worsend.
    Twenty-six PCA beagles were fed a laboratory diet alone or together with intravenous infusion of an aromatic amino acid solution. Their neurological status and activities were observed daily and blood chemisry was determined weekly. EEGs were recorded twice or thrice a week. In 2 weeks or more after PCA, abnormal neurological signs such as salivation, lacrimation, and vomit were appeared and was followed by ataxic gait and abnormal visual sensation. In the terminal stage the beagles become comatose and died with marked emaciation.
    From these behavial observation, their neurological status and activities were graded into 5 stages-from Grade 0 (normal) to Grade IV (coma). Infusion of a specially formulated amino acid solution for hepatic failure to the PCA beagles with Grade II or higher, improved their grades by 1 to 2.
    These results suggest that the grading is suitable and useful for a means of the studies of hepatic encephalopathy.

    Two males with autoimmune hepatitis who were positive to HBs antibodies in blood were experienced.
    Case 1 was 47 y.o. male, affected by general fatigue and arthralgia since April, 1979. From the first examination, hepatomegaly was detected and HBs antibodies were positive, His laboratory data were of moderate levels. RA and LE cell phenomena were intermittently positive, while ANA was continuously positive. Histolgically, CAH was noted.
    Case 2 was 59 y.o. male, treated elsewhere with acute hepatitis in July 1980. His transeaminase level was elevated in January 1981. Since the initial examination, HBs antibodies were positive, with slight rise in his laboratoty data. RA and LE cell phenamena were megative, but ANA and ADNAA were strongly positive. Then ANA level was elevated into 2560 times, resulting in mild jaundice. Histologically, CAH (IIB) was noted. Predonine therapy was instituted, resulting in marked improvements clinically and histologically. HLA of both cases were examined.

    Two patients with compensated liver cirrhosis showing liver failure triggered by acute pneumonia and acute cholecystitis, respectively, were treated with plasma exchange therapy.
    One who manifested grade IV coma became alert after three exchanges and survived. The other who showed grade I coma and profound jaundice didn't recover from liver failure after as much as thirteen exchanges and died of renal failure.
    Although plasma exchange therapy is not generally considered to be indicated for hepatic coma in decompensated liver cirrhosis, it is a treatment for choice for hepatic coma in compensated liver cirrhosis which is induced by curable complications. Even in this case, however, contineous high bilirubin level in spite of the treatment seems to indicate unfavorable prognosis.

    A case of successfully resected minute hepatocellular carcinoma diagnosed by peritoneoscopy was reported.
    The patient was a 45-year-old male and an alcoholic. Serum hepatitis B surface antigen was negative and α-fetoprotein was less than 50ng/ml. Peritoneoscopically, a small encapsulated yellowish white tumor was seen at the margin of the right hepatic surface. On operation, the tumor was bound by a distinct fibrous capsule, measuring 2.5×2.5×2.5cm. Histologically, hepatitic liver cirrhosis.
    Peritoneoscopy is very useful for the diagnosis of a solitary hepatoma on the hepatic surface.

    An effective therapy for unresectable liver cancer has not yet established. Intermittent hepatic artery occlusion and intraarterial infusion chemotherapy is a method in which insertion and immobilization of a double lumen balloon catheter into artery supplying the tumor is accomplished, and ligation of the arterial collateral circulation to the liver is achieved at laparotomy, and by inflating the balloon postoperatively, occulusion of the hepatic artery is performed frequently for a predetermined time.
    The injection of carcinostatic agents is done by continuous administration by means of chronofuser connected to the catheter in combination with one shot injection at the time of blood flow occlusion.
    This paper describes a technique of intermittent hepatic artery occlusion and the course of one autopsied case which died 18 months postoperatively, with some considerations of the problems in the interruption of the hepatic artery for unresectable liver cancer.

    The item of this study was early diagnosis of liver tumor accompanying with thorotrastosis. For this study, we followed 5 patients over 2 years. Two out of 5 patients developed to cholangiocarcinoma and malignant hemangioendothelioma respectively, and one patient was discovered cavernous hemangioma in liver. The case with cholangiocarcinoma had partial hepatectomy and he is alive now. As the useful examinations for the early diagnosis of liver tumor accompanying with thorotrastosis, there were biochemical tests and imaging tests. Increased levels of γ-glutamyltranspeptidase, alkaline phosphatase and indocyanine green retention test are available. For the existent diagnosis, liver scintigraphy and abdominal computed tomography are available, and for the qualitive diagnosis, ultrasonographic examination and angiography are available.

    This is a report of one case of a 65 year-old female with primary leiomyosarcoma of the liver. The patient visited to our hospital because of right hypochondralgia and was found to have a hepatomegaly.
    Echo, angiography, enhanced CT, and US guided aspiration biopsy showed diagnosis of sarcoma of the liver. The resected tumor by extended right lobectomy of the liver was 3365g in weight and 22×19×10.5cm in size. The histological findings showed primary leiomyosarcoma of the liver.
    The patient was alive 12 months after operation.

    Two patients with Budd-Chiari syndrome complicated by hepatocellular carcinoma in whom CT and inferior venacavography were carried out are described. The imaging study with CT disclosed: 1) calcified inferior vena cava (IVC) at the level of the diaphragm and lack of enhancement of the IVC and hepatic veins after contrast enhancement, 2) markedly dilated inferior right hepatic vein, 3) marked compensatory enlargement of the caudate lobe, 4) icile-like thrombus extending caudally in the IVC and dilatation of that portion of the IVC, and 5) enlarged and dilated azygos, hemiazygos and anterior longitudinal venous trunk of the internal vertebral venous plexus, and with cavography, 1) the inferior right hepatic vein as a venous outlet to maintein the liver functions, and 2) occlusion of the right and left hepatic vein openings and a communicating vein between the left hepatic and caudate lobe vein.
