    To assess the immunopathogical role of immune response cells in liver deseases, the inflammatory infiltrate was examined by the method for the use of immunofluorescence on paraffin-embedded liver tlssues.
    Leu 2a+cells(suppressor/cytotoxic T cells)were significantly increased in periportal areas from patients with hepatitis. This was examplified by the increased numbers of Leu 2a+ cells in these areas with piecemeal necrosis(PMN)from patients with chronic aggressive hepatitis.
    On the contrary Leu 7+ cells(NK/K cells)in periportal areas were reduced in cases of chronic hepatitis but were increased in those of acute hepatitis.
    These findings indicate that suppressor/cytotoxic T cells are probable operators for chronic liver injury in the periportal liver parenchyma with PMN, and that accumulation of these cells in the periportal area may be primary mediator for the chronicity of viral hepatitis.

    The peritoneal exudate macrophages activated with lipopolysaccharide were shown to inhibit the protein biosynthesis in the isolated hepatocytes by a nonphagocytic mechanism that requires intimate cell contact. These macrophage-mediated hepatocytotoxicity were modulated by serum components. A high molecular weight fraction of senlm(Fr-1)was found to inhibit macrophage hepatocytocidal activity and a modulately low molecular weight fraction of serum(Fr-3)was shown to enhance it. The modulatory activity of Fr-1 component was not affected by treatments with RNase, DNase or neuraminidaae, but this activity was remarkably decreased by trypsin. These were also the cases in Fr-3 modulatory activity. However, heat-stability was somewhat different between active component of Fr-1 and Fr-3. These results suggest that these serum components were protein in nature.

    An autopsied sample of combined hepatocellular and cholangiocarcinoma tissue from the 64-year-old female was transplanted to nude mouse, and the transplatable tumor strain designated"KNOCH"tumor was established.
    In the patient serum AFP was 141ng/ml and CEA was 16.9ng/ml. The original hepatic tumor consisted of two different types of tissue; a combined feature of trabecular and glandular forms.
    A part of tissue was transplanted subcutaneously to nude mouse. Thereafter the tumor has been serially transplanted during 3 years.
    Light and electron microscopical findings on the KNOCH tumor were differenciated glandular cell arrangement, showing characteristics of cholangiocellular carcinoma.
    The high level of CEA was shown in the serum of tumor bearing mouse, but low level of AFP.
    Erythropoietin was detected in the tumor homogenates, but erythrocytosis was not observed in the tumor bearing mice.

    Hepatocelluar tumors and tumor-like lesions include hepatocellular carcinoma (HCC), hepatocellular adenoma (HCA), focal nodular hyperplasia (FNH), and nodular regenerative hyperplasia (NRH). Liver cell dysplasia (LCD) has been thought to be associated with HCC. For comparison and further characterization of these lesions, karyometric analysis using three parameters, which involve anisonucleosis, DNA content and irregularity index, was performed.
    HCCs showed a variable degree of nuclear atypism from minimal one similar to that of normal hepatocytes to remarkable one. Edmondson grade I of HCC, HCA and FNH had a minimal degree of nuclear atypism alike in the normal liver and they are difficult to be differentiated one another by nuclear atypism alone. Since NRH showed nuclear atypism as the same as that of regenerating hepatocytes of chronic active hepatitis, NRH may be lesions consisting of irregularly regenerating hepatocytes. LCD showed a striking anisonucleosis and an increase in DNA content similar to those in Edmondson grade III of HCC; however, the average of irregularity index was quite similar to that of the normal liver. The difference of nuclear atypism between these lesions did not lead a conclusion that LCD is putatively precancerous.

    Inhibitory effects of horse anti-rat alpha-fetoprotein (AFP) serum (antibody) on uptake of 2-deoxy-Dglucose (DOG) for AFP producing rat ascites hepatoma AH 66 cells were studied.
    AH 66 cells cultured in normal Eagle's MEM with 10% calf serum had a 1.5 fold high rate of sugar uptake than AH 66 cells cultured in antibody containing medium. The inhibition of DOG uptake by antibody was dependent on both time and titer of pre-exposured antibody. The rate of DOG uptake in AH 66 cells that were preincubated for 2 hours and 6 hours with antibody prior to the measurement of sugar uptake decreased 70.1% and 58.2%, respectively, lower than that of control cells.
    Antibody did not inhibit the phosphorylation of DOG by an intracellular hexokinase.
    Kinetic constants for the uptake of DOG in both AH 66 cells treated with antibody and control were calculated from Lineweaver-Burk plots. The Km remained constant at approximately 1.2mM, but the Vmax was 2 fold smaller for antibody treated cells than controled cells (571 and 923n moles12×105 cells/min. for AH 66 cells cultured in antibody containing medium and normal medium, respectively).
    Therefore the inhibition of DOG uptake by antibody was suggested the result of adecrease the number of transport site, carrier protein of the sugar, on the surface of the plasma membrane in AH 66 cells.

