    To analyze the function of the pre-C region of DHBV DNA, we introduced a frameshift mutation into the Acc I site in this region, and transfected this mutant DNA into the liver of 1-day-old ducklings. The ducklings transfected with mutant DNA developed DHBV infection, and this mutant virus was also infectious to susceptible ducklings. The direct sequencing confirmed the presence of pre-C mutation in the progeny genomes. By Western blotting, no DHBeAg was detectable in serum of ducks infected with pre-C mutant virus. The histological examination showed more active histological changes in wild-type than in pre-C mutant. These results suggest the pre-C region is not essential for viral replication and infection, but required for the systhesis and secretion of DHBeAg, and it has become possible to study the pathogenicity of the DHBeAg producing/nonproducing virus in liver.

    Ten patients with stable decompensated liver cirrhosis were studied. They received equal amounts of protein and calories for the entire study period, and the composition of amino acid intake changed stepwise three times every two weeks.
    The results obtained by the present study were as follows:
    1) Plasma amino acid concentration, i.e. the average of the daily fluctuation, changed in proportion to the amount of amino acid intake.
    2) The ratio of BCAA/AAA of plasma amino acid increased as that of amino acid intake increased. Serum albumin, prealbumin and retinol binding protein were unchanged. Plasma ammonia level fell, although the nitrogen balance deteriorated only slightly. It should be possible to normalize the plasma amino acid profile while retaining nitrogen if half the amount of the amino acids required (total 10.2 mmoles/kg/day) is administered in a form of liver diet and the remaining half to be given in a BCAA-rich which lacks AAA, Met crystalline amino acid mixture.

    The effects of estradiol-17β-D-glucuronide (E2 17G) on bile excretion were studied in the rat and primary cultured rat hepatocytes.
    E2 17G induced an immediate, reversible and dose-dependent inhibition of bile flow.
    Electron microscopy revealed significant reduction of alkaline phosphatase activity on bile canaliculi during cholestasis without their ultrastructural alterations, and also showed no marked changes either of structure or permeability of tight junctions to Lanthanum.
    The radioactivity of intravenously-administrated 14C-taurocholate (14C-TC) increased immediately in the liver homogenate and then showed a rebound elevation in serum with E2 17G treatment.
    In primary cultured rat hepatocytes the uptake of E2 17G into hepatocytes demonstrated Michaelis-Menten kinetics and a mutual inhibition between E2 17G and TC was observed.
    These data suggest that inhibitions of both bile acid secretion into bile canaliculi and, in some extent, inhibition of bile acid uptake of into hepatocytes are major factors in E2 17G-induced cholestasis in the rat.

  • Possible model for intrahepatic cholestasis

    We studied bile retention biochemically and histologically by collecting blood and bile samples, and liver tissue using rat models at 6, 12, 18 and 24hrs after choledocho-caval fistula (CCF). Biochemical serum data in rats at 24hrs after preparation of CCF were similar to those of cholestasis, but the histological findings of liver tissue remainded in minimum changes. Bile flow reached in high levels by 6hrs after CCF. The bile volume correlated well to both biliary bile acid and bicarbonate excretions, but did not to bilirubin output in bile. Serum bile acid concentration increased rapidly than that of serum bilirubin after CCF, and was associated with elevation of biliary bile acid secretion. On the other hand, in rats at 24hrs after CCF serum bilirubin achieved in high level but bilirubin excretion rate in bild did not increase correspondingly. It seemed that CCF could contribute to experimental model of the biochemical cholestasis.

    We attempted to image tissue hemoglobin distribution by analyzing images obtained with an electronic peritoneoscope, using two narrow-band of 569nm and 650nm interference filters. At 569nm, variations in oxygen saturation have no effect, and at 650nm, hemoglobin concentration has no influence. The images of hemoglobin distribution obtained with the present system offer many useful advantages which are not available with conventional endoscopy. These are:
    1. The ability to produce two-dimensional hemoglobin distribution charts.
    2. The average hemoglobin concentration (hemoglobin index, Hb-I) can be measured within any region of interest (ROI).
    3. Measurement can be performed under physiological conditions because of non-contact method.
    4. High reproducibility is assessed because of the abscence of inter or intraindividual errors.
    5. Instantaneous and repetitious measurements are possible.
    Because of these advantages, this imaging method is considered the most excellent method of blood flow measure both in experimental and clinical research.

