    Molecular form of IgM antibody to hepatitis B core antigen (IgM anti-HBc) was analysed by high speed liquid chromatography and enzyme-immunoassay in sera from patients with actue and chronic hepatitis B virus infection
    IgM anti-HBc was detected in only 19S IgM with both initial stage and recovery stage of acute hepatitis, but it was detected in not only 19S IgM but also 7S IgM with chronic hepatitis.
    Furthermore, 19S IgM was predominant in acute exacerbation phase and conversely 7S IgM was predominant in inactive phase of chronic hepatitis.
    These findings suggest that the main molecular form of IgM anti-HBc was changed from 7S IgM to 19S IgM with acute exacerbation of chronic hepatitis.

    Sera from HBV infected patients were tested for the presence of antibodies to HBV X-protein by Western blotting using recombinant-derived protein, and the expression of X-related protein in the liver of those patients was examined by immunohistochemical method using antibody to synthetic X protein peptide. Antibodies to X-protein were detected in 3 out of 100 sera of patients. In those three positive sera, one from an asymptomatic HBV carrier and another from a patient with chronic hepatitis were strongly positive, and the remainder serum from a patient with hepatocellular carcinoma was weakly positive. The patient with chronic hepatitis positive for antibody to X-protein was treated with interferon. Although fluctuations of transaminase levels and decrease of HBV DNA polymerase activity were observed in the patient described above, there was no significant change in titer of antibody to X-protein. In the immunohistochemical examination, X-related protein, which was observed in the cytoplasm of hepatocytes and was distributed in sporadic or focal pattern, was observed in 11 out of 41 patients. There was no statistically significant relationship between the presence of X-related protein and the HBeAg/anti-HBe system. However, two patients positive for antibodies to X-protein in sera were also positive for X-related protein in the liver.

    Serum HCV antibody titers in patients with non A non B (NANB) liver diseases were measured by ELISA assay which used chemically-synthesized peptide, 42 amino acid sequence, from the C5-1-1 region of HCV gene. Some of them were compared with those of C-100 antibodies (Chiron Co. Ltd.). Positive rate of serum HCV antibody was 69.3% in NANB chronic hepatitis, 68.5% in chirrosis, 74.4% in hepatic carcinoma, and 20.0% in acute hepatitis. In comparisen with that of C-100 antibodies, the positive rate in this method was low by about 5-10%. It is unknown at percent whether these results are attributable to a pseudpositive response of C-100 antibody or to a low sensitivity of detection in this method. However, this method yielded a high percentage of positive response in NANB liver disease, and seemed useful for the clinical diagnosis of hepatitis C.

    Fourteen patients with type non-A, non-B fulminant hepatitis (FH) and related diseases (in one patient accompanying type A and one patient accompanying type B respectively) were treated with interferon (IFN) in combination with or without an artificial liver support (ALS) we have originally developed. Eight patients showed histologically definite liver regeneration, and seven survived. Including 8 survivors out of 10 patients mainly with type B who were treated with the ALS alone, 15 patients survived in recent four years (survival rate 62.5%).
    Serial determination of 2'-5' oligoadenylate synthetase (2-5 AS) activity was performed in 12 patients undergoing IFN therapy and 6 survivors not undergoing IFN therapy. Maximum 2-5 AS in patients survived after IFN therapy was 9.33±8.88 times higher than that before the treatment, and was 3.52±1.26 times higher than that at the onset of coma in survivors without IFN therapy. While, it was only 1.73±0.50 times higher in those who died after IFN therapy.
    Thus, the responsiveness of 2-5 AS is indicative of prognosis in fulminant viral hepatitis no matter if patients receive IFN therapy or not.

    In an attempt to elucidate one of possible mechanisms of multi-organ failure (MOF) in acute hepatic failure (AHF), we investigated in vitro effects of circulating substances, which levels elevated in AHF, on the Na+, K+-pump function in rat brain and kidney as well as human leukocyte. Bile acids [CDCA, GCDCA, DCA], short-chain fatty acids (SCFA) [iC5, nC6 and C8], endotoxin (LPS), and α-palmitoyl lysolecithin showed inhibitory effects on the rat brain and kidney Na+, K+-ATPase activities in a dose-dependent manner. At lower concentrations, inhibitory effects of DCA, C8 and LPS were observed to be synergistic. In addition, CDCA and C8 also inhibited human leukocyte Na+, K+-pump activity. In summary, bile acids and SCFA were shown to be potent Na+, K+-pump inhibitors and suggested to play an important role, at least in part, in the pathogenesis of MOF (particularly brain edema, renal failure and infection) in AHF, by presumably damaging the sodium pumping of cell membranes in these organs. A PGI2 analog, OP-2507, recovered the leukocyte Na+, K+-pump activity reduced by C8, possibly by exerting a membrane stabilizing action.

    Phenacetin breath test (PBT) using 13C-Phenacetin was performed to test its sensitivity in evaluating cirrhotics' liver function, comparing to Aminopyrine breath test (ABT). Percent dose of 13CO2 expired per min. (13CO2%dose/min.) were significantly lower (p<0.001) in cirrhotics up to 90min. 13CO2%doses/min. of PBT were larger from 30 to 120min. than those of ABT when compared incontrol. They were also larger from 60 to 240min. than those of ABT in cirrhotics. Cumulative % doses expired (Cum % dose) in PBT of cirrhotics were significantly smaller from 30 to 240min. than those of control group. The values were significantly larger from 120 to 240min. than those of ABT. The mean residence time of phenacetin in cirrhotics were longer than that in controls, while that of aminopyrine was the same in cirrhotics and controls. Better correlations were obtained between PBT and liver function tests which indicate functioning liver cell mass than ABT.

