    The serum level of transaminase activity have been widely used as a clinical marker of hepatic cell necrosis. Discrepancy is, however, frequently observed between the values of these enzyme activities and the degree of corresponding histological damage of hepatic cells. In this study, we examined the serum contents of β-glucuronidase, a lysosomal hydrolase, in terms of its usefullness as a new clinical indicator of hepatocellular destruction.
    The serum immunoreactive β-glucuronidase was determined by enzyme-linked immunosorbent assay using the and human liver β-glucuronidase antibody in patients with various liver diseases. Serum β-glucuronidase contents were significantly increased in patients with chronic active hepatitis, liver cirrhosis and primary hepatoma. The degree of histological hepatic cell damage were examined in 31 patients with chronic hepatitis. Significant positive correlation was found between the degree of hepatic cell necrosis and the β-glucuronidase contents, while no significant correlation was found between the cell damage and the transaminase activity.
    It was suggested that the measurement of serum immunoreactive β-glucuronidase would be useful clinical indicator for the evaluation of the degree of hepatic cell necrosis.

    Alterations of actin and myosin were observed by the effects of cytochalasin B (CB) and phalloidin (Ph) using primary cultured rat hepatocytes. The morphology of cells was observed by phase contrast microscopy and actin was observed by fluorescent microscopy using DACM-HMM. Myosin was observed by immunofluorescence techniques using anti-rat liver myosin antibody. In normal controls, after a 3hour incubation, actin and myosin were both intensely observed in the region along the plasma membrane, especially around the bile canaliculi. After a 24hour incubation, hepatocytes spread and actin and myosin were observed around the bile canaliculi, and appeared as filamentous patterns in the cytoplasm. In the CB group, hepatocytes did not spread even after 24hours, and actin and myosin were identified as granules around the widely dilated bile canalicli. Filamentous patterns of actin and myosin were not observed in the cytoplasm. In the Ph group, cytoplasmic vacuoles and protrusions from the cell membrane were observe. The hepatocytes did not spread even after a 24 hour incubation. Actin and myosin were identified as granules in the region along the plasma membrane, especially around the bile canaliculi. Present studies show that actin and myosin distribute almost to the same place in the hepatocytes and the integrity of each component of the cytoskeleton is important for normal cellular function.

    We studied the detection of cyclooxygenase and 5-lipoxygenase activity in rat hepatocytes. After separated hepatocytes were sonicated and centrifuged, the supernatants were used as enzyme solution which possibly contained cyclooxygenase or 5-lipoxygenase. Radiolabeled 14C-arachidonic acid were added to this enzyme solution, and then, synthesized 14C-prostaglandins E2, F or 14C-5-HETE, 5, 12-diHETE were measured after purification by reversed-phase high pressure liquid chromatoglaphy. As a result, this enzyme solution produced prostagland in or HETE from exogeneous arachidonic acid, which indicated the existence of cyclooxygenase or 5-lipoxygenase activity in rat hepatocytes.
    In this experiment, we showed that rat hepatocytes have the ability of not only metabolism and inactivation, but also production, of arachidonic acid metabolites.

    Ito cells (Fat-storing cells) were isolated and cultured. Cytoskeletons, i.e. actin, myosin and desmin, were observed during the course of culture. Ito cells spread on the culture dish, and actin became filamentary, as the stress fiber. Myosin also showed filamentous patterns similar to the stress fiber, and some revealed stripes. Desmin became filamentary and a network was observed in the cytoplasm. When cytochalasin B was added to culture medium, filaments of actin and myosin were transformed into short rods or granules. These findings suggest that Ito cells, during culture, transform in a similar way to fibrogenic cells (fibroblasts), and also maintain characteristics resembling myogenic cells (myoblasts), and that Ito cells are the precursor cells of myofibroblasts.

    In 101 alcoholic patients performed serial liver biopsies at the intervals of more than six months, histological changes of the liver were examined and the factors that affected those changes were studied.
    In patients who stopped drinking (40 cases), histological improvement was seen in 45.7% and the histologically progressed cases (9 cases) had significantly high incidence of the history of blood transfusion, drug addict and/or viral hepatitis.
    In patients who continued drinking (61 cases), histological deterioration was seen in patients with the history of longer and larger amount of alcohol consumption.
    In patients who showed the histology of chronic hepatitis, the abstained case with inactive chronic hepatitis was improved to hepatic fibrosis, but the case with active chronic hepatitis developed to liver cirrhosis at the high incidence.
    In patients with serum antibody to alcohol altered hepatocyte plasma membrane, the histological deterioration after alcohol consumption was observed in a short period and higher incidence compared with patients with no antibody.
    Therefore, mode of alcohol intake, infection of NANB hepatitis virus and association with the antibody to alcohol altered hepatocyte plasma membrane play an important role in the histological development of alcoholic liver disease.

