    We studied hepatitis B virus (HBV) DNA in liver and serum as well as serum pre-S2Ag in 17 patients with chronic hepatitis B (CHB), who were treated with interferon (IFN). Liver biopsy specimens were obtained immediately prior to the administration of IFN and within one week following the termination of IFN. HBV DNA in liver was studied by Southern blot hybridization. Before treatment, hepatic HBV DNA were positive with various replicative forms in all but 2 patients. After treatment, hepatic HBV DNA became negative in 4 patients and decreased in the remaining 8. In 2 of the 8 patients, only supercoiled form of HBV DNA remained. Six of these 17 patients were followed for up to 12 months after the end of treatment. In 3 of the 6 patients who remained all replicative forms of HBV DNA in liver after treatment, serum alanine aminotransferase (ALT) levels remained elevated. In one patient who remained only SC form of HBV DNA and 2 patients who lost all replicative forms of HBV DNA in liver, ALT levels normalized. In the latter 2 patients, however, ALT levels became elevated with positive HBV markers in serum. These findings suggest that loss of HBV DNA in liver result in ALT normalization and SC form of HBV DNA is the last form to disappear after IFN treatment. Pre-S2Ag is an useful marker in predicting the long-term prognosis of CHB.

    Chimpanzee experiments and clinical findings showed that patients became HBV carrier state very often after superimposed acute hepatitis type B upon preexisted chronic non-A, non-B (NANB) hepatitis. Immune abnormalities may coexist which alter their responsiveness to vaccination. This study compares the immune response to plasma-derived hepatitis B vaccine in the patients with NANB hepatitis and healthy controls. By the seven month after the initial vaccination, 42.9% in NANB hepatitis and 81.8% in controls had a response of HBs antibody. The titer of HBs antibody was significantly lower in NANB hepatitis than in controls. In non-responders with NANB hepatitis, significant lower lymphocytes count and higher OKT4/OKT8 ratio were observed as compared to responders.

    The suppressive effect of human interferon (IFN) products, commercially available in Japan, on the production of hepatitis B virus surface antigen (HBsAg) was investigated comparatively with PLC/PRF/5 human primary hepatocellular carcinoma cell line which proliferated in vitro releasing HBsAg and α-fetoprotein (AFP). Four kinds of human interferon products, recombinant IFN-α2a 18 million International Unit (IU), lymphoblastoid IFN-α 6 million IU, fibroblast IFN-β 3 million IU and recombinant IFN-α2b 10 million IU were tested.
    All IFN products clearly suppressed the production of HBsAg controlled by hepatitis B viral gene which persisted in PLC/PRF/5 cells, although the production of AFP controlled by cellular gene was affected little. The aspect of dose-response relationship between IFN concentration and HBsAg production per cell was quite similar among all of IFN products. However the potency for the suppression of HBsAg production by those IFN products was different to a certain extent.

    The serum levels of arylesterase (EC 3.1.1.2, ArE) were measured in patients with various liver diseases, by the method using a newly synthesized substrate (No. 4505) which is specific to the enzyme. While serum ArE levels in healthy subjects were 5.96±0.77U/ml (mean±SD), significant decreases in the serum levels of the enzyme were observed in patients with liver diseases: 4.25±0.45 in acute hepatitis; 4.65±0.90 in chronic active hepatitis; 4.34±0.99 in compensated liver cirrhosis; 3.50±1.02 in decompensated liver cirrhosis. In addition, the levels of serum ArE were well-correlated with those of serum cholinesterase and albumin.
    These results suggested that the measurement of serum ArE levels would be a good index in evaluating the degree of hepatic damage.

    Control of proliferation of human hepatocytes in primary culture was studied. Hepatocytes were isolated by a "two step" method (perfusion followed by shaking) from small specimens of human liver. The hepatocytes could be maintained in monolayer culture for three weeks in serum-free medium, and grew in response to insulin and epidermal growth factor (EGF). L-proline was essential for their DNA synthesis stimulated by these two hormones. Cis-hydroxyproline, a specific inhibitor of collagen synthesis, strongly inhibited DNA synthesis, suggesting that their growth required endogenous synthesis of collagen. In co-cultures of hepatocytes and non-parenchymal liver cells, addition of L-proline, insulin and EGF did not induce DNA synthesis in non-parenchymal liver cells and in fact rather suppressed their DNA synthesis, indicating a reciprocal relation between the growth of these two types of cells. These results should be useful in elucidating the mechanism of liver regeneration, and the pathogenesis of hepatic fibrosis.

