    The state of HBV DNA in the liver of 39 HBV-related liver disease patients and the clinical features were investigated in order to find out when and how HBV DNA integration occurs.
    Of 34 HBV carriers (non-cancerous tissue); 3 had only free forms, 20 had both free and integrated forms, 10 had only integrated forms and 1 had neither free nor integrated form, all of the integration were randomly patterned. In only one case having hepatocellular carcinoma, clonal integration was observed.
    Integration was detected regardless of the histological activity. Integration was observed in 18 out of 20 HBeAg positive patients, and this indicates that it occurs prior to seroconversion. There was no relation between exacerbation and HBV integration. Most of the adult HBV carriers were observed to have had HBV DNA integration. These findings suggest that integration of HBV DNA occurs spontaneously to the hepatocyte of HBV carriers.

    In order to clarify the protective effect against different subtypes of HBV and long-term protection in family inhabitants of HBV carrier by administration of Hepatitis B vaccine derived from HBV carrier plasma. We investigated 76 family inhabitants of adr or ayw type HBV carrier. HB vaccine were administrated in 37 cases.
    The HB vaccine recipients acquired anti-HBs in 32 out of 37 (86%). After 5 years, the titers of anti-HBs were decreased in 29 cases (78%), disappeared in 6 cases (16%), who were not infected by adr or ayw HBV.
    In the non-HB vaccine recipients, anti-HBs became positive in 9 out of 39 (23%) and anti-HBc became positive in 2 out of 20 (10%). In these family inhabitants of adr HBV carrier, anti-HBs became positive in 21% and anti-HBc became positive in 0%. In family inhabitants of ayw HBV carrier, 17%, 20%, respectively.
    These data suggest that Hepatitis B vaccine is useful in preventing the infection of both adr and ayw HBV in family inhabitants of HBV carrier for a long time.

    Ninety-six patients admitted in the departments of gynecology and urology were enrolled in this study. They were devided into either group to be infused intravenous intact immunoglobulin (Venoglobulin-I, V-I) or not, as alternatively as possible. 9 (22.5%) out of 40 patients (V-I group) who received each vial containing 2.5g of V-I just before and after blood transfusions and 15 (26.8%) out of 56 patients who did not (control group) developed non-A, non-B post-transfusion hepatitis (PTH).
    In the patients transfused less than 1, 800ml, 5 patients (15.2%) in V-I group and 10 (23.3%) in control group developed PTH.
    There was no differrence of the incubation period, the highest level of GPT and the progression rates to chronic liver diseases in the patients with PTH in both group.
    No PTH type B developed.
    In conclusion no evidence of any clinical effectiveness of V-I infusion was found.

    Calcium antagonists nicardipine and nifedipine (10-4M) protected against the increases in medium LDH activity of rat cultured hepatocytes injured by calcium ionophore A23187 by 95%, but neither verapamil nor diltiazem showed such a suppressive effect. Calmodulin antagonist W-7 (10-6-10-5M) suppressed LDH by 40%. It, however, increased cell death conversely at 10-4M. C-kinase inhibitor H-7 decreased LDH activity by 30% DBcAMP and cAMP phosphodiesterase inhibitor did not affect the release of LDH significantly. Dihydropyridine Ca antagonists may show protective effect on hepatocyte after calcium ion influx into the cell. Calmodulin and C-kinase participate partly on the course of cell death.

    Both tumor necrosis factor (TNF) and interleukin 1 (IL 1) are synthesized by Kupffer cells and they possibly regulate Kupffer cell function. To investigate whether they have some effect on prostaglandin (PG) synthesis by Kupffer cells, we measured the levels of PGE2 in culture medium of Kupffer cells with various treatment. As a result, we found that TNF, not IL 1, promote PGE2 synthesis by mouse Kupffer cells.
    This result indicated the presence of interaction between cytokines and PGE2 around Kupffer cells.

    Effects of purified recombinant hepatitis B virus core antigen (HBcAg) on the reaction of peripheral blood mononuclear cells (PBMC) to interleukin-2 (IL-2) was investigated. PBMC preincubated with interleukin-2 (IL-2) and HBcAg for 72 hours showed a significantly decreased LAK cytotoxicity against Daudi cells. This inhibitory effect was dose-dependent. The proliferative reaction of PBMC after the culture with IL-2 and HBcAg was significantly diminished as compared with that of PBMC after a 72-hour-culture with IL-2 and without HBcAg. These results suggest that recombinant HBcAg may inhibit the IL-2-mediated induction of LAK activity by interference of HBcAg with the proliferative reaction of lymphocytes to IL-2.