    We examined the expression of E type cadherin, which was a Ca2+-dependent cell-cell adhesion molecule, in hepatocellular carcinoma (HCC) tissues using immunohistological ABC method. Mouse monoclonal antibody against human E-cadherin was used as the first antibody. In all the tissues of HCCs and non-cancerous portions examined, both hepatoma cells and hepatocytes were positive for E-cadherin. Positive staining was located at cell membrane, especially at the intercellular contact sites. However, the difference of staining intensity was noted in each hepatoma cell. PLC/PRF/5 cell lines were also positive for E-cadherin staining. Although, all of normal gastric mucosa tissues showed positive staining for E-cadherin, the positive staining was 68% in gastric cancer tissues. It was less frequent than that in hepatoma cells (p<0.05). These results suggest that the expression of E-cadherin might play an important role for the characteristic growth of hepatoma cells.

    Female rats of the Wistar strain, aged 4 weeks, had been daily given a dose of 0.075mg of ethynylestradiol and 6mg of norethindrone acetate (Pill) dissolved in 0.5ml of olive oil through a gastric tube for one year. The levels of total, unoccupied and occupied estrogen receptors of cytosol and nuclei in the liver during carcinogenesis were measured by the exchange assays, and the occupied and total estrogen receptors were confirmed immunocytochemically by using anti-estradiol antibody and anti-estrogen receptor antibody, respectively. The levels of occupied cytosol estrogen receptor were higher in Pill administrated rats than in those with olive oil alone. The levels of occupied nuclear estrogen receptor in the liver were gradually increased during the course of Pill administration, with the development and growth of hyperplastic nodules, especially this levels were extremely high in the area of hepatocellular carcinoma, and these changes were also proved by the immunocytochemical assays. These results suggest that oral contraceptives initiate hepatocarcinogenesis by increasing the estrogen-estrogen receptor complex in the nucleus, directly acting on DNA.

    Adoptive immunotherapy was performed administrating autologous lymphokine-activated killer cell via hepatic artery for 7 cases with primary hepatocellular carcinoma. Both of the natural killer activity and the lymphokine-activated killer activity in the peripheral blood lymphocyte were significantly increased after the treatment. A reduction of the tumor size was recognized in 2 cases out of the 7 cases after the treatment and, also a decrease in the serum alpha-feto protein level was found in all cases who showed higher level before the treatment.

    Tumor infiltrating lymphacytes (TIL) isolated from human hepatocellular carcinoma (H.C.C.) were cultured. Expansion and proliferation of the tumor infiltrating lymphocytes were investigated following addition of interleukin-2 (IL-2) into the cultured media and the antitumor activities were evaluated. TILs were separated from tumors of 15 patients with H.C.C. by an enzyme digesting method. The number of lymphocytes obtained from the tumor was 1.35±0.33×107(mean±SE) with a range 5×105 to 5×107 per gram of tumor tissue. The majority of TIL were T lymphocytes (CD3+). Lymphocytes were cultured in a fresh media containing IL-2 (2000JU/ml). By 10 days after initiation of culture, the mumber of lymphocytes increased with elimination of the tumor cells. Higher levels of cytolytic activity have continued during the culture period ranging from 2 to 4 weeks. The results indicate that TIL from H.C.C. could gain expansion and proliferation to provide high levels of antitumor effect with rIL-2.

    This study was aimed to evaluate the hepatic hemodynamics after portal vein ligation (PVL). Terminal portal venous (TPV) and central venous (CV) pressures measured by a micropuncture method and the relationships among the prssures, the hepatic blood flow (HBF) and the formation of collateral veins to the liver were studied in the rat model with porta-systemic shunt. Although the TPV pressure, the TPV-CV pressure differences, and the HBF were decreased immediately after PVL, the TPV pressure and the TPV-CV pressure differences were normalized after 4 weeks of PVL and the HBF was returned to the prevalues 3 days after PVL. Portogram showed that collateral veins to the liver were found to be created after 1 week of PVL, and that they were remarkably developed after 4 weeks with clearly shown intrahepatic portal veins. Because the improvement of the TPV pressure and the TPV-CV pressure differences were well correlated with the formation of collateral veins to the liver, the results suggest that the hepatic arterial blood flow was compensationally increased after 3 days of PVL and that the portal venous blood flow was recovered accompanying with the formation of the collateral veins to the liver.