  • A study of fatty change in HCC

    In order to clarify the pathomorphologic significance of fatty change in hepatocellular carcinoma (HCC), 86 surgically resected HCCs smaller than 5cm in diameter were studied histologically. Among the 86 HCCs, fatty change was observed in 20 cases (23.3%), and was mainly found in welldifferentiated HCC. Fatty change was the most frequent (60.0%) in HCC ranged from 1.1 to 1.5cm in diameter. The incidence of fatty change decreased in accordance with the tumor diameter. According to the distribution pattern, fatty change was divided into diffuse type (6 cases) and focal type (14cases). Among the 25 HCCs larger than 3cm in diameter, fatty change of diffuse type was not seen. Fatty change of focal type was localized in a part or in the periphery of the tumor. Clear cell change of the cancer cells was also found in 13 (65.0%) of 20 HCCs with fatty change. Thus, fatty change was considered as one of pathomorphologic characteristics of minute HCC. It was predicted that fatty change of the tumor was caused by some metabolic disorder or circulatory disturbance which were localized in the tumor in the early stage.

    The in vitro lymphokine activated killer (LAK) activity of pheripheral blood lymphocytes (PBL) from 33 patients with hepatocellular carcinoma was investigated. The activity was diminished in 13 patients (<15% lysis) and was higher than 85% lysis in 7. A flowcytometrical analysis using PBL disclosed that the percentage of OKM1+, Leu7+·11b+, Leu7-·11b+ cells was decreased and the percentage of OKT8+, Leu7+·11b- cells was increased significantly in the 13 lower LAK activity patients, compared with the values of the 7 higher LAK activity patients. Furthermore, the response of PBL to interleukin 2 (IL2) was deficient in the lower activity group. However, between these groups, there was no significant difference in IL2 production by PBL. These findings indicate that diminished LAK activity in hepatoma patients is due to a decreased number of LAK cells and a defective response of LAK precursor cells to IL2.

    Hepatocarcinogenesis was studied by oral administration of synthetic estrogen and progestogen (Pill) in female rats of Wistar strain aged 4 weeks, with special interest to progression and biological characteristics of the developed hepatocellular carcinoma. Under 18-month daily administration of ethynyl estradiol of 0.15mg and norethindrone acetate of 12mg dissolved in 1.0ml of olive oil through the gastric tube, all rats developed hyperplastic nodules and 21.3% of them was proved to have hepatocellular carcinoma at the end of 12th month. Thereafter, the incidence of hepatocellular carcinoma rapidly increased, showing 74.5% at the end of 18th month. More than 75% of the liver was occupied by hepatocellular carcinoma in 77.4% of tumor-bearing rats at the end of 18th month. Doubling times of these carcinomas were 139.5±127.1 days, and DNA histograms revealed aneuploidy and polyploidy, showing the proof of malignant potential. From the study of allotransplantation of this carcinoma to the subcapsular space of kidney, hormone administration to recipient rats was required to establish the transplantation, demonstrating a highly hormone dependent tumor.

    It has been very difficult to manage hepatocellular carcinoma with esophageal varices. In this study, we reviewed 54 cases diagnosed hepatocellular carcinoma and esophageal varices almost simultaneously to discuss combination of treatments for the two disease.
    Treatments performed for hepatocellular carcinoma were partial hepatic resection (6 cases), TAE (15 cases), intraarterial infusion chemotherapy (hepatic artery, 7 cases), intraarterial infusion chemotherapy (abdominal aorta, 6 cases) and no treatment (16 cases). Treatments for esophageal varices were nonshunting operation (23 cases), endoscopic injection sclerotherapy (23 cases) and no treatment (8 cases). The overall one year survival rate and two were 61% and 41% respectively. Most important factors which determine the patients' prognosis were severity of hepatic dysfunction, clinical stage of hepatocellular carcinoma, degree of portal invasion and history of variceal bleeding. Combination of at least two treatments for both diseases is essential for better prognosis.

    We treated 138 patients with hepatocellular carcinoma (HCC) by combination therapy (LP-TAE) with transcatheter arterial embolization (TAE) and arterial administration of anticancer agents dissolved in Lipiodol (LP) for these 5 years. 212 patients with HCC were studied to compare LP-TAE and other therapeutic modalities in survival rate (SR). SR of LP-TAE was almost same level of hepatic resection in stage I-III and slight low in all stage. But, SR of LP-TAE was best therapeutic modality in no-resected and unresectable cases with HCC. Computed tomography after arterial administration of LP (LP-CT) contributed to staging of HCC and decision on effectiveness of LP-TAE. So LP-TAE was the leading role of treatment for HCC. Although, the cases with poor retention of LP by LP-CT were necessary to employ another effective therapeutic modality for HCC.