    We report here the variation of hepatic glutathione S-transferase (GST) isozymes in normal livers and livers of patient with malignant tumor. The GST isozyme pattern was determined by chromatofocusing of 40 liver samples. Immunohistochemistry was performed using peroxidaseantiperoxidase technique for 20 liver samples with anti-cationic GST-C2 which crossreacted with all species of cationic GST.
    Following results were obtained. (1) In 11 normal livers, the occurrence of cationic C1, C2 and neutral N1 was high, whereas the presence of cationic A1 was not recognized. On the other hand, in 8 apparently normal livers with metastatic lesion from GI tract carcinoma, the occurrence of C2 and N1 was low, and the occurrence of A1 was highly recognized. (2) Although the heaptic GST isozyme A1 and GST of the placenta were eluted at anionic pI from the chromatofocusing, the pI and molecular weight of both were different each other. (3) Immunohistochemistry of the cationic GST revealed the negative GST staining in tumor cells of 3 hepatocellular carcinomas and 6 of 7 metastatic carcinomas, while the positiveness of GST in hepatocytes of such cases was various from low-grade to high-grade compared with that in hepatocytes of 3 normal livers, suggesting the interindividual difference in synthesis of the enzyme.

    To evaluate the association between serum γ-glutamyl transpeptidase (GGT) and liver regeneration and prognosis of acute hepatic failure (AHF), we examined the sequential changes of serum GGT in 53 patients with fulminant and subacute hepatitis (FH and SAH). The serum GGT levels on admission were significantly decreased in patients with acute and subacute type of FH and SAH, as compared with ordinary acute hepatitis (AHo) (102±108, 60±43 and 53±43IU/l vs 274±145IU/l, all p<0.001). Repeated determinations of serum GGT showed prominent increases in all 6 surviving cases with FH. On the other hand, very low levels of serum GGT persisted in all patients with fatal outcomes. 3 of 6 surviving patients in the high serum GGT group had simultaneously increased serum alkaline phosphatase levels, while there were no changes in other 3 patients. Furthermore, maximum elevation of serum GGT levels were observed in ultimate relation to increases of serum α-fetoprotein levels. Analysis of GGT isoenzyme by using polyacrylamide gradient gel electrophoresis and gel filtration showed that the high serum GGT in surviving patients with FH could be associated with the states of liver regeneration and intrahepatic cholestasis after hepatic massive necrosis.
    Experimentally, the activity of GGT in D-Galactosamine hepatitis rat liver increases in parallel to the increase of DNA synthesis of the liver.
    These results suggested that the serum GGT was a useful indicator for functioning liver mass, liver regeneration and prediction of prognosis in AHF and that GGT could be attributed to liver regeneration after massive necrosis of hepatocyte.

    We used the Doppler duplex system to investigate changes in the portal hemodynamics of healthy adults and patients with chronic liver disease (chronic hepatitis and liver cirrhosis) induced by infusion of dobutamine into the peripheral vein at rates of 2, 3 and 5μg/kg/min. The blood flow in portal, splenic, and superior mesenteric veins of both patients with chronic hepatitis and healthy adults significantly increased during infusion of dobutamine. The average change in blood flow in patients with liver cirrhosis did not however change statistically.
    The mean arterial pressure in healthy adults increased significantly during infusion of dobutamine. However, that in patients with liver cirrhosis did not change. The heart rate in patients with chronic liver disease increased significantly. However, that in healthy adults did not show statistical change.
    There are two possible explanations for the difference in the effect of dobutamine between these three groups: One is that many patients with liver cirrhosis have extrahepatic portosystemic shunts, thus the increase of portal venous inflow bypasses the liver into the systemic circulation through the portosystemic shunts; the other is that the hyperdynamic circulatory state in patients with chronic liver disease modifies the changes in portal circulatory system induced by dobutamine.