    A 43-year-old man with non-alcoholic steatohepatitis was reported. He had no history of heavy drinking and showed liver dysfunction compatible with fatty liver. The laboratory data turned for the worse and he received tiopronin at a practitioner. Two months later sever intrahepatic cholestasis was appeard and continued for four months. Laparoscopy and liver biopsy were done and the liver histology showed centriloblar inflammation, fibrosis and Mallory bodies predominantly in zone 3. This findings were compatible to the histology of alcoholic hepatitis. We diagnosed this case as non-alcoholic steatohepatitis associated with tiopronin induced liver injury.

    A 49-year-old female case of autoimmune hepatitis associated with mixed connective tissue disease and Sjögren syndrome was reported. She was admitted to our hosgital because of abnormal liver test with camplaints of arthralgia, Raynaud phenomenon and general malaise. Histology of the biopsied-liver tissue revealed chronic inactive hepatitis. After one-year follow up, S-GOT and γ-globulin elevated up to 507IU and 3.2g/dl, respectively. Polyarthritis, puffiness of the dorsal aspect of bilateral hands, leucocytopenia and dryness af the nnouth were evident. Anti-RNP antibody, antinuclear antibody and anti-smooth muscle antibody were pasitive. The second biopsy of the liver revealed chronic active hepatitis with rossette formation of hepatocytes. Sialography showed the apple-tree lesian, and both Shirmer test and Rose-bengal test were positive. The level of serum transaminase and her symptom remarkably improved after steroid therapy. To our knowledge, this is the first report on association between autoimmune hepatitis, mixed connective tissue disease and Sjögren syndrome.

    We report an autopsy case of hepatocellular carcinoma (HCC) that showed invasive growth into the stroma among pseudolobules. The patient, 72-yr-old man, had been treated by transcathether arterial embolization (TAE) for HCC. At autopsy, six HCC nodules (<2cm in diameter) were found. In two HCC nodules, tumor cells were necrotic, but a few viable tumor cells were found within the cirrhotic stroma adjacent to the necrotic zone. In the remaining four HCC nodules, tumor cells were viable and invading into the pre-existing cirrhotic stroma and portal tracts. These intrastromal tumor cells occasionally contacted with blood vessels, and sometimes invaded into the vascular lumina. Despite TAE, tumor cells in the stroma remained viable, suggesting that they were nourished by portal blood. It was suggested that stromal invasion of small HCC pose problems regarding tumor growth, intrahepatic metastasis and treatment.

    We have reported a rare case of multiple intrahepatic portal-hepatic venous shunts, who developed hepatic coma as the first clinical manifestation.
    A 60-year-old female was entered to our hospital because of recurrent coma attacks accompanied with hyperammonemia and triphasic wave on EEG. Laboratory data were almost normal except for hyperammonemia and retardation of ICG, and marked increase of heart/liver ratio was found on 201Tl perrectal scintigraphy. Imaging studies using ultrasonography, CT during arterial portography (CT-AP) and MRI suggested multiple intrahepatic portal-hepatic venous shunts, and hepatic venography demonstrated a total of eight macroscopic shunts retrogradely. There was no finding of chronic liver disease on peritoneoscopy or histopathological study of liver specimen. Administration of lactulose and branched chain amino acids has improved serum ammonium level, and she has been completely free of hepatic encephalopathy after medication.

    A 39-year-old female with right hypochondralgia was diagnosed as having multiple, irregular shaped giant cavernous hemangiomas involving both hepatic lobes and underwent surgery, leaving only the anterior segment of the right hepatic lobe. Regeneration of the residual liver was satisfactory and postoperative course was uneventful. Although cavernous hemangioma of the liver is a benign disease, it is documented that the disease may rarely become progressive and eventuate in spontaneous ruputure or the occurrence of Kasabach-Merritt syndrome. The disease appears therefore to require regular monitoring from the outset and surgical treatment should be considered, as in malignancies, whenever subjective symptoms become manifest or hemangioma is noted to grow.