    We report a drug abuser whose HBsAg positive chronic hepatitis was once aggravated by superinfection with delta hepatitis virus but whose life was finally saved. The patient was a 22 years old man, and he had no history of overseas travel and of being a blood transfusion recipient. In June 1988, persistent malaise and nausea occurred, and he was admitted to a nearby hospital on 12th June. Abdominal pain and nocturnal insomnia continued, and he was transferred to our hospital on 15th June. On admission, the consciousness was clear, the levels of T-bilirubin, GOT and GPT were 11.6mg/dl, 1126u and 3709u. The prothrombin time was 30%, the result of the hepaplastin test showed a reduction to 13%, and the delta antibody was positive. The HBsAg was changed to negative during an acute stage then to positive 3 weeks later. These changes in the antigen seem to suggest that the delta and HBs antigens interacted with each other and suppress the proliferation of HB virus. The delta antigen is seen in nucleus of hepatocyte by indirect immunostaing counterstain with hematoxylin: This case is the first report of delta hepatitis with drug abuser in Japan.

    Two cases of non-alcoholic fatty cirrhosis were demonstrated. Both the laparoscopic and histologic findings were almost similar to those of alcoholic cirrhosis.
    Case 1 (male, 45 years-old): Biochemically, hypo-beta-lipoproteinemia and indirect hyperbilirubinemia were recongnised. Using 40g oil load test did not show the disturbance of fat absorption from the small intestine. Thus, it was thought that the hypo-beta-lipoproteinemia was idiopathic, having been related to fatty deposition in hepatocytes.
    Case 2 (female, 41 years-old): The patient was obese (obestic degree 24%), and had persistently abnormal hepatic tests and glucose intolerance, but the serum insulin level was high. The fatty deposition in the liver was considered to be due to obesity, glucose metabolic defect and hyperinsulinemia.
    The ratios of GOT/GPT, IgG/IgA and the measurement of ApoB seemed to be useful for the differentiation between alcoholic and non-alcoholic fatty cirrhosis.

    In a 54 year-old alcoholic male treated with Cyanamide, a solitary nodule with a diameter of 2.5cm was detected in the liver. The resected nodule was almost necrotic and accompanied by severe hematoidin deposition and a few goast cells. Mainly in the periportal region of the non-tumorous portion ground glass appearance was found but negative with Sikata's Orcein stain for HBsAg. It is wellknown that ground glass appearance in liver cells is often observed in patients treated by drugs for aversion of alcohol. But there has been no reports on concomitant tumors in those patients. It was a important case to discusse about causative relationships between alcohol and the drug and an occurrence of liver tumor.

    A 58 year-old male patient with ATL developed acute hepatic failure as the initial manifestation is reported. The patient was admitted to our hospital because of hepatic coma (grade V) and subcutaneous bleeding in March 1988. Total bilirubin in serum was 4.8mg/dl, GOT 2093 IU/L, GPT 814 IU/L and NH3 350μg/dl. The leukocyte count in peripheral blood was 43, 600/mm3, consisted of 41% abnormal lymphocytes showing convoluted or lobulated nuclei. Serum anti-ATLA antibody was positive and HBs antigen was also positive. By the therapy for acute hepatic failure and chemotherapy for ATL, he recovered from the coma on the 8th hospital day with a decrease of peripheral leukocyte count. However, he suffered from sepsis on the 36th hospital day and died on the 45th hospital day. The pathological examination of the liver in autopsy revealed post-hepatitic liver cirrhosis accompanied with the infiltraiton of the ATL-cells in the portal area.

    A 57-year-old female with nodular regenerative hyperplasia of the liver (NRHL) associated with Cruveilheir-Baumgarten (C-B) type porto-systemic shunt was reported. She had been suffering from repeated hepatic encephalopathy. she did not have any diseases or disorders commonly associated with NRHL except for diabetes mellitus. NRHL was diagnosed by laparoscopy and needle biopsy. Laparoscopy revealed non-nodular undulated liver surface, splenomegaly and C-B type collateral. Hepatic venography (retrograde portogram) revealed similar findings to idiopathic portal hypertension.
