    Pre-S2 antigen and anti-pre-S2 antibody were tested in 56 sera of 6 patients with acute hepatitis type B and 867 sera of 265 cases with chronic HBV infection.
    Pre-S2 antigen could be detected in almost all sera HBsAg titer more than 24 by RPHA.
    On the other hand, anti-pre-S2 antibody could be detected in 4 of 6 patients with acute hepatitis type B 2 or 3 weeks before the clearance of serum HBsAg.
    Anti-pre-S2 antibody could be detected in 85 of 238 cases with chronic HBV infection, and the titer of HBsAg and DNA-polymerase activity in anti-pre-S2 positive patients were statistically lower than those in anti-pre-S2 negative patients.
    Serially long-term follow-up study in 15 of 27 chronic hepatitis patients demonstrated that detection of anti-pre-S2 antibody was divided into three patterns; persistent positive, fluctuating and one-point positive. Although only 2 out of 7 patients with persisting high level HBe antigen were positive for anti-pre-S2, it could be detected in 7 out of 10 cases recognized spontaneous disappearance of HBeAg. Detection pattern in 5 of those 7 patients showed one-point pattern just before the clearance of serum HBeAg. These results may suggest the correlation between appearance of anti-pre-S2 and the clearance of HBeAg.

    In order to clarify the pathogenic role of interleukin 2 (IL2) and IL2 receptor (IL2R) system in HBV carrier state, expression of IL2 and IL2R gene were investigated using Northern blot hybridization in cultured peripheral blood mononuclear cells stimulated with phytohemagglutinin (PHA) and 12-O-tetradecanoyl-phorbol-13-acetate (TPA). The amounts of IL2 mRNA were found to be decreased in 8 out of 15 patients with type B chronic active hepatitis (CAH) and in 6 out of 8 asymptomatic HBV carriers (ASC). The expression of IL2R gene was also decreased in a part of patients with decreased IL2 mRNA levels, while mRNA levels of c-myc gene were almost the same in all patients. Furthermore, IL2 concentration in culture supernatant of peripheral blood mononuclear cells from patients with type B CAH was significantly lower than that from normal subjects, suggesting that abnormality of IL2 and IL2R gene expression was related to HBV carrier state.

    The mechanism of hepatocyte dysfunction induced by endotoxin was investigated using a coculture system of hepatocytes and Kupffer cells from the view point of the role of Kupffer cell.
    During hepatocyte protein synthesis measurement, it was shown that endotoxin was capable of decreasing in the incorporation of 3H-leucine when the hepatocytes were cocultured with Kupffer cells.
    α-tocopherol, anti-tumor necrosing factor serumn (anti TNF), diltiazem and dexamethazone were respectively incapable of blocking the decrease, but superoxide dismutase, partially blocked the decrease in hepatocyte protein synthesis.
    The results indicated that the secretions raleased by Kupffer cells such as oxygen radicals excluding TNF, IL-I, arachnoid acid metabolite and protease, were assumed to be important in causing hepatocyte dysfunction induced by endotoxin.

    To evaluate the speed of growth and change of sonographic features of the small hyperechoic hepatocellular carcinoma (HCC), we analyzed sonographically 18 small hyperchoic nodules detected in 16 patients with chronic liver disease, which were followed without treatment for more than 3 months. Seven nodules in 6 patients were HCC, and those nodules showed enlargement of diameter and change of internal echogenecity showing "Bright loop" pattern. Five cases were slow-growing HCC, in whose doubling time was more than 140 days. Intrahepatic metastasis occured in 5 of 6 cases during follow-up period. Most of these cases were hypovascular HCC, and aimed tissue-core biopsy was indispensable to establish early diagnosis.
    Histological examination showed that most of cases were well differentiated HCC and the hyperchoic area showed fatty metamorphosis of the liver cells.

    Seventy nine small liver nodules, less than 2cm in diameter, rescted for the clinical diagnosis of HCC are reported in detail.
    About half of the nodules showed obscure margin tumor macroscopically. The other half were distinct margin nodules. At the margin of the obscure nodule, cancer cells showed replacing growth, and histological atypia of cancer cells was so mild that it was very difficult to differentiate cancer cells from surrounding hepatocytes. However, these cancer cells invaded portal tracts frequently. On the other hand, the distinct margin nodules showed a distinct cellular atypia like that seen in the large advanced HCC. Around the center of the obscure nodular tumors, atypical cells appeared forming small nodules or scattered randomly. As the central atypical cells proliferated, pre-existing well differentiated cancer cells that surrounded them were compressed outward to the edge of the nodule. The other type of obscure nodule showed expanding growth as a whole, and compressed surrounding hepatocytes with capsule formation resulting in a distinct margin like the advanced HCC.
    These findings suggests that the obscure margin nodules represent early stage of HCC, and the distinct margin nodules represent advanced HCC, even if they are small.

    The chromosomal site of the c-myc proto-oncogene was identified in a woodchuck hepatocellular carcinoma-derived cell line (WH257GE10) by the in situ hybridization technique using a 3H-labelled woodchuck c-myc gene (8.0kb Hind III fragment) as a probe to investigate the relationship between woodchuck hepatitis virus (WHV) DNA integration and the c-myc proto-oncogene. The c-myc protooncogene was located at 32 region of the long arm of chromosome 8 (8q32): in the vicinity of one of WHV DNA integration sites (8q31). These findings suggest that the c-myc gene assigned closed to WHV DNA integration site may play some role in the pathogenesis of woodchuck hepatocellular carcinoma.