    We reported a rare case of acute hepatitis type A associated with various extrahepatic involvements including muscle, myocardium, pancreas and kidney.
    A 36-year old male was admitted with a high fever and general fatigue. Laboratory studies showed markedly elevated serum transaminase, lactic dehydrogenase, creatinine phosphokinase activity of muscular type, serum amylase, proteinuria and hematuria. Moreover, the finding of electrocardiography was compatible with myocarditis. Serum IgM anti HAV was positive. The elevation of circulatory immune complex and decrease of serum complement had continued for over four months since the admission.
    This suggested the possible role of immune complex in the pathogenesis of the above extrahepatic involvements.

    Three cases of severe acetaminophen poisoning are reported. Case 1 (27y.o., male) and Case 2 (36 y.o., female) had 6.4g and 3.2g of acetaminophen, respectively, and subsequently developed fulminant hepatic failure with grade IV coma. Disseminated intravascualr coagulopathy, gastrointestinal bleeding and acute renal failure also appeared in both cases, but they survived after intensive treatment including plasma exchange, hemodialysis and administration of gabexate mesilate and H2-receptor antagonist. In Case 3 (21y.o., female) who was admitted at our hospital one hour after intake of 14.4g acetaminophen, immediate gastric lavage and administration of high dose of N-acetyl-L-cystein were performed and she showed no sign of acute hepatic failure. Early preventive therapy as well as intensive cares are important in treatment of patients with severe acetaminophen poisoning.

    A case of cystadenoma of the liver is reported, stress being laid the usefullness of immunohistochemical investigations for CEA, CA19-9 and Vimentin to define the lesion. The histological examination revealed three distinct layers of the cyst wall: (1) an epithelial layer of mucin producing columnar or cuboidal cells lining the cyst lumen; (2) a layer of undifferentiated meenchymal cells beneath the epithelial lining; and (3) a dense layer of collagenous connective tissue just outside the stromal band of cells, which met the original criteria of Edmondson, et al. Eight cases of cystadenoma of the liver from the Japanese literature were reviewed, but only four cases including ours, all female, had features compatible with the original criteria. Immunohistochemically, CEA and CA19-9 were mainly demonstrated on the apical membrane of the epitherial cells, and Vimentin was positive in the densely cellular stroma. Although mucinous cystadenoma and/or cystadenocarcinoma of the ovary or endometrial cyst show histological resemblance to cystadenoma and/or cystadenocarcinoma of the liver, CA19-9 was negative among those extrahepatic lesions, and Vimentin was negative in the endometrial stroma.

  • Direct correlation between ultrasonography and histology

    An incidental hepatic pseudolipoma was found in a patient with liver cirrhosis at autopsy. Direct correlation between ultrasonography and histology was examined by using formalin-fixed autopsied liver.
    Histologically, the tumor was a degenerated, necrotic and fatty lesion encapsulated by a thick collagenous capsule. Scattered calcium deposits were found in the necrotic fatty tissue. On ultrasonography, the lesion was a hyperechoic nodule with lateral shadowing and some acoustic shadow.
    Hepatic pseudolipoma is one of the hyperechoic hepatic nodules, which can be diagnosed qualitatively by ultrasonography, because a good correlation between histological architecture and ultrasonographic image exits for this lesion.

    A 63 years old man visited a hospital because of general fatigue and weight loss, and was pointed out a marked eosinophilia (69% of WBC) and multiple nodular lesions in the liver on echogram. He was reffered to our University Hospital for further examinations.
    On admission, there were no abnormalities on physical examinations except for slight hepatomegaly. Laboratory data showed leukocytosis (12, 300/mm3) with 55% of eosinophils. Imaging studies including CT scan, echogram and angiogram showed multiple nodular lesions in the liver with about 1cm in diameter. The antibody to Toxocara cati was markedly positive immunoelectrophoretically. All these findings suggested the presence of viscreal larva migrans by Toxocara cati.
    By therapeutic trial with diethylcarbamazine (150mg/day) for 3 weeks, eosinophils were effectively reduced to 12% of WBC (6, 200/mm3) and hepatic nodular lesions disappeared, thus confirming the diagnosis.
    In the present study, we report a case of visceral larva migrans by Toxocara cati infection.