    BrdU labeling indices of liver biopsy specimens from 19 control cirrhotic patients without HCC, 19 cirrhotic patients with HCC, and from 6 control subjects were examined using an in vitro labeling technique. The mean BrdU L.I±S.D. of HCC cancerous portion, HCC non-cancerous cirrhotic portion, control LC, and of control subjects were 7.2±2.9%, 3.3±1.5%, 2.1±1.7% and 0.25±0.09% respectively. Interesting enough, there was a significant difference (p<0.05) between the noncancerous LC portion of HCC patients and control LC. While all 17 non-cancerous portion belonged to the high L.I. group (≥1.5%), 10 of 19 in the control LC belonged to the low L.I. (<1.5) group (p<0.001). Of the 9 control LC with high L.I., 3 developed HCC within 2 years as compared to none in 10 control LC with low L.I. Conclusion: HCC would develop in the cirrhotics with high DNA synthetic potency.

    α-L-fucosidase, lysosomal enzyme which catabolizes fucoproteins, was studied in sera from 40 controls, 35 patients with hepatocellular carcinoma, 10 patients with metastatic liver carcinoma, 45 patients with cirrhosis and 30 patients with chronic hepatitis.
    Serum α-L-fucosidase (Fucosidase) was increased in hepatocellular carcinoma (201±58μmol/ml/hr) with a statistical significe versus controls, metastatic liver carcinoma, cirrhosis and chronic hepatitis.
    A level exceeding 150 has been considered as a useful marker for the diagnosis of hepatocellular carcinoma. Especially simultaneous assaies of Fucosidase and AFP seems to be a more useful marker for the diagnosis and management of hepatocellular carcinoma.

    Four human hepatoma cell lines (KIM-1, KYN-1, KYN-2, KMCH-1) established in our labolatory were studied about the secretion of C-reactive protein (CRP) and it's antigenic or structural differences from standard human plasma CRP. Two out of four hepatoma cell lines secrete CRP with no evidence of the existence of any inducers, such as IL-6, IL-1 and tumor necrosis factor (TNF). CRP is highly secreted at the late-logarythmic growth phase or the stationary phase in the growth cycle of KIM-1 cells. No obvious differences were detected between CRP secreted from these hepatoma cells and that of standard human plasma about antigenicity and molecular weight.
    This is the first report describing the secretion of CRP without cytokines from the cultured hepatoma cells. Further study should be necessary to know the mechanisms of the CRP secretion.

    Three hepatoma cell and Chang liver cell lines were examined for interleukin 1 (IL-1) production. IL-1 activity was determined using thymocyte co-stimulating assay. In the result, three hepatoma cell lines showed high level IL-1 in their conditioned media. In contrast, Chang liver cell showed no significant IL-1 activity. These activities were completely neutralized with anti-IL-1α antiserum. Furthermore, in examinaiton of mRNA level by dot blot hybridization, two of three hepatoma cell lines showed high expression of IL-1α mRNA.
    These findings suggest the possibility of IL-1α production by hepatoma cells. Further study will be needed to clarify the role and significance of IL-1 in the patients with hepatocellular carcinoma.

    In order to reveal the mechanism of postoperative recurrence of liver cancers in humans, we attempted to analyze the mechanism of the enhanced growth of liver cancers following partial hepatectomy (PH), from the standpoints of changes in host postoperative immunity, expression of cytosol estrogen receptors (E2R) in liver cancer cells and liver regeneration factor, using WKA/H rats and the hepatocellular carcinoma KDH-8 cell line induced by 3'-Me-DAB.
    The changes in postoperative immunity (NK activity and Blastogenesis) did not correlated with changes in liver cancer growth.
    Although serum estradiol (E2) increased significantly after PH, E2R was not detected in the KDH-8 cells used in this experiment.
    Serum was obtained from healthy rats 24 hours after PH, and 20mg of serum, as calculated from total protein, was eluted into 50 fractions by high performance liquid chromatography (column; TSK G3000SW). When we investigated which fractions stimulated the growth of both primarily cultivated hepatocytes and KDH-8 cells, only the fraction Fr. 30, the molecular weight of which was over 100Kd, enhanced both.
    Furthermore, we performed an in vivo assay to determine the number of days needed for tumor appearance: PHs were done 2 months, 5 days and 1 day before, at the same day of, and 1 day and 5 days after KDH-8 cell (500 cells/100μl) inoculation (sc). We also observed from in vivo tests that liver regeneration factor enhanced the growth of liver cancers.