    To investigate the pathophysiology of prolonged jaundice after biliary decompression for obstructive jaundice, choledochoduodenostomy with or without 70% hepatectomy was performed one to four weeks after common bile duct ligation and cholecystectomy in dogs with or without cholangitis. Prolonged jaundice was associated with a longer period of jaundice, with cholangitis, with slowly decreasing conjugated bilirubin levels and with a higher delta bilirubin/direct bilirubin ratio in the serum. Among dogs with prolonged jaundice after biliary decompression alone, the rate of decrease of serum conjugated bilirubin was significantly better in long survivors than in dogs dying early. After biliary decompression with hepatectomy, a better prognosis was significantly related to the decline of indirect and conjugated bilirubin levels in the serum. Thus, measurement of serum bilirubin fractions appears useful in estimating prolonged jaundice.

    We recently found that a specific inhibitor of leukotrienes protected from the experimentally induced liver injury, and that the leukotriene C4 directly injured the liver cellsin vitro. It is thought that leukotrienes may play an important role in liver injuries.
    We studied about the leukotriene B4 receptor and the leukotriene C4 receptor of isolated rat hepatocytes, in order to analyze the mechanisms of the liver cell injury induced by leukotrienes. As a result, there is no leukotriene B4 receptor, but there is the specific leukotriene C4 receptor in the rat hepatocyte. This result indicates that the leukotriene C4 has some functions against hepatocytes through the binding with specific receptors of hepatocytes.

    Two patients with anti-HBe positive chronic hepatitis B developed severe hepatitis. Their sera tested positive for DNA polymerase (DNA-P), however, negative for HBV DNA by dot blot hybridization technique. To understand the mechanism of "acute on chronic" hepatitis, we analyzed HBV DNA by Polymerase Chain Reaction (PCR) method, products of DNA-P by agarose gel electrophoresis, and presence of HBc antigen by histochemical assay for liver tissues. HBcAg was not detected in the liver tissues, but HBV DNA tested positive for both sera by PCR. Thus, the quantity of HBV DNA was considered a little. The products of DNA-P demonstrated smear by the gel electrophoresis. Therefore, we concluded that the elevation of DNA-P activity did not come from HBV DNA-P. One of the two cases showed positive for HCV antibody. Understanding of DNA-P assay should be careful for these patients.

    Two cases of hepatocellular carcinoma (HCC) with intrahepatic bile duct tumor were reported. We didn't observe obstructive jaundice and upper abdominal pain due to the invasion to the bile duct. Imaging diagnosis by ultrasonography and X-ray computed tomography revealed the tumor growth in the bile duct in both cases and the portal vein tumor thrombus in one of them. The main tumors were less than 3cm in diameter. HCC with bile duct tumor growth often presents obstructive jaundice as icteric type hepatoma. It was considerd that a small main tumor and a localized tumor growth in the intrahepatic bile duct were the reason for having no symptoms due to the invasion to the bile duct.

    A 65-year-old man had suffered from abdominal fullness and a gluteal and femoral pain on the left side. On imaging diagnosis, we found gallbladder with irregular wall and a stone, metastatic multiple liver tumors and a big pelvic tumor. Serum AFP and CEA were 6200ng/ml and 204ng/ml, respectively. The diagnosis of primary origin wasn't made in his life.
    A summary of autopsy findings is as follows: 1) The papillary adenocarcinoma was found in gallbladder. 2) Histologically the gallbladder tumor was mixed well and poorly differenciated adenocarcinoma. 3) The histology of the hepatic and pelvic tumors are very similar to the component of poorly differenciated adenocarcinoma of gallbladder. And then, hepatic and pelvic tumors are regarded as metastatic tumors of gallbladder carcinoma. On pathologic examination, both AFP and CEA stains weren't located in the tumors of gallbladder, liver and pelvis and the tissues around each tumor.

    Plasma levels of α-human atrial natriuretic peptide (α-hANP), renin activity and aldosterone, central venous pressure and urine volume were measured before and after the peritoneovenous shunt implantation in 3 cases of decompensated cirrhotic patients with intractable ascites.
    After the shunt implantation, ascites decreased immediately in amount. Plasma levels of α-hANP in all cases before the implantation were above the upper normal limit. The α-hANP levels increased significantly as compared with the preoperative levels in the early stage of post-implantation. However, the α-hANP levels showed to be lower than the preoperative levels in the late stage of the post-implantation. These data suggest that the elevation of plasma α-hANP levels in cirrhotic patients with intractable ascites are related to the decrease of renal blood flow and increased reabsorption of sodium and water.

    We reported a rare case of Wilson disease with multiple low-echoic SOLs which were diagnosed regenerative nodules by meanes of DSCTI-AP and tumor biopsy. A 22-year-old male had been treated with D-penicillamine due to Wilson disease from May 1985. In June 1987, multiple low-ecboic SOLs in the liver were found, and he was admitted to our hospital for the working up for the SOLs. CT scan did not revealed SOLs in the liver, and celiac arteriography showed neither neovascularization nor tumor stain. But DSCTI-AP showed multiple low density areas in the liver, indicated partial disturbance of portal blood flow. From these results, SOLs were suspected regenerative nodules. Tumor biopsy was performed for diagnosis, and malignant cells were not seen in the SOLs, and we diagnosed the SOLs to regenerative nodules.