    The separation and primary culture of carbon tetrachrolide-induced cirrhotic rat hepatocytes was studied. The cirrhotic liver was perfused with 500ml of 0.25% collagenase solution for 30 minutes. The viability of the separated hepatocytes was 83.2±6.0% (mean±SD) and 0.84±0.28×107 viable hepatocytes/g liver weight were obtained (mean±SD). By using culture dishes coated with collagen and fibronectin, the hepatocytes could be cultured for 3 days maintaining 75% cell viability. Hepatocytes cultured for 24 hours maintained the same morphological characteristics as at separation. It was considered that primary culture of cirrhotic rat hepatocytes is useful for study of the biochemical characteristics of the hepatocytes in cirrhosis.

    The authors describe a case which suggested that partial nodular transformation (PNT) of the liver is identical to idiopathic portal hypertension. A 55-year-old female patient developed splenomegaly and portal hypertension, and died of rupture of esophageal varices. PNT was diagnosed based on the presence of coarsely nodular hyperplastic changes at the perihilar regions of the liver. The histopathology of the liver was similar to that of idiopathic portal hypertension in the following findings: subcapsular parenchymal atrophy, circular fibrous enlargement of the portal tract, collapse and narrowing of the portal vein, and systemic portal sclerosis of the middle and large portal vein. Histometrical study of the liver also showed morphological similarities between the two conditions. The pathogenesis of nodular changes was unknown, however, the collapse and narrowing of the peripheral portal vein was observed both in the subcapsular and perihilar regions, while the degree of the patency of the hepatic artery was almost same as that of the normal controls. It was suggested that compensatory hyperplasia of the hepatocytes for the subcapsular atrophy might focally develop in the hillar regions to form nodular changes. Maintained hepatic arterial flow in the hillar regions might contribute to the genesis of the parenchymal hyperplasia.

    A 58-year-old male with liver cirrhosis was pointed out hypoechoic SOL in the liver by ultrasonography. By echo-guided tumor biopsy, this SOL was diagnosed as HCC, and was resected.
    Tumor measuring 4.0×5.5mm revealed well-differenciated HCC (Edmondson's grade I-II) with replacing pattern of growth and fatty change histologically.
    In spite of the fact, hepatic angiography showed a definite tumor staining.
    It is very important to investigate the cause of this discrepancy for early detection of small HCC.

    To examine the mechanism of elevated PIVKA II production in patients with hepatocellular carcinoma (HCC), the present clinical study was conducted. The subjects were two HCC patients showing elevated plasma PIVKA II levels. The patients were intravenously administered with vitamin K (menaquinone 4, 30mg) everyday for 4 to 7 weeks, respectively. The chronic administration of vitamin K resulted in a significant decrease in plasma PIVKA II levels, but not down to the normal range (less than 0.1AU/ml). In addition, the levels of vitamin K in the livers of the two patients were determined post-mortem. Vitamin K levels in the liver were found to be increased by 7-and 11-fold (in the cancerous portion) and 11-and 19-fold (non-cancerous portion) in respective patients, compared with the values for HCC patients who received no vitamin K treatment. These results suggested that vitamin K defficiency in the cancerous tissue could not necessarily explain the increased production of PIVKA II in HCC patients. We rather speculated that there is a defect(s) in the hepatic γ-glutamyl carboxylation system which requires vitamin K as the coenzyme in HCC patients.

  • Consideration in the differential diagnosis with well-differentiated hepatocellular carcinoma

    A 33-year-old female patient underwent extended right lobectomy for a large tumor in the liver. Histologic examination of the resected specimen disclosed obvious differentiation toward hepatocytes in tumor cells and scanty atypia in nuclei. Liver cirrhosis was not combined. These findings were compatible with those of liver cell adenoma. Although differention between liver cell adenoma and well-differentiated hepatocellular carcinoma (Edmondson grade I) is deemed difficult, it is our opinion that the diagnosis of liver cell adenoma can be made by following characteristics: 1) incidence in young adult 2) no underlying liver disease 3) large tumor in the majority of cases 4) histologic findings of monotonous growth and scanty atypia. However, it is not unequivocally proven that liver cell adenoma is really a benign neoplasm. Further follow-up study is necessary to confirm its biological property.

    A 60-year-old woman with a primary leiomyosarcoma of the liver growing ectra-hepatically is reported. Review of the literature reveals only twenty previous reports.
    After she was admitted on our hospital, it was a huge polycystic tumor, extra-hepatically growing, which procedures some specific findings with ultrasonography and computed tomography. There was partially septum and mural nodule in the cystic tumor. The resected tumor was about 27×24×8cm respectively, and about 3820g. The fluid was serous-bloody and/or coffee-like.
    Histological feature of the resected tumor demonstrated the interacing bands of spindle-shaped cells with blunt-ended and elongated nuclei. We diagnosed the primary leiomyosarcoma of the liver. This is the primary leiomyosarcoma with the specific growth type which has never been reported, and it was suggested that we could research the origin of the leiomyosarcoma.

    A 62 years old man, previously worked ina coal mine for 18 years, was a dimitted due to general fatigue. On admission a chest X-ray film, blood gas analysis, and respiratory function test showed an interstitial imparment of the lung. Histological examination for the biopsied liver showed alcoholic liver fibrosis and revealed the brown pigment deposition in portal macrophages and Kupffer cells. Electron microscopy demonstrated electron-dense materials in enlarged macrophages and Kupffer cells with reduced cystoplasmic processes. An energy dispersive X-ray analyzer demonstrated that electron dense materials in macrophages and Kupffer cells contained a significatn amount of silicium and aluminium. These findings indicated that the present case presented a very rare deposition of coal pigment in the reticuloendothelial system of the liver.