    The Purpose of this study was to evaluate the reversibility of the liver with obstructive jaundice on the ultrastructural changes of the liver.
    In obstructive jaundice, caused by ligation of the common bile duct with cholecystectomy in dogs, membrane surface of cristae per mitochondrion and surface density of endoplasmic reticulum were decreased, while volume density of lysosome was remarkably increased, and bile canaliculi were dilated accompanying loss of microvilli especially in the centrilobular area.
    When biliary decompression was performed within the first two weeks, the morphological changes of the liver showed good recovery, but poor thereafter. Changes of mitochondrial cristae were the most important indicator in evaluating reversibility of the liver.
    On the other hand, Kupffer's cells were remarkably enlarged with phagocytosis of bile pigment and occupied the sinusoidal space, not only following biliary obstruction, but also after biliary decompression. This condition could cause a circulatory disturbance in the sinusoid, and it might lead to a delayed recovery of the liver following biliary decompression.

    Intrahepatic collateral pathways of the liver with Idiopathic Portal Hypertension (IPH) were studied pathomorphologically.
    Increase of vascular channels in the large portal tract (cavernomatous transformation) was seen in 7 of 15 autopsy livers with IPH. These vascular channels anastomose each other and form plexus and mainly receive portal blood.
    Dilated vessels adjacent to the portal tract and in the hepatic lobule (so-called averrant vasculature) are part of the intrahepatic collateral circulation. Aberrant vasculature terminates into the sinusoids and plays a more important role in blood supply to the hepatic parenchyma than the hepatic artery.
    These vasculatures compensate for portal circulatory insufficiency due to obliteration of portal branches. It is predicted that the development of these collateral vessels is initiated by the obliteration of portal branches and is accelerated by portal hypertension or high portal inflow and/or regenerative mechanism toward partial parenchymal atrophy.

    A 33-year-old man was admitted with chief complaints of skin purpura family history and past history were nothing particular. On admission, RBC was 387×104/mm3, WBC 2700/mm3, plateletes 1.6×104/mm3 and nucleated cell counts (NCC) was 5600/mm3. He had also jaundice and hypertransaminasemia. HBsAg, antiHBs and antiIgMHA were all negative. On the basis of clinical course and laboratory data, the diagnosis of hepatitis associated with aplastic anemia was made. Since the usual treatments were not effective, plasma exchange therapy was performed 3 times. After this therapy, jaundice was decreased and blood cell counts were increased gradually. 60 days later, NCC was increased to 6000/mm3, 130 days Iater, 27500/mm3 and 180 days later, 37200/mm3. At present, 1 year and 4 months after the onset, his RBC was 430×104/mm3, WBC 6200/mm3, plateletes9×104/mm3. We emphasized the usefullness of plasma exchange therapy for this type of illness.

    A 33 year-old male alcoholic was admitted with symptoms such as fever, jaundice, anemia, hepatosplenomegaly and leucocytosis with neutrophils. General condition was rapidly aggravated inspite of cessation of alcohol abuse, taking bed reat, diet therapy and other suppportive treatment. Leucocyte count was increased in parallel with clinical deterioration to 85, 400 (maximun count) with no evidence of bacterial infection. On 36th hospital day, the patient died from multiorgan failure. Autopsy showed that liver weight was 3150g without appearance of liver cirrhosis. Histology of the liver revealed massive hepatocellular necrosis, fatty change, infiltration of neutrophils and extensive fibrosis. Mallory bodies were seen in many of the residual hepatocytes.

    A 22 year old female, ingested 60 tablets of Sedes A(R), equivalent 4.8g of acetaminophen, as a suicidal attempt, was admitted to our hospital under semicoma state. Her serum levels of GOT and GPT reached over 10000 IU on the 5th day, although serum bilinlbin level was moderately elevated to 2.2mg/dl. Prothrombin time was prolonged to 17.4 sec and hepaplastin test was 34%. Abdominal CT on the 17th day showed almost normal liver and liver biopsy specimen at the 24th day showed balloonig change in hepatocyte. She was treated by intravenous glucose containing 1-2g glutathione and serum levels of GOT and GPT became normal within 1 month.