    In 24 cases of hepatocellular carcinoma with the trunk and/or the first order branch of the portal vein obstructed by tumor thrombi, we performed arterial infusion of SMANCS-Lipiodol.
    In this series, the size of the main tumor was decreased in 7 cases. The 6-month survival rate was 33.4% and the 1-year 15.0%.
    In the cases in which the pre-therapeutic value of total bilirubin in the serum was more than 3.0mg/dl, the prognosis was poor.
    A few cases administered more than 6ml of SMANCS-Lipiodol at a time were dead within 3 months. From the above-mentioned results, an adequate dose of SMANCS-Lipiodol infused into the hepatic artery was considered to be about 4ml.

    Two adult cases of cytomegalovirus (CMV) hepatitis with positive immunoglobulin M antibodies against human CMV were reported. case 1 was a 28-years-old man who complained of fever and general fatigue. Case 2 was a 22-years-old woman who complained of fever, headache and general fatigue. In two cases, serum transaminase was elevated moderately, while serum total bilirubin was not elevated. Serum viral markers associated with type A and type B hepatitis were all negative. Histological findings showed hepatocyte necrosis and lymphocyte infiltration in some places slightly without evidence of inclusion bodies in hepatocytes.
    Characteristic features of two cases were long-term fever and anicteric hepatitis.

    A rare case of hepatic epithelioid hemangioendothelioma was reported. A 58 year old female visited a doctor for anorexia and weight loss in January, 1985. Laboratory data revealed only elevated serum alkaline phosphatase, elevated serum LAP and elevated γ-GTP. Tumor markers were all negative except for ferritin. Numerous calcified lesions of the liver were pointed out on the plain film of the abdomen.
    Then needle biopsy of the liver was done. But the precise diagnosis was not obtained and only calcified lesions were shown in the obtained specimen. Calcifications in the liver gradually increased. She admitted the Wakayama Red Cross Hospital for severe general malaise and dry coughs in July, 1987. She died of respiratory failure 3 days after admission and 2.5 years after the onset. Autopsy was done. Multiple calcified lesions of epithelioid hemangioendothelioma were seen diffusely in the liver, diffusely in the lung, and in the abdominal lymph modes. Calcifications were seen in the centers of the tumors. Carcinogens to her illness were not supposed.

    A 73 year-old man was admitted in July, 1983. He had been followed up for abdominal distension and found to have a habit of eating raw fish. Examination revealed ova of Clonorchis sinensis in the stool, eosinophilia (25%), ALP 497U/L. Computed Tomography of the liver showed several small low density areas. Laparoscopy showed micronodular cirrhotic liver with sporadic dark blue flecks, the locally dilated bile ducts on S2, S5 and a small whitish nodule on S4. The biopsied tissue of the nodule showed adenomatous hyperplasia of the bile duct. The biopsied liver tissue showed secondary biliary cirrhosis with stromal eosinophilic infiltration.
    ERCP disclosed cystic dilatation of the peripheral intrahepatic bile ducts and excretion of many flukes.
    The relationship between Clonorchis sinensis infestation and secondary biliary cirrhosis was suggested.

    A forty-eight-year old Japanese woman with mixed connective tissue disease (MCTD) who developed primary biliary cirrhosis (PBC) during the course is reported. She complained of fever, polyarthralgia and Raynaud's phenomenon thirteen years ago and was admitted to the Kyushu University Hospital. Anti-RNP antibody was positive while anti-Sm antibody was negative. She was diagnosed as MCTD and had been taking prednisolone. Two months before the second admission serum alkaline phosphatase level has gradually increased and the patient was readmitted to our hospital.
    Anti-mitochondrial antibody was positive. Biopsy of the liver disclosed non-supprative destructive cholangitis and the patients was diagnosed as stage I of PBC. Coincidence of the both diseases is very rare but it is interesting that PBC occurred during the course of MCTD. Some common immunological disorder may be attributed to the occurrence of the diseases.