    A case of hepatic artery aneurysm in a 18 year-old man is reported. He was admitted to our hospital for severe epigastric pain, No abnormalities were found on physical examination, laboratory data and various routine examinations of the upper abdomen. However ultrasound showed a tumor-like mass lesion with some transonic zones around the celiac trunk. Selective angiography then demonstrated an aneurysm arising from the common hepatic artery (1.2× 0.6cm in diamer). Aneurysmorrhaphy was successfully performed, and eight months after the procedure he became asymptomatic. Histologic findings of the tumor-like retroperitoneal lymphnodes revealed benign, non-specific lymphadenitis. Nontraumatic, non-infectious hepatic artery aneurysm in young age group is very rare. According to our survey, this is the 25th case reported in Japanese literrature. Ultrasonography is one of the most important tools in the diagnosis of hepatic artery aneurysm today.

    The 14-year-old girl presented with a 5 month history of amenorrhea. She developed jaundice and was hospitalized elsewhere. She was treated as acute hepatitis.
    The persistence of severe biochemical liver function tests led her to consult to the Kyoto University Hospital. Physical examination results revealed mild jaundice and anemia. Laboratory findings included hemoglobin of 7.9g/dl, reticulocyte count of 96‰, serum total bilirubin of 3.3mg/dl, senlm copper of 102μg/dl and serum concentration of heptoglobin of 18mg/dl. Coomb's test was negative. The diagnosis of Wilson's disease was confirmed by a low serum ceruloplasmin, high liver and urinary copper values and a presence of Kayser-Fleischer rings. Hypothalamic lesion was suspected according to LH-RH and clomiphene tests.
    After 7 months of D-penicillamine treatment, her hemoglobin returned to normal and her serum albumin concentration increased from 2.8 to 4.9g/dl. Reapperance of her menstruation occured after 7 months of treatment.

    Focal nodular hyperplasia (FNH) was found in a 31 year-old female patient with no history of taking oral contraceptives. The enucleation of the tumor was carried out in the suspicion of hepatocellular carcinoma on the basis of angiographic findings with several abnormal vessels and tumor staining in the right and left lobe of the liver. Histologically, enucleated tumors were composed of liver parenchyma intersected by fibrous septa radiating from the center of the tumor. These septa contained proliferated ducts and arteries with thickened walls. Electron microscopically, Mallory bodies (Yokoo's type II) were found in FNH. In this patient four years before the operation celiac angiography revealed the partial obstruction of portal and hepatic veins, but no tumor staining. One year later, LeVeen shunting operation was performed because of intractable ascites. These facts suggests that development of FNH was not related to oral contraceptives but to abnormal blood flow caused by portal and hepatic vein obstrulction in this patient.

    We treated fifty patients with hepatocellular carcinoma by transcatheter arterial embolization (TAE) for these three years. In four of them, superselective catheterization into proper hepatic artery could not be achieved by Seldinger method because of severe atherosclerosis or other reasons.
    We applied surgical procedures to the TAE therapy in order to avoid gastroduodenal or pancreatic artery embolization.
    In three elderly patients, we put angiographic catheter into gastroduodenal artery under surgical laparotomy. These canulated tubes were later used for repeated TAE therapy and for CT-arterigraphy. One of them received first TAE therapy during surgical operation. All patients responded well to this way of treatment.
    In the remainder case, embolization of left hepatic artery could not be performed by conventional method because the artery branched directly from celiac artery so complicatedly. He was successfully managed with TAE to the vessel which was carried out during surgical laparotomy, and at the same time spleno-left hepatic artery anastomosis was made with the intention of repeated TAE therapy by transcutaneous method thereafter.
    If TAE therapy was failed by Seldinger method in patient with inoperable hepatocellular carcinoma, surgical procedures should be applied to the treatment.

    Biliary cystadenocarcinoma is a rare tumor and the 30 odd case have been reproted so far. We have recently experienced the case of biliary cystadenocarcinoma that originated in the left lobe of the liver.
    The patient was a 65-year-old female who complained of anorexia and weight-loss and who was diagnosed as biliary cystadenocarcinoma by angiography, echo, C.T. and aspiration biopsy. She underwent abdominal surgery and then the metastatic lesion was found in the right lobe of the liver, so that she was inoperable. Since then, the patient's condition had gradually been deteriorating and died at last.
    At necropsy, the liver weighed 1500g and some cysts were found under its capsule. On cut surface, multiple cysts were observed, ranging from thumb to fist sized. The lumens of the cysts were filled with mucinous material the cysts were white and partially solid.
    Microscopically, they showed papillary cystadenocarcinoma. Some cysts were lined with malignant transformed ones. We considered that these findings revealed the malignant change of biliary cysts.
    We documented this comparatively rare case with literary studies.
